Application of whole-tumor histogram analysis of enhanced MRI for posterior fossa tumors in children

Objective: To study the value of whole tumors enhanced MRI gray histogram analysis of differential diagnosis in three common pediatric posterior fossa tumors(ependymoma, astrocytoma, medulloblastoma). Methods: A retrospective analysis was conducted by brain MRI examination and pathology diagnosis of 76 cases of posterior fossa tumors in children in our hospital. Among them, there were 25 cases of ependymoma, 26 cases of astrocytoma, 25 cases of medulloblastoma. Respectively, we drew the region of interest(ROI) in the enhanced MR sagittal images of three groups on each layer of tumor level by using Mazda software and analyzed the whole tumors gray histogram, then performed statistical analysis on the three sets of parameters obtained from histograms to find out statistical difference of each parameter. Results: Through histogram analysis of 9 parameters, the difference of these 6 parameters were statistically significant(all P<0.05), including mean, variance, skewness, Perc.10%, Perc.50% and Perc.99%, the remaining 3 parameters, including kurtosis, Perc.01%, Perc.90% had no significant difference(all P>0.05). Conclusion: The MRI gray histogram analysis based on whole tumors is helpful for the identification of three kinds of pediatric posterior fossa tumors,the Perc.50% and variance had a high diagnostic efficiency.

Effect of DRD4 receptor-616 C/G polymorphism on cerebral oxygen metabolism in pediatric primary nocturnal enuresis patients, an MR study using TRUST technique

Objective: To determine whether the 616 C/G single nucleotide polymorphism(SNP) in the dopamine D4 receptor(DRD4) promoter rs747302 was associated with abnormal whole-brain cerebral metabolic rate of oxygen(CMRO2), cerebral blood flow(CBF) and oxygen extraction fraction(OEF) in children that suffer from primary nocturnal enuresis(PNE) using T2-relaxation-under-spin-tagging(TRUST) methodology. Methods: Genomic and TRUST imaging data were obtained from 56 PNE pediatric patients and 58 healthy controls. SNP of rs747302 was genotyped. TRUST imaging data were collected on a 3.0-Tesla MR scanner, OEF and CMRO2 data were calculated using an in-house package. OEF and CMRO2 were compared between different group(PNE/control) and genotype(C/G) using two-way ANCOVA analysis. Results: Group-genotype interactions were found in OEF and CMRO2, post-hoc testing demonstrated that the C-allele carriers in the PNE group had a significantly increased OEF and CMRO2. Conclusion: Among PNE children, C-allele carriers of rs747302 are associated with abnormal OEF and CMRO2.

CT and MR imaging diagnosis of primary nasal cavity and paranasal sinuses mucosal malignant melanoma

Objective: To evaluate the diagnostic value of CT and MR in the diagnosis of primary nasal sinus mucosal malignant melanoma. Materials and Methods: The images, pathology and clinical data of 11 cases of primary nasal sinus mucosal malignant melanoma confirmed by biopsy or pathology were retrospectively analyzed. Eleven patients underwent CT plain scan, and 5 underwent CT contrast enhancement. Three of them underwent through both MR scan and enhanced scan. The study examined the CT and MRI findings in the following aspects: the lesion location, size, border, the relationship with the surrounding tissue, and the lymph nodes. Results: In 3 cases, the tumor was located in the nasal cavity, 6 cases in both the sinus and the nasal cavity, and 2 cases in the sinus. The CT findings showed that all the tumors were irregularly shaped, with soft tissue density, but without calcification or cystic degeneration. Severe enhancement was observed in 2 cases, moderate enhancement in 3 cases. Among the 3 cases that underwent MR examination, 1 case was hyperintense on T1WI, and hypointense on T2WI. The other 2 cases were isointense on T1WI, T2WI. The mass showed mild enhancement. All the tumors invaded the surrounding bone tissue. Immunohistochemistry showed that the tumor cells were positive for HMB-45, S-100 and Vimentin. Conclusion: Primary nasal sinus mucosal malignant melanoma is not specifically manifested on CT imaging. But typical malignant melanoma has typical manifestation on MR. CT combined with MR can accurately show the location of the tumor, and the invasive relationship between the lesion and the surrounding tissues.

Correlation between stability of carotid plaque evaluated by ultrasound and the level of plasma Lp-PLA2

Objective: To evaluate the correlation between the stability of carotid plaques evaluated by ultrasound and the level of plasma lipoprotein-associated phospholipase A2(Lp-PLA2). Methods: One hundred and eighty-five patients with carotid plaque were divided into stable plaque group(97 cases) and unstable plaque group(88 cases). Two-dimensional ultrasound were used to evaluate the characteristics of all the carotid plaque. Plasma Lp-PLA2 were detected for all the patients. Results: Compared to the stable plaque group, the level of Lp-PLA2 were higher in the unstable plaque group, there was a significant difference between them(P<0.05). The characteristics of stable plaque: irregular morphology, plaque ulceration, eccentricity index >2, liquefaction of the plaque are significantly associated with high plasma Lp-PLA2(P<0.05). High plasma Lp-PLA2 was an independent risk factor for instability of carotid plaques(P<0.05). Conclusion: The level of plasma Lp-PLA2 is correlated with the stability of carotid plaque. High plasma Lp-PLA2 is an independent risk factor for instability of carotid plaques(P<0.05).

The diagnostic value of 99mTc-sestamibi SPECT/CT in secondary hyperparathyroidism

Objective: To explore the clinical value of dual-phase 99mTc-sestamibi(MIBI) scintigraphy and SPECT/CT imaging in diagnosing secondary hyperparathyroidism. Methods: Retrospectively analyzed the image features of dual-phase 99mTc-MIBI scintigraphy and SPECT/CT of 38 patients with secondary hyperparathyroidism, which were confirmed by postoperative pathology. Results: The sensitivity of 99mTc-MIBI scintigraphy and SPECT/CT in diagnosing secondary hyperthyroidism was 86.52% and 95.74% respectively. There were significant difference between dual-phase 99mTc-MIBI scintigraphy and SPECT/CT. Conclusions: Preoperative 99mTc-MIBI SPECT/CT imaging is significantly better than dual-phase 99mTc-MIBI scintigraphy in evaluating secondary hyperparathyroidism. 99mTc-MIBI SPECT/CT imaging is an ideal examination method for secondary hyperparathyroidism in preoperative location.

Prognostic value of 18F-FDG PET/CT imaging and VEGF expression in non-small cell lung cancer

Objective: To evaluate the value of PET/CT imaging and VEGF expression in predicting postoperative outcome of non-small cell lung cancer. Methods: Forty patients with non-small cell lung cancer underwent preoperative whole-body PET/CT imaging examination. Tissue samples were collected for VEGF immunohistochemical analysis. Mattern’s semi-quantitative analysis was used to evaluate the VEGF expression in the tumor tissue. According to the clinical and pathological characteristics, the differences of SUVmax and VEGF expression scores among different groups were compared and their correlations were analyzed. Results: SUVmax and VEGF expression scores were significantly correlated with tumor size and differentiation (P<0.05). SUVmax and VEGF expression scores in lung cancer were significantly correlated(r=0.394, P<0.01). Kaplan-Meier analysis showed that SUVmax and VEGF scores were the influencing factors of patient survival. The disease-free survival time of SUVmax ≤7.3 was longer than >7.3 groups(P<0.05). The disease-free survival time of VEGF score ≤3 was also higher than that of VEGF score >3 groups. Cox multivariate analysis showed that SUVmax and TNM staging were independent prognostic factors for patient survival. Conclusion: SUVmax and VEGF expression are related to tumor differentiation and recurrence. SUVmax and VEGF expression scores can be considered as index to evaluate the prognosis of patients.

Automatic preparation of 11C-N-mythyl-3-amino-1, 2-propanediol and study of rat liver in PET/CT imaging

Objective: To develop a fully automatic synthesis for the preparation of aquaglycerporin imaging agent 11C-N-mythyl-3-amino-1, 2-propanediol, and to study the liver uptake in rat using PET/CT. Methods: 11C-N-mythyl-3-amino-1, 2-propanediol was automatically prepared by using TRACERlab FX C Pro system of GE Healthcare and dynamic liver scans were observed on rats, which were injected by the molecular probe 11C-N-mythyl-3-amino-1, 2-propanediol. The changes of distribution of them in liver with time elapse were observed and the curve of time-activity was plotted. Results: The synthesis efficiency of the probe was over 50%, the radiochemical purity was over 98%, the time for the process was about 30 min. The scans of PET/CT showed the distribution of the probe in liver were clear and the radioactivity decreased significantly with time going. Conclusion: The automatic preparation in producing the probe of high radiochemical is simple and easy to control, and the preclinical study of rat liver may be carried out with the new probe.

Endoscopic ultrasonography elastography score and strain ratio in the differential diagnosis of benign and malignant solid pancreatic lesions

Objective: To evaluate the accuracy of the endoscopic ultrasonography(EUS) elastography score and strain ratio(SR) in the differential diagnosis of benign and malignant solid pancreatic lesions. Methods: A total of 38 patients with solid pancreatic lesions identified by endoscopic ultrasound were enrolled in the study to evaluate the efficacy of elastography and SR in differentiating malignant from benign lesions. The semiquantitative score of elastography was represented by the SR method. Sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV), and accuracy were calculated by comparing the diagnoses made by elastography, SR and final diagnoses. According to Young’s modulus, a receiver operating characteristic(ROC) curve was drawn, and it used to identify the value of strain ratio for differential diagnosis of solid pancreatic lesions. Result: Elastography was successfully performed in all patients and there were no procedure related complications. Twenty-six patients were finally diagnosed with pancreatic carcinoma, 2 with benign endocrine tumor and 10 with focal pancreatitis. According to our diagnostic criteria of elastography, the overall accuracy of EUS elastography for the differential diagnosis of focal pancreatic masses was 89.5%(34/38), with a sensitivity, specificity of 92.3%, 83.3%, respectively. Malignant lesions had significantly higher median SR(34.04±18.33) than benign lesions(11.64±13.47). The best cut-off level of SR to obtain the maximal area under the curve was 22.46 with a sensitivity of 79.6%, specificity of 91% through ROC curve. Combined elastography to SR resulted in an accuracy of 94.7%, sensitivity of 96.2%, specificity of 91.7%, PPV of 96.2%, and NPV of 91.7% for the diagnosis of solid pancreatic lesions. Conclusion: EUS elastography has a relatively high accuracy for the differentiation of benign and malignant pancreatic lesions, combined with SR would increase the accuracy.

Synchronous lymph node metastasis can help to discriminate sarcomatoid renal cell carcinoma and clear cell renal cell carcinoma

Objective: To investigate and compare the synchronous metastasis distribution of sarcomatoid renal cell carcinoma(SRCC) and clear cell renal cell carcinoma(CCRCC), so as to contribute to their differential diagnosis. Methods: Synchronous metastasis means metastasis at the time of diagnosis. A retrospective analysis of the medical records and imaging data of 45 SRCC and 143 CCRCC which diameter larger than 6 cm, all the cases had their preoperative abdominal enhancement CT, analysis their synchronous metastasis distribution by χ2 or Fisher’s exact test. Results: There were 16 SRCC and 10 CCRCC had synchronous metastasis(16/45 vs 10/143), 11 SRCC and 1 CCRCC had synchronous lymph node metastasis(11/45 vs 1/143), 6 SRCC and 6 CCRCC had synchronous lung metastasis(6/45 vs 6/143), 6 SRCC and 1 CCRCC had multiple synchronous metastasis(5/45 vs 1/143), and the differences were statistically significant(P=0.000, 0.000, 0.039, 0.003). However, only synchronous lymph node metastasis remained statistically significant(P=0.030) by Logistic regression. Conclusion: Synchronous lymph node metastasis can help to discriminate SRCC and CCRCC, as this feature is more common in SRCC.

Comparative study of ultrasound and radiology in the diagnosis of pediatric ureteral polyps

Objective: To investigate the diagnostic value of ultrasound and radiology in pediatric ureteral polyps and compare the results. Methods: Thirty-nine cases of ureteral fibroepithelial polyps in children comfirmed by surgical pathology, were retrospectively analyzed and preoperative ultrasound results with radiological examination results were compared with the results of surgery. The reasons of missed diagnosis as well as misdiagnosis by ultrasound were summarized and the diagnostic value was evaluated. Results: Thirty-three cases were diagnosed correctly by ultrasound in which 31 cases were unilateral on left and 2 cases were bilaterally multiple(totally 35 polyps). Four cases were missed for only hydronephrosis. Two cases were misdiagnosed for inflammatory deposition and uretaral calculi respectively. The diagnostic accuracy rate of ultrasound was 84.6%. CT scan was performed in 25 children with different degrees of hydronephrosis and 19 cases were positively described as soft-tissue density lesions in the ureteral wall. The diagnostic accuracy of CT was 76.0%. IVP was performed in all cases. Eleven cases were positive on IVP which revealed filling defects. Nine children showed blunting UPJ which was suspicious for fibroepithelial polyp. The remaining 19 cases showed only UPJ obstruction on IVP without additional findings. The accuracy rate of IVP was 51.3%. Conclusions: Ultrasound, which can show the location, number and structural features of polyps, is an effective inspection method of pediatric ureteral fibroepithelial polyps and is superior to other methods owing to its features of simplicity, free radiation and high accuracy.

The evaluation of levator ani contraction function in early postpartum women by three- dimensional ultrasound and pelvic floor muscle electrophysiological assessment

Objective: To evaluate the application value of the levator ani contraction function in early postpartum women by using the three-dimensional ultrasound and pelvic floor muscle electrophysiological assessment. Methods: A total of 60 primiparas without pelvic floor dysfunction who review in postpartum 6~8 weeks were selected as study objects in Xuzhou Maternity and Child Health Care Hospital from March 2016 to March 2018, and then they were divided into natural delivery group 30 cases, and cesarean section group 30 cases, randomly according the ratio 1∶1. Meanwhile, cases of 30 healthy nulliparas were chosen as control group during the same period. The three-dimensional ultrasound and pelvic floor muscle electrophysiological assessment were performed in the three groups. The differences between the three groups were compared, and the correlations between the relative variation quantity of pelvic diaphragm hiatus under anus contracting condition and pelvic floor electrophysiological parameters were analyzed. Results: The relative variation quantity of pelvic diaphragm hiatus under anus contracting condition were positively correlated to pelvic floor electrophysiological parameters, and the correlation coefficient was 0.37~0.68. Conclusion: The three-dimensional ultrasound and pelvic floor muscle electrophysiological assessment have significances in evaluation on the levator ani contraction function.

MRI evaluation of Dega osteotomy in the treatment of developmental dislocation of the hip in children

Objective: A follow-up was performed to make the radiological evaluation after Dega osteotomy in developmental dysplasia of the hip(DDH) in children. The postoperative developmental characteristics of the hip were observed and the relationship of short-term efficacy, radiographic indicators and operative age was analyzed. Methods: Thirty-eight untreated patients included 46 dislocation hips(5 boys and 33 girls; age range from 13 months to 72 months; mean age 25.2 months) were diagnosed with DDH. Those patients include 19 in left, 11 in right and 8 bilateral; in total was 38. All the dislocation hips were divided into A and B groups, the age of group A was less than two years old, the age of group B was equal or greater than two years old. Results: The difference of all the parameters preoperative and postoperative between the dislocation hip and the normal side had statistical significance(P<0.01). The difference of acetabular index(AI) and bone acetabular index(BAI) respectively compared with that of cartilage acetabular index(CAI) had significant statistical significance(P<0.01), but the difference between AI and BAI had no statistical significance(P>0.05). The data was divided into 2 groups(group A and group B), comparison of group A and group B before operation, the parameters of AI, BAI and CAI had statistical significance(P<0.01). Comparison of group A and group B after operation, the parameters of AI and BAI had no statistical significance(P>0.05), the parameter of CAI had statistical significance(P<0.01). The differences between preoperative parameters and the postoperative parameters were negatively correlated with age. Conclusions: The change of CAI could reflect the change of acetabulum more sensitively compared with that of BAI in the short term after Dega osteotomy operation, which suggests the change of CAI could reveal the improvement of acetabulum most, especially for the younger children.

CT and MRI appearances of spinal osteitis deformans

Objective: The aim of this study was to investigate CT and MRI appearances of spinal osteitis deformans. Methods: The clinical features and images of 6 patients with pathologically proved spinal osteitis deformans after operation or biopsy were retrospectively analyzed. All patients underwent CT plain scan, 4 patients underwent MRI scan and 1 patient underwent radionuclide bone scintigraphy. Results: Solitary lesion was detected in 2 cases, and multiple lesions were found in 4 cases. There was a total of 12 lesions. Nine lesions located in lumbar vertebrae and 3 lesions located in thoracic vertebrae. CT images displayed meshy low density region with thickened bone trabeculae in vertebral body and appendix in 8 lesions, diffuse sclerosis was found in 2 lesions and 2 lesions normal appearance. Six lesions showed bone expansion, especially in the spinous process. MRI images showed mixed areas of hyperintensity and hypointensity on T1WI and T2WI in 6 lesions. Two lesions demonstrated prominent diffuse or patchy low signal intensity on T1WI and T2WI and 1 lesion showed subtle low signal intensity on T1WI and T2WI. Four lesions had a band-like low signal intensity along the endplates with a “picture-frame” spine appearance. Bone scintigraphy showed increased uptake of the vertebral body in 1 lesion. Conclusions: Spinal osteitis deformans has some characteristic appearances on CT and MRI. The combined analysis of imaging and clinical features is helpful to accurate diagnosis of this disease.

Feasibility of dynamic contrast-enhanced CT in evaluating perfusion of diabetic patients’ plantar soft tissue

Objective: To prospectively investigate the feasibility and repeatability of multi-slice spiral computed tomography(MDCT) in the quantitative evaluation of perfusion of plantar soft tissue in patients with diabetic and to analyze the microcirculation perfusion characteristics of diabetic foot soft tissue. Methods: Institutional review board approval and informed patient consent were obtained. Thirteen patients with diabetes mellitus(9 males and 4 females; mean age 63.5±11 years, range 40~77) were prospectively included. All patients underwent CTA to make a definite diagnosis and were classified into abnormal group(12) and normal group(14) as having major vascular lesions according to CTA. A Toshiba 320-row spiral CT(Aquilion ONE) was used with acquisition of dynamic volume perfusion of double-sided foot. Time density curve(TDC), blood flow(BF), blood volume(BV), mean transit time(MTT) and time to peak(TTP) of skeletal muscle in normal or abnormal foot were obtained with perfusion software. Results: CT perfusion scans of lower extremities were performed in all patients. Thirteen patients completed the examination successfully. The TDC parameters(BF, BV, MTT and TTP) of the diabetic foot were measured. The abnormal group and normal group respectively were (21.1±12.3) mL/(100 mg·min) vs (34.8±20.2) mL/(100 mg·min), (1.7±0.6) mL/100 g vs (1.8±0.7) mL/100 g, (5.9±1.9) s vs (3.8±1.9) s, (55.5±16.6) s vs (38.9±15.6) s. Good inter-observer and intra-observer agreement was obtained in all patients. By comparing perfusion parameters for normal foot, a significantly longer MTT and TTP was obtained. Conclusion: CT perfusion of the foot is a feasible and reproducible technique that can assess the level of microcirculation in the plantar soft tissue and can prompt the existence of early skeletal muscle ischemia for patients with diabetic foot. The method can be valuable for clinical early diagnosis.
