Perfusion-weighted imaging in hypertensive encephalopathy: animal model study

Objective: To investigate the dynamic changes of hypertensive encephalopathy in animal models by perfusion-weighted imaging(PWI). Methods: Twenty adult New Zealand white rabbits were randomly divided into control group(group A) and study group(group B), 10 cases in each group. A modified Gloidblatt 2K1C were used in rabbits to establish chronic hypertension model for control group, and intramuscular injection of Metaraminol Bitartrate to induce acute hypertension in the study group. Routine MRI scanning and PWI of brain were performed in rabbit models on the 3.0T MRI scanner. The values of CBV, CBF and MTT were calculated in selected region of interest(ROI) at workstation, and were compared among anterior, middle and posterior brain regions. Results: The changes of CBF were most marked in posterior brain regions. The CBV, CBF increased after injection of Metaraminol Bitartrate and reached the peak on the ninth day, later than MAP which reached the peak on the sixth day, and then decreased to a basically normal level after drug withdrawal for one week. The change of MTT was not significant. Conclusions: During the course of hypertensive encephalopathy, the level of rCBF first increases and then declines, the peak of which is later than those of MAP.

Application of automatic segmentation tool in MRI structural analysis of sub-cortical gray structures in mild Alzheimer’s disease

Objective: To evaluate the atrophy pattern of subcortical gray matter structures in patients with mild Alzheimer’s disease(AD) by using an automatic segmentation tool. These structures include hippocampi, amygdalae, accumbens, caudates, putamen, pallidum, thalami. Methods: Twenty-nine cases of mild AD and 30 normal aging volunteers as control subjects were scanned with 3D FSPGRIR sequence using 1.5T MR system. The subcortical gray matter structures were segmented using FIRST tool in FSL. The mean values of each structure’s relative volume(gray matter volume/intracranial volume×100) were compared with 2-sample t test. The correlation between the relative volume of each structure and MMSE scores were evaluated using partial correlation analysis. Binary logistic regression analysis and ROC curve were applied to assess the diagnostic significance of the relative volume of each structure. Results: Compared with normal control group, the relative volume of left hippocampus, right amygdala, bilateral thalami, right caudate, left putamen and bilateral pallidum in mild AD group was significantly smaller than that in the control group(P<0.05). The relative volume of left hippocampus, right amygdala, left thalamus and left putamen was correlated with MMSE scores. It was the relative volume of left hippocampus that entered the final model of binary logistic regression analysis. The accuracy was 86.3%. The area under the curve of ROC curve was 0.897. In the cut-off point, i.e. maximal value of Yuden index, the sensitivity was 83.3% and the specificity was 96.3%. Conclusion: In addition to hippocampus, volumetric atrophy was found in other subcortical gray matter structures in mild AD. The atrophy of right amygdala, left putamen and left thalamus may take part in the pathological mechanism of cognitive impairment in AD patients. Among these structures, hippocampus is of the most diagnostic significance.

Ultrasonic elastography and 18F-FDG PET/CT in differential diagnosis of thyroid nodules

Objective: To compare the value of ultrasonic elastography(UE) with 18F-FDG PET/CT in differentiating benign and malignant thyroid nodules. Methods: Eighty-nine patients with 115 lesions in the thyroid were retrospective studied. All the patients underwent UE and 18F-FDG PET/CT. Pathologic outcome was used as the gold diagnosis criterion. The sensitivity, specificity, accuracy of UE in diagnosing thyroid nodules were compared with 18F-FDG PET/CT. Results: Sensitivity, specificity and accuracy of UE were 87.06%, 93.05% and 72.4%, and that of 18F-FDG PET/CT were 96.55%, 66.28% and 73.91%. The accuracy of UE was higher than 18F-FDG PET/CT in differentiating benign and malignant thyroid nodules(χ2=14.29, P<0.05). Conclusion: UE has better diagnostic value than 18F-FDG PET/CT in differentiating benign and malignant thyroid nodules.

Application of 18F-FDG PET/CT combined with HRCT in diagnosing pneumonia type of bronchioloalveolar carcinoma

Objective: To evaluate the application value of 18F-FDG PET/CT combined with HRCT in diagnosing pneumonia type of bronchioloalveolar carcinoma(PTBAC). Materials and Methods: The 18F-FDG PET/CT images of 26 cases with pathologically confirmed PTBAC were studied. Fifteen of the 26 cases were followed up after 0~3 days by HRCT study. The diagnostic accuracy rate of 18F-FDG PET/CT combined with HRCT were analyzed. Results: According to 18F-FDG PET/CT imaging, definite diagnosis of malignant was made in 9 cases, no exclusion of malignancies in 13 cases, and definite diagnosis of pulmonary inflammation in 4 cases. The diagnostic accuracy rate of 18F-FDG PET/CT imaging was 34.6%. The misdiagnosis rate of 18F-FDG PET/CT was higher. Associating 18F-FDG PET/CT with HRCT, 13 of 15 cases were diagnosed as malignant tumors. Pulmonary inflammation was diagnosed firstly in 1 case by PET/CT, and then definite diagnosis of malignancy was confirmed by HRCT. In addition, the other one case of malignant tumor was analyzed as pulmonary inflammation by PET/CT and HRCT. However, 18F-FDG PET/CT displayed metastasis during the delay scanning, which was approved by pathology. Based on 18F-FDG PET/CT and HRCT results, 15 cases were diagnosed correctly. Conclusion: PTBAC displays a variety of characteristics in 18F-FDG PET/CT and HRCT. Combining 18F-FDG PET/CT with HRCT is reasonable and practicable for PTBAC diagnosis, which may be due to the synergistic effect on diagnosing PTBAC and can greatly improve the diagnostic accuracy.

Perfusion CT in assessing the early efficacy of the chemotherapy of lung cancer

Objective: To observe the changes of parameters in perfusion CT before and after chemotherapy for patients with lung cancer and its relationship with the efficacy of treatment. Methods: Twenty-five patients were enrolled into the study who were pathologically diagnosed as lung cancer and would undergo conventional chemotherapy for lung cancer. According to RECIST criteria, the patients were divided into remission group and no-remission group. Each patient underwent two perfusion CT scans respectively within a week before and about 90~100 days after chemotherapy. The relationship of parameters changes before and after earlier chemotherapy and the efficacy of chemotherapy was studied. Results: Among the 25 cases, 15 cases had remission and the other 10 cases without remission. For all the 25 patients, the changes of tumor size in the two groups were not significant before and after chemotherapy(with an average interval of 21 days), but the changes of BV and PBV was statistically significant(in the remission group, BV and PBV before and after treatment were (71.0±20.4) mL/1 000 mL, (49.8±31.4) mL/1 000 mL; (73.1±34.1) mL/1 000 mL, (45.2±19.0) mL/1 000 mL; t values of 2.558 and 3.325; P values of 0.023 and 0.005, and the two parameters showed a downward trend. In the group without remission, BV and PBV before and after treatment were (50.4±61.0) mL/1 000 mL, (120.5±15.7) mL/1 000 mL; (38.0±9.3) mL/1 000 mL, (62.0±38.3) mL/1 000 mL; t value of -3.062, -2.471; P value 0.016, 0.036, upward trend). The relativity of the remission rate and the parameters of BV or PBV group χ2 test revealed: regardless of whether the parameter values increased or decreased, the changes of BV and efficacy of chemotherapy were not statistically significant. The changes of PBV correlated well with the effects of chemotherapy. The remission rate of group with reduced PBV was higher than that of the elevated one. Conclusion: No obvious change of size is observed between the lesions before and after chemotherapy in early stage. The changes of the perfusion CT parameters in the early stage after chemotherapy have potential application in predicting the efficacy of chemotherapy in the treatment for lung cancer.

Evaluation of endocardial and epicardial rotation in type 2 diabetes mellitus patients by two-dimensional speckle tracking imaging

Objective: To observe the rotation parameter of left ventricular short-axis views in the subendocardial and subepicardial myocardium layers by two-dimensional speckle tracking imaging(2DSTl), and to evaluate its performance in patients with type 2 diabetes mellitus(T2DM). Methods: Fourty-three patients with T2DM(20 patients without microangiopathy, 23 patients with microangiopathy), and 30 cases of healthy volunteers as control were included in the study. High frame rate dynamic two-dimensional images were recorded at the left ventricular short-axis view, including basal and apical planes. The peak subendocardial rotation(Prot-endo), peak subepicardial rotation(Prot-epi), peak bulk rotation(Prot), and peak mural rotation(Prot-mur) were measured. Results: ①The apical Prot-endo, Prot-epi, Prot and the basal Prot-epi were higher and the apical Prot-mur was lower in control group than the T2DM group with microangiopathy(P<0.05). ②The basal Prot-endo of T2DM patients with microangiopathy was significantly lower than that in control group. ③When compared to the control group, the basal Prot-mur decreased while the basal Prot increased in the T2DM group(P<0.05). Conclusions: The 2DSTI can accurately evaluate the characteristics of left ventricular rotation parameter and sensitively evaluate the cardiac function in patients with T2DM.

Right heart echocardiography analysis in 315 cases of native adult

Objective: To test whether the diagnostic indicators and criteria in the guidelines for the echocardiographic assessment of the right heart in adults by American Society of Echocardiography in 2010(guidelines) is adaptable to native people. Methods: 210 patients presenting with signs and symptoms of cardiopulmonary disease in patient group and 105 healthy individuals in control group accepted right heart assessment following the guidelines. The student’s t test was used to compare the mean value difference. The ROC curve was used to evaluate the indicators value. Results: All indicators in the patient group were statistically different to those in the control group. The thresholds for diagnosis of right heart failure recommended by the guidelines were of high specificity but poor sensitivity. Conclusions: The guidelines are valuable, and should be incorporated into the clinical material to diagnose right heart failure.

Gastric schwanomas: CT features and correlation with pathology

Objective: To investigate the CT features and pathology of gastric schwanomas. Methods: The CT imaging of 10 cases of gastric schwanomas proven by surgery and pathology were retrospectively analyzed and correlated with pathology. Results: All of the 10 cases had solitary tumor in stomach, with 6 in the body, 2 in the antrum and 2 in the bottom. All the lesions varied in size, but mainly less than 3 cm. Most of the lesions were of homogeneous iso-density or slightly low density. One case had slightly inhomogeneous attenuation, without any necrosis or cysts. Two cases had ulcer surface. After enhancement, all of the leisons had moderate enhancement in venous phase. Conclusion: CT features of gastric schwanomas had some characteristics and are helpful for the characterization and location of the tumor as well as assessing the relationship of the tumor and surrounding structures. Nevertheless, the concrete diagnosis needs pathology.

A study of three-dimensional CT measurement of Zhuang people and Han people’s craniofacial bone(comparison of 100 cases)

Objective: To study the characteristics of craniofacial bone of Zhuang people by CT. Methods: The craniofacial bones of 100 Zhuang people and 100 Han people were scanned by spiral CT. Three-dimensional reconstructed image and three-dimensional measurement were performed. All data was compared between different peoples. Results: The results showed defference between male and female in the 100 Zhuang people. Compared with Han people, Zhuang people has their facial characteristics, such as wider and higher cheek bone. No specific features in the other parts. Conclusions: The measurement of the Guangxi Zhuang people’s craniofacial bone, such as Tz1-Tz1’, Tz2-Tz2’, angle Tz1-Go-Me were significantly greater than those of the Guangxi Han people, which can accurately reflect the Guangxi Zhuang people cheekbones prominent, morphological features and help to distinguish from the local Han people.

The diagnostic value of fluid-fluid levels in distinguishing between benign and malignant bone lesions using CT/MR

Objective: To analyze the diagnostic value of fluid-fluid levels in distinguishing benign and malignant bone lesions using CT/MR. Methods: Sixty-six cases of primary bone lesions proven by pathology were reviewed, among which 12 cases were examined by CT, 51 cases were examined by CT and MR, and the other 3 cases were examined by MR. Fluid-fluid levels(FFLs) were identified in all of the cases. The proportion of the lesion occupied by FFLs was estimated as <1/3, 1/3~<2/3, 2/3~<100% and 100%. The proportion was correlated with the pathological diagnosis and confirmed by double blind method. Results: Pathology revealed 58 benign bone lesions and 8 malignant bone lesions. When the degree of FFLs was <1/3, malignant lesions were relatively common(3/8, 37.5%), and all of which were osteosarcoma. With the increasing of FFLs degree, benign lesion became more frequent. When the degree was >2/3, 93.6% of lesions were benign, most of which were giant cell tumors with aneurysmal bone cysts or primary aneurysmal bone cysts. When the lesions were entire FFls, all of them were benign. Conclusions: More proportion of FFLs in primary bone lesion suggests more possibility of benign bone tumor.

The role of 18F-FDG PET-CT in the detection of primary lesions for patients with tumor metastasis

Objective: To evaluate the 18F-FDG PET-CT in detecting primary foci in cases of suspected metastatic tumor. Methods: One hundred and sixty-four patients with pathologically proved metastasis of unknown primary tumor underwent 18F-FDG PET-CT scan. Statistical analysis was performed after the final follow up. Result: The total positive rate, specificity, sensitivity, accuracy, negative predictive value and positive predictive value were 51.2%(84/164), 97.3%(71/73), 92.3%(84/91), 94.5%(155/164), 91.0%(71/78), 97.7%(84/86) respectively. The positive rate of the primary foci for metastatic adenocarcinoma, metastatic squamous carcinoma and metastatic undifferentiated carcinoma were 54.0%(47/87), 56.8%(21/37) and 40.0%(12/30) respectively. According to the statistical analysis, there was no statistical difference among the positive rate of the three groups. Conclusion: 18F-FDG PET-CT might be of importance in the detection of primary site in unknown primary tumors
