Evalution of 64-slice spiral CT angiography in postoperative follow-up of intracranial aneurysms

Objective: To investigate the value of 64-slice spiral CT angiography(CTA) in the postoperative follow-up for intracranial aneurysms. Methods: From January 2009 to November 2009, 63 patients with aneurysm were operatived consecutively in the Affiliated Hospital of Luzhou Medical College were collected, and the 63 patients were followed-up after operative with CTA using 64-slice helical CTA. Observations were done to judge whether the aneurysm is completely obliterated, whether the parent and distal artery is stenosis, whether the parent artery is occluded, whether there was cerebral infarction, artery vasospasm, hydrocephalus and recurrent aneurysm, the relationship between the parent artery and clips and the quantity and location of clips. Results: Sixty-three patients were confirmed with intracranial aneurysm, including 68 aneurysms. For these cases using 64-slice helical CTA could clearly reveal the occlusion of the aneurysms, the relationship between the parent artery and the clips and the quantity and location of aneurysm clips, the parent artery and vicinal artery are occluded, ventricle and brain matter reveal clearly. Conclusion: 64-slice helical CTA can be used as postoperative follow-up tool for the aneurysms treated by titanium clips. And it is convenient, cheap and low risk. It can provide more information.

MR perfusion weighted imaging combined with MR diffusion weighted imaging in differentiating recurrent cerebral gliomas and radiation injury

Objective: To evaluate the value of MR perfusion weighted imaging(PWI) combined with MR diffusion weighted imaging(DWI) in the diagnosis of cerebral glioma recurrence and radiation injuries. Methods: Fifty-two patients who had cerebral gliomas were treated with radiotherapy after operation. They underwent routine MR, PWI and DWI due to the discovery of new lesions after radiotherapy. The value of rCBVmax and ADCmin in the new lesion were measured. According to pathology and clinical follow up, the value of combined PWI and DWI in differentiating tumor recurrence and radiation injury were analyzed. Results: In the 52 cases, recurrence of cerebral glioma were 30 cases, radiation injury 22 cases. The rCBVmax value in the recurrent lesion was 2.43±1.52, the value of ADCmin was 0.86±0.21, while the rCBVmax in the radiation injured lesion was 0.67±0.44, and the ADCmin value was 1.17±0.27, there were statistical differences, P=0.000. The accuracy rate of DWI and PWI were 76.9% and 82.7% respectively, the accuracy rate of combined PWI and DWI was 90.4%. Conclusion: MR PWI and DWI are valuable in differentiating recurrent cerebral glioma and radiation injury, combined the two sequences can raise up the accuracy rate of diagnosis.

MRI and pathological features of hemangiopericytoma in central nervous system

Objective: To analyze MRI features of hemangiopericytoma(HPC) and correlated with pathological findings. Methods: MRI appearances and pathological features of 15 patients with HPC confirmed by pathology were analyzed retrospectively. Results: The tumor located inside the skull and outside the brain in 15 cases. On MR imaging, the tumors showed lobulated borders, isointensity or slight hypointensity on T1WI and slight hyperintensity on T2WI with cystic degeneration, blood vessel flow void sign and slight perifocal edema. The solid parts of HPC enhanced markedly after contrast agent was injected. Dural tail sign was seen in 2 cases. Immunohistochemical examination performed in 12 cases showed CD34, vimentin expression, and epithelial membrane antigen, cytokeratin, glial fibrillary acidic protein and S-100 showed no expression, Bcl-2 was expressed in 6 cases. Proliferating cell nuclear antigen Ki-67 immunohistochemical staining was expressed in 10 cases, about 10% expressed in 2 cases, 10%~15% expressed in 1 case. Conclusion: HPC in the central nervous system has certain MRI features, which is helpful in improving the accuracy of the preoperative diagnosis.

The value of coarse calcification in thyroid nodules on CT in the diagnosis and differential diagnosis between benign and malignant lesions

Objective: To summarize the computed tomgraphic characteristic of thyroid nodule with coarse calcification and assess the value of coarse calcification in the diagnosis and differential diagnosis between benign nodules and malignant nodules. Methods: CT findings of 217 nodules with coarse calcifications in 175 patients were retrospectively analysed. All of the patients had pathological diagnosis. Coarse calcifications were divided into simple coarse calcifications(SCC) and non-simple coarse calcifications(non-SCC) according to an associated mass around them, and non-SCC were subdivided into regular coarse calcifications and irregular coarse calcifications according to calcification patterns. Observing the distribution of SCC, regular coarse calcifications, irregular coarse calcifications and clearer borderline around coarse calcifications on enhanced CT than plain CT, and analyse the sensitivity, specificity, positive predicive value and accuracy of SCC for benign nodules, clearer borderline around coarse calcifications on enhanced CT than plain CT for benign nodules of non-SCC and irregular coarse calcifications for malignant nodules of non-SCC. Results: Among 217 coarse calcifications, 169 coarse calcifications were benign nodules, and 48 coarse calcifications were malignant nodules. SCC was more common in benign nodules(P<0.05), clearer borderline around coarse calcifications on enhanced CT than on plain CT was more common in benign nodules of non-SCC(P<0.05), and irregular coarse calcifications was more common in malignant nodules of non-SCC(P<0.05). The sensitivity, specificity, positive predicive value and accuracy of SCC for benign nodules, clearer borderline around coarse calcifications on enhanced CT than on plain CT for benign nodules of non-SCC and irregular coarse calcifications for malignant nodules of non-SCC were 29% vs 60.8% vs 57.1%, 87.5% vs 71.4% vs 71.7%, 89.1% vs 85.9% vs 41.4% and 41.9% vs 63.6% vs 67.9% respectively. Conclusions: SCC are helpful for the diagnosis of benign nodules, clearer borderline around coarse calcifications on enhanced CT than on plain CT indicate the diagnosis of benign nodules of non-SCC, and irregular coarse calcifications for malignant nodules of non-SCC.

Quantitative analysis of contrast-enhanced ultrasonography in the thyroid nodules of different sizes

Objective: To explore the perfusion and enhancement features of the thyroid nodules in different characteres and different sizes using contrast-enhanced ultrasonography(CEUS). Methods: According to the length and postoperative pathology of the thyroid nodules, 118 thyroid nodules from 99 patients given CEUS were divided into four groups: group Ⅰ: thyroid malignant nodules length of lesion ≥10 mm(n=49), group Ⅱ: thyroid malignant nodules length of lesion <10 mm(n=52), group Ⅲ: thyroid benign nodules length of lesion ≥10 mm(n=9), group Ⅳ: thyroid benign nodules length of lesion <10 mm(n=8). The parameters from time-intensity curve of thyroid nodules and the adjacent thyroid parenchyma were acquired through Qontrast software, including peak intensity(PEAK), Time-To-Peak(TTP), regional blood volume(RBV), regional blood flow(RBF) and mean transit time(MTT). The quantitative analysis findings of CEUS of each group and their adjacent thyroid parenchyma were compared adopting analysis of variance. Results: There were significant difference in PEAK, TTP and RBF among group Ⅰ, group Ⅱ and their adjacent thyroid parenchyma. There were significant difference in PEAK, TTP and RBF between group Ⅰ and group Ⅲ. While there were no significant difference in PEAK, TTP and RBF among group Ⅱ, group Ⅲ and group Ⅳ. And there were no significant difference in RBV and MTT among each groups. Conclusions: The data of this study suggested that up to now overlapping and lacking specificity findings had seen in the characteristics of each group of thyroid nodules. CEUS combined time-intensity curve can provide efficient and supplementary information but can not differentiate the specific type of thyroid nodules.

The value of 16 slice spiral CT gastrography with hypotension and water filling of stomach for gastric cancer staging

Objective: To assess the value of 16 slice spiral CT gastrography with hypotension and water filling of stomach for gastric cancer staging. Methods: From January 2010 to June 2013, 152 patients diagnosed as gastric cancer by gastroscopy and biopsy underwent multislice CT before surgery. MSCT examination was performed 1 week before operation. The patients were fasting and were given anisodamine hydrochloride 20 mg through muscular injection 10~15 min before scanning, after that 800~1 000 mL pure water was given orally. Scanning position was arranged according to the site of the lesion discovered by endoscopy and biopsy. The CT features of infiltration of the cancer within and outside the gastric wall(T staging), metastatic lymph nodes(N staging), metastases to adjacent organs(M staging) were observed on axial and reformatted images. Results: CT manifestation of thickening of gastric wall, soft tissue mass were seen in the 152 cases, lesion detection rate was 94%. Accurary of T staging was 74.5%. Accurary of N staging and M staging were 63.7% and 83.3% respectively. Conclusion: Contrast enhanced MSCT gastrography of water filling the stomach with hypotension is an effective examination which can raise up the accuracy of TNM staging before operation and also can accurately evaluate local infiltration and distal metastases.

The value of MRI in assessing the effect of neoadjuvant therapy for stage T3 and T4 rectal cancer before surgery

Objective: To evaluate MR examination in assessing the effect of neoadjuvant therapy for stage T3 and T4 rectal cancer before surgery. Methods: MRI data of the pre- and post-neoadjuvant therapy before surgery in 45 cases pathologically confirmed to be stage T3 and T4 rectal cancer were analyzed retrospectively. The following items were assessed: the size of tumor, the extent of tumor invasion including infiltration of the intestinal circumference, the length of tumor invasion, MRI characteristics after enhanced imaging, intestinal fascia invasion, lymph nodes enlargement, conditions of the adjacent structures and the staging of tumor. Results: Pre-neoadjuvant therapy: In 15 cases the tumor was 3~≤5 cm, >5 cm in 30 cases. In 17 cases the tumor involved less than 1/2 of the whole circumference of the rectum, involved entire circumference in 28 cases. In 14 cases the length of tumor invasion was 3.0~≤5.0 cm, >5.0 cm in 31 cases. Tumor infiltrated to muscular layer in 19 cases, infiltrated the rectal mesentery 15 cases, and the mesenteric fascia in 11 cases. Tumor in all the 45 cases showed medium enhancement, the adjacent organs involved in 26 cases. On DWI, tumor in the 45 cases all showed hyperintensity. The ADC value decreased. Nineteen cases belong to stage T3 and 26 cases were T4. After neoadjuvant therapy, the size of tumor: <1.0 cm in 10 cases, 1.0~<3.0 cm in 18 cases, 3.0~<5.0 cm in 13 cases, ≥5.0 cm in 4 cases. Involvement of rectum circumference <1/3 in 7 cases, 1/3~<1/2 in 19 cases, ≥1/2 but without involvement of the entire circumference in 10 cases, involved the entire circumference in 9 cases. The length of involvement <1.0 in 8 cases, 1.0~<3.0 cm in 25 cases, 3.0~<5.0 cm in 9 cases, ≥5.0 cm in 3 cases. Invasion to mucosal and submucosal layer in 15 cases, invasion of muscular layer in 25 cases, recal mesentery in 5 cases. The tumor in 45 cases all showed mild to medium enhancement. Involvement of the adjacent organs in 5 cases. On DWI all the tumors were hypointense. ADC value increased. The change of staging: T1 in 5 cases, T2 in 28 cases, T3 in 7 cases, and T4 in 5 cases. Pathological staging after operation: T1 in 4 cases, T2 in 29 cases, T3 in 7 cases, T4a in 2 cases, and T4b in 3 cases. Conclusion: MRI is important and valuable in staging of T3 and T4 rectal cancer pre- and post-neoadjuvant therapy before surgerical operation, and in assessing the effect of the neoadjuvant therapy.

Clinical prognosis of isolated absence of septum pellucidum diagnosed with prenatal ultrasonography

Objective: To evaluate the clinical characteristics and the corresponding outcome of fetuses with isolated absence of septum pellucidum(ASP) diagnosed by prenatal ultrasonography. Methods: Clinical data of 14 fetuses with isolated ASP diagnosed in our hospital were from January 2009 to December 2012 analyzed retrospectively. Postnatal follow-up was also conducted, and children’s nervous system development was emphatically observed. Results: Totally 14 ASP fetuses were born and successfully followed up, 12 had normal neurological examinations, 2 got nervous system developmental retardation(one had language deficit and behavioral problem, the other one diagnosed as septo-optic dysplasia and language development delay). Five fetuses underwent MRI examination, of which 4 had normal optic chiasma and optic track, one was difficult to assess. Conclusion: Ultrasonography is an effective method for prenatal identification of ASP. MRI serves to confirm the sonographic findings as well as offering additional diagnostic information. These lesions, when isolated, may be associated with a favorable outcome, however, postnatal follow-up should be strongly emphasized, especially the assessment of neuropsychological and psychiatric development of children with isolated ASP.

The application of DTI and DTT in Brown-Sequard syndrome caused by trauma

Objective: To investigate the value of diffusion tensor imaging(DTI) and diffusion tensor tractography(DTT) in spinal cord injury with Brown-Sequard syndrome caused by trauma. Method: The routine MRI and DTI examnation were performed for patients with Brown-Sequard syndrome caused by trauma within a week. The FA value and the ADC value were measured in more than one vertebral bodies, including the injury side(called A) and relatively normal side(called B) of spinal cord in the level of injury, and the upper(called C) and lower(called D) level of spinal cord injury, then mapping the DTI parameters and DTT figure. Compare and statistical analysed the differences of FA values and ADC values in different regions. Results: The FA value was 0.296±0.011, 0.734±0.059, 0.799±0.076 and 0.716±0.078 in the group A, B, C and D respectively. The ADC value was (1.957±0.41)×10-3 mm2/s, (1.287±0.416)×10-3 mm2/s, (0.924±0.113)×10-3 mm2/s and (0.995±0.097)×10-3 mm2/s in the group A, B, C and D respectively. The differences of FA values and ADC values were statistically significant(P<0.05) between group A and B. The differences of FA values were statistically significant(P<0.05) between group C and D, while the differences of ADC values were not(P>0.05). The reduction of FA values and the rise of ADC values were statistically significant(P<0.05) in group A, B, C and D, while the reduction of FA values were not in group B, C, and D(P>0.05). DTT images showed the fiber bundles of the injured side were fractured and closed to the relatively normal side in the level of spinal cord injury. Conclusion: The FA and ADC values change obviously in the group A, B, C and D. DTI and DTT images can show the damage and degree of asymmetry in the nerve fiber bundles more precisely, and consistent with the nerve function injury of patients. DTI and DTT can provide the image information of patients with Brown-Sequard syndrome caused by trauma accurately.

A comparative study of Fatsat and Smartfat methods in fat suppression at knee joint on GE 1.5T optima 430 s MR system

Objective: All the traditional fat suppression methods have their own limitations, such as limitation of image quality, this study is through the comparative analysis of fat saturation technology of Fatsat and Smartfat methods for knee joint on GE 1.5T optima 430 s, to evaluate the fat suppression effect, MR image quality, examination time of the 2 methods in order to obtain the best optimal fat saturation technique. Materials and Methods: MR fat suppression sequences of the knee joints in 30 healthy volunteers were prospectively collected. There were 15 men and 15 women enrolled into this study, T1WI, T2WI, PD of bosh Fatsat and Smartfat were performed on each individual for comparison. The images of the ligament, cartilage, muscle, menisci and the signal uniformity, fat suppression effect and image quality were evaluated on each of these sequences by 2 experienced radiologists. The score of 3 points was indicated as image clear, 2 points as image less clear, and 1 point as image blurred. Each radiologist evaluated 2 times, so 4 sets of evaluation data were obtained. Fat suppression effect, image quality, examination time of the two methods were compared using t-text analysis, and the diagnostic consistency of the two radiologists and the same radiologist at different time were evaluated using the kappa test. Result: Fatsat T1WI got 7.5±0.98 points, Smartfat T1WI got 12.2±1.51 points, Fatsat T2WI got 8.4±0.43 points, Smartfat T2WI got 15.1±0.75 points, Fatsat PD got 8.7±0.21 points, Smartfat PD got 12.3±0.43 points. The differences were statistically significant. The diagnostic consistency of the 2 radiologists was very good(kappa>0.75). The total examination time of Fatsat was (637±29.6) seconds, Smartfat was (600±25.6) seconds, the difference was statistically significant. Conclusion: The fat suppression effect and image quality of Smartfat sequence are substantially better than those of Fatsat, the examination time of Smartfat is relatively shorter than that of Fatsat.

Early assessment of radiotherapy response of Walker 256 tumor in Wistar rats by 18F-FLT and 18F-FDG PET-CT imaging

Objective: To evaluate the 18F-FLT and 18F-FDG uptake as an indicator of early tumor response after radiotherapy. Methods: 18F-FLT and 18F-FDG PET-CT imaging were performed before treatment, 24 h and 48 h after radiotherapy on Wistar rats bearing Walkar 256 tumor. Quantitative data were expressed as percent of injected dose per gram(%ID/g) and tumor to non-tumor radioactivity ratios(T/NT). Correlation between %ID/g and T/NT with proliferation index(Ki-67) were measured. Results: 18F-FLT uptake in tumor decreased early after radiotherapy(P<0.001), LI-Ki-67 decreased early after radiotherapy(P<0.001) compared with controls. 18F-FDG uptake in tumor not decreased significantly 24 h and 48 h after radiotherapy(P>0.05,P>0.01), 18F-FLT uptake was positively correlated with LI-Ki-67(r=0.889). 18F-FDG uptake was not positively correlated with LI-Ki-67(r=0.379). 18F-FLT and 18F-FDG uptakes(T/NT) were positively correlated with %ID/g(r=0.807 and 0.813, respectively). Conclusion: 18F-FLT uptake decreased early after radiotherapy, and the uptake correlated significantly with proliferation of tumor cells early after radiotherapy. 18F-FDG uptake decreased slightly. 18F-FLT is better than 18F-FDG for the assessment of changes of cell proliferation early after radiotherapy.
