837 Value of MRI perfusion quantitative parameters in differentiating undifferentiated and differentiated nasopharyngeal carcinoma

Objective: To explore the potential value of perfusion-weighted magnetic resonance imaging(PWI) parameters in differential diagnosis between undifferentiated nasopharyngeal carcinoma(NPC) and differentiated NPC. Materials and Methods: Ninety-four patients with nonkeratinizing NPC(71 undifferentiated, 23 differentiated) underwent PWI. The difference of PWI quantitative parameters(Ktrans, Kep and Ve) was compared between the undifferentiated NPC and differentiated NPC. Results: The Ktrans, Kep and Ve values of the undifferentiated NPC were (0.378±0.161) min-1, (1.514±0.488) min-1 and 0.272±0.123, respectively. For the differentiated NPC, the Ktrans, Kep and Ve values were (0.266±0.059) min-1, (1.204±0.279) min-1 and 0.232±0.063, respectively. Both Ktrans and Kep values revealed significant difference between the undifferentiated NPC and differentiated NPC. For Ve value, no significant difference was found between the two NPC groups. Based on the receiver operating characteristic(ROC) curve, Ktrans had a sensitivity of 84.5% and Kep had a specificity of 91.3% in differentiating the two NPC groups. Conclusion: There is some difference of PWI quantitative parameters between the differentiated NPC and undifferentiated NPC. Ktrans and Kep are possibly helpful in differential diagnosis between the two NPC groups.

840 The value of CT features and the combination in diagnosing papillary thyroid carcinoma

Objective: To investigate the value of CT features and the combination in diagnosing papillary thyroid carcinoma(PTC). Methods: We retrospectively identified the CT features of 192 lesions in 184 cases with diameter larger than 1.0 cm, including the morphology, the bite cookie sign, enhancement features and microcalcifications, and compared with 222 lesions in 170 cases of nodular goiters with diameter larger than 1.0 cm. Then we evaluated the sensitivity, specificity and accuracy with different CT features pattern. Results: Twelve lesions in 12 cases of PTC and 6 lesions in 5 cases of NG were excluded because of complete calcification, influence from Hashimoto thyroiditis, ambiguous morphology or enhancement feature. Thus, 180 lesions in 172 cases of PTC and 216 lesions in 165 cases of NG were included in this study. Morphology, bite cookie sign, enhancement features and microcalcifications showed statistically significant difference between papillary thyroid carcinomas and nodular goiters(P<0.05 for all). The bite cookie sign showed the highest sensitivity, specificity and accuracy of 84%, 94% and 90%, respectively, compared with the other three CT features. Combining two signs together, irregular shape and bite cookie sign showed the highest sensitivity, specificity and accuracy of 71%, 100% and 86%, respectively. Combining three signs together, irregular shape, bite cookie sign, enhancement features or microcalcifications showed the highest sensitivity, specificity and accuracy of 23%, 100% and 65%. Combining four signs together showed a sensitivity, specificity and accuracy of 19%, 100% and 63%. Conclusion: Single CT feature shows a high sensitivity and low specificity relatively in diagnosing PTC, while the combination of different CT signs significantly improves the specificity, reducing the incidence of misdiagnosis.

844 Diagnosis of primary diffuse amyloidosis of large airways by multi-slice CT

Objective: To study multi-slice CT findings of primary diffuse amyloidosis of large airways. Methods: The CT features including lesion location, morphology, wall thickness and density, lumen changes were reviewed in 7 consecutive patients pathologically proved tracheabronchial amyloidosis. Results: The lesions were continuous. Larynx to trachea was involved in 1 case. The trachea to the first, third, fourth and fifth degree of bronchus was involved in 1 cases, 1 case, 2 cases and 2 cases, respectively. The wall of airway was circumferentially thickened with a range from 3.2 mm to 5.9 mm(median 5.5 mm). The inner margin of tracheal or bronchial wall appeared as nodular in 5 cases and smooth in 2 cases. Nodular, patchy or circular calcification of wall was seen in 6 cases with a predilection for bronchi than trachea. Lumen stenosis was seen in all 7 cases. Complete occlusion of the bronchus was seen in 1 case. Conclusion: The multi-slice CT findings of primary diffuse amyloidosis of large airways are continuous and circumferential airway wall thickening, which often results in irregular narrowing of lumen. Calcification of airway wall especially bronchi is very common.

848 Significance of second-look ultrasound for breast lesions

Objective: To assess the significance of second-look ultrasound for breast lesions with different diagnosis between enhanced magnetic resonance imaging(MRI) and ultrasound. Materials and Methods: From August 2010 to June 2012, female patients undertaking ultrasound and enhanced MRI were collected successively. For breast lesions with different diagnosis between enhanced MRI and ultrasound, second-look ultrasound(SLUS) was applied. Finally, 14 women were collected, and 18 lesions were acquired. The final diagnosis for the breast lesions was based on the pathological diagnosis, so-called golden standard. BI-RADS(Breast imaging reporting and data system) was used in the diagnosis with ultrasound and MRI: lesions of BI-RADS 3, BI-RADS 4a and BI-RADS 4b were considered as benignity. However, lesions of BI-RADS 4c and BI-RADS 5 were considered as malignancy. Sensitivity, specificity, accuracy, positive and negative predictive values(PPV and NPV) of second-look ultrasound and enhanced MRI were calculated. The diagnostic rate was tested by Fisher’s exact test. When P<0.05(double tail), the test was significant. Results: In the 18 lesions confirmed by pathology, 14 lesions were benign and 4 lesions were malignant. The sensitivity, specificity, accuracy, PPV and NPV of second-look ultrasound and enhanced MRI were 75%, 92.86%, 88.89%, 75%, 92.86% and 75%, 50%, 55.56%, 30%, 87.5%, respectively. Conclusion: For breast lesions with different diagnosis between enhanced MRI and ultrasound, second-look ultrasound is valuable. The specificity of second-look ultrasound is superior to enhanced MRI.

852 The value of MRI in diagnosis of phyllode tumors of the breast

Objective: To analyze the MRI features of phyllode tumors of the breast(PTB), and to improve the diagnosis and differential diagnosis of this disease by MRI. Materials and Methods: We retrospectively reviewed MRI features of 9 cases of PTB confirmed by pathology, including morphology, signal intensity, the type of TIC, signal on DWI and ADC value. Results: Six lesions were benign, 1 lesion was borderline, and 2 lesions were malignant. In the 6 benign lesions, 5 were lobulated and 1 was oval. All lesions were hypointensity on T1WI. Four lesions were inhomogeneous and 2 were homogeneous hyperintensity on T2WI. 4, 1, and 1 lesions showed TICs of typeⅠ, type Ⅱ and type Ⅲ, respectively. One borderline lesion was lobulated, and showed mixed signals on T1WI and T2WI, with TIC of type Ⅲ. Two malignant lesions were lobulated, and showed hypointensity on T1WI and inhomogeneous hyperintensity on T2WI, with TICs of type Ⅱ and type Ⅲ, respectively. All 9 tumors showed high signal on DWI, and ADC values of 5 cases were lower than the boundary value for differential diagnosis between benign and malignant breast lesions. Conclusion: MRI is helpful to the diagnosis of PTB. Large lesion with fissure, cystic degeneration or septation, TIC of type Ⅱ or Ⅲ and low ADC value should be considered as borderline or malignant lesion, but biopsy is still needed to confirm.

856 Evaluation of imaging quality for coronary angiography in extensiveness ECG-gated angiography using 256-slice CT

Objective: To evaluate imaging quality of the coronary arteries with wide range ECG-gated thoracic and abdominal angiography using 256 slices CT. Methods: Thirty-four patients with chest pain undertook wide rage ECG-gated scanning using 256 slices CT, and coronary arteries were reconstructed. The image quality and diagnosis were evaluated and compared with conventional coronary CT scanning. Results: All 508 segments in the group of wide range ECG-gated scanning were characterized, among which 89.7%(456/508) of the segments was of high-quality and 95.5%(485/508) of the segments could be evaluated. And 512 segments in the group of conventional scanning were characterized, among which 89.8%(460/512) of the segments was of high-quality and 96.3%(489/512) of the segments could be evaluated. There was no significant difference in image quality between the two groups(χ2=1.072, P=0.192). Conclusion: Imaging quality of the coronary arteries with wide range ECG-gated thoracic and abdominal angiography using 256 slices CT can meet the requirement in patients with arrhythmias, and has high value of clinical application.

860 Application of multi-b-value DWI in the differential diagnosis of benign and malignant liver tumor

Objective: To study the value of multi-b-value DWI sequence and ADC value as biological markers in the differential diagnosis of benign and malignant tumors in the liver. Methods: Fifty-two patients with liver tumors confirmed by comprehensive imaging examination and biopsy(7 cases of liver cancer and liver metastases in 16 cases, liver hemangioma in 18 cases, 11 cases of hepatic cysts) were chosen to take DWI scan of multiple b values and conventional MRI. ADClow, ADChigh, ADCperf, ADC3b and ADC10b value were calculated and difference between benign and malignant tumors was analyzed. The receiver-operating characteristic curve(receive operating characteristic curve, ROC) was established. Results: The difference of ADC value between benign and malignant liver lesions was significantly(P<0.05), but the difference between liver cancer and liver metastases, liver hemangioma and liver cyst showed no statistical significance(P>0.05). The ADC value of tumor between benign and malignant lesions had statistical difference(P<0.05), but no significant difference was found between benign tumors(P>0.05). The ADC values of various liver diseases was statistically different with 10b and 3b(P<0.05). With b value of low, the ADC values of different liver tumors showed no statistical significance(P>0.05). The sensitivity, specificity were 96.6%, 87% and 93.1%, 82.6% respectively, when using a threshold ADC of less than 0.102×10-3 s/mm2, 0.285×10-3 s/mm2 in group of 10b and 3b, respectively. Conclusion: Multi-b-value DWI sequence can help in the identification of benign and malignant tumor diagnosis through multiple quantitative parameters from tumor microcirculation and cell metabolism of liver, especially the ADC10b, ADC3b, ADChigh and ADCperf values.

864 Analysis of appendiceal mucinous cystic lesions with imaging and clinical manifestations

Objective: To evaluate the CT and MR imaging findings and clinical features of appendiceal mucinous cystic lesions and to improve diagnosis capability. Methods: Nineteen cases of histologically proved appendiceal mucinous cystic lesions were retrospectively reviewed, including 1 case of simple mucinous cyst, 17 cases of mucinous cystadenoma, 1 case of mucinous cystadenocarcinoma with pseudomyxoma peritonei. The data were analyzed including imaging and clinical manifestation. Results: The clinical manifestations included nausea and right lower quadrant abdominal pain in 15 cases(78.9%), palpable mass in the right lower abdomen in 9 cases(47.4%), right lower quadrant tenderness in 6 cases(31.6%). None of these cases presented with fever. Imaging features included: tubular dilation of appendiceal lumen in 14 cases, gourd-like expansion in two cases. The average diameter of the lesions was 2.95 cm. There were 13 lesions(68.4%) with smooth margin and 3 lesions with rough edge. Nine cases(47.4%) had wall thickness of 2 mm or less. Inner separation of the lesions was seen in 5 cases(26.3%), including one case of mucinous cystadenocarcinoma. Enhanced scan revealed that the wall had no enhancement in 5 cases and slightly enhancement in 7 cases. The synchronic enhancement of the separations was showed in 3 cases. The wall of the lesions enhanced significantly in 3 cases. Calcification was seen in 7 cases(36.8%), including a mucinous cystadenocarcinoma and 6 mucinous cystadenoma. MR imaging features of the lesions were typically homogeneous hypointensity on T1-weighted and hyperoinstensity on T2-weighted imaging. One of the 2 cases showed slightly enhanced cystic wall while the other didn’t. Conclusion: The CT and MR imaging features of appendiceal mucinous cystic lesions are characteristic. Combining with the correlative clinical manifestations, we can make an accurate preoperative diagnosis.

870 The diagnostic value of DWI combined with DCE-MRI in staging of prostate cancer

Objective: To explore the diagnostic value of diffusion weighted imaging(DWI) and magnetic resonance dynamic contrast-enhanced imaging(DCE-MRI) in the staging of prostate cancer. Materials and Methods: DWI and DCE-MRI data of 48 cases of pathologically proved prostate cancer was retrospectively analyzed, and correlated with pathology and tumor staging. Results: Of the 48 cases, DWI and DCE-MRI diagnosed 11 cases of stage ≤B, 23 cases of stage C and 14 cases of stage D. The accuracy of tumor staging was 85.4%. The mean apparent diffusion coefficient(ADC) values of the tumor regions were (0.85±0.15)×10-3 mm2/s, (0.73±0.13)×10-3 mm2/s and (0.64±0.09)×10-3 mm2/s, respectively. There was statistical significance among the three stages(F=8.06, P<0.05). Spearman correlation analysis showed the ADC values in cancerous regions were negatively correlated with tumor staging(r=-0.53, P<0.01). Conclusions: The accuracy of ADC and DCE-MRI is high in staging of prostate cancer, and ADC combined with DCE-MRI might be useful to the staging of prostate cancer.

874 The value of perfusion CT in determining the foci of stage ⅠA cervical cancer

Objective: To assess the ability of perfusion CT in showing the foci of stage ⅠA cervical cancer and to analyze the characteristics of the CT perfusion parameters and the contribution of clinical application. Material and Methods: CT perfusion scans were performed in 20 patients who were clinically diagnosed as cervical cancer of stage ⅠA by biopsy. Then all patients were confirmed as cervical cancer of stage ⅠA by cervical cone cutting operation and pathology. Fourteen cases of cervical cancer patients with normal cervical tissues were used as control. The maps of perfusion parameters were observed and hemodynamic parameters including blood flow(BF), blood volume(BV), mean transit time(MTT) and permeability surface(PS) were calculated and statistically analyzed. Results: There was statistically significant difference in BV and PS between cervical cancer of stage ⅠA and normal cervical tissue(P<0.05). The BV and PS of stage ⅠA cervical cancer were higher than those of normal cervical tissue. There was no statistical difference in BF and MTT between stage ⅠA cervical cancer and normal cervical tissue(P>0.05). Conclusion: CT perfusion maps can intuitively show the foci of stage ⅠA cervical cancer which helps the localization diagnosis. CT perfusion parameters can be used to analyze the blood supply of cervical cancer.

877 The application of MRI T2-mapping in the knee cartilage of the non-professional athletes after marathon

Objective: To evaluate the value of T2-mapping imaging in the knee articular cartilage of the non-professional athletes after marathon. Methods: Cartilage thickness and MRI T2 values of 13 marathon runners(age range: 24~40 years, mean (32±6.3) years) were compared before and after the marathon. A 3.0T scanner was used to calculate thickness and T2 values of the weight-bearing and non-weight-bearing area of the femoral and tibial cartilage. Statistical difference was analyzed. Results: There was no significant difference of conventional magnetic resonance imaging before and after the marathon. MRI T2 values of superficial cartilage were higher than deeper layer both before and after the marathon. After marathon running MRI T2 values decreased in superficial cartilage of the weight-bearing area of the femoral and tibial along with a decrease of cartilage thickness. Conclusion: MRI T2-mapping imaging provides quantitative assessment of articular cartilage correlated with the biochemical changes in cartilage. MRI T2 value of superficial cartilage is higher than deeper layer. After marathon running MRI T2 values decrease in superficial cartilage from the weight-bearing area of the femoral and tibial along with a decrease of cartilage thickness.

882 Research on accuracy of image fusion with hybrid PET/MR

Objective: To evaluate the accuracy of image fusion between PET and MR data by 3D whole body PET/MR scanner with the phantoms and Na point sources simulating clinical practice situation. Materials and Methods: Phantoms and Na point sources were provided by Philips company. The six Na point sources were placed at the fixed positions of the phantoms for image fusion coregistration, and then were fixed on the two sides of the bed for simulating different positions of the patient. The PET and MR images were acquired at 2 different loading conditions: ①uniformly loading phantoms of 10 kg on the table; ②uniformly loading phantoms of 60 kg on the table. The Na point sources were manually outlined on axial MR and PET images. The difference between the point sources in PET and MR was obtained by calculating the distance of the respective center of gravities(COGs). Results: The maximal difference was 3.01 mm in the upper and right part of the table, with loading phantoms of 10 kg. The maximal difference was 4.66 mm on the center and right part of the table, with loading phantoms of 60 kg. Conclusions: The accuracy of image fusion can be measured more objectively with these methods, and it can be used for the acceptance test and routine quality control of PET/MR scanner.
