685 Comparative study of peritumoral area of high- and low-grade astrocytoma with CTP

Objective: To probe the difference of CT perfusion(CTP) parameters of peritumoral area between high- and low-grade intracranial astrocytoma, and to explore the pathologic basis. Methods: Thirty cases of intracranial asytrocytoma, proven by surgery and pathologic examination, including 10 cases of low-grade tumors(WHO grade Ⅰ and Ⅱ) and 20 cases of high-grade tumors, were examined for precontrast CT, postcontrast CT and CTP. We mearsured the CTP parameters, including CBF, CBV, MTT and PS at the area of 1cm from the margin of enhancement or the margin of the non-enhanced lesion. Paired t test was made for comparison of the CTP parameters of high- and low-grade astrocytoma with the software SPSS 17.0 for refering CTP and postcontrast CT maps, biopsy was made before incision. And the pathologic findings of peritumoral area was observed. Results: CBF, CBV, MTT and PS of peritumoral area of high- grade astrocytoma were higher than those of low-grade astrocytoma. The CBF, CBV, MTT and PS value, peritumoral area of high-grade astrocytoma were(50.05±14.05)ml/(min·100g), (3.50±1.79)ml/100g, (5.84±2.95)s and (1.23±1.75)ml/(min·100g) respectively. And low-grade astrocytoma were (25.20±8.67)ml/(min·100g), (1.61±0.68)ml/100g, (4.83±1.63)s and (0.33±0.27)ml/(min·100g) respectively. There were significant difference of CBF and CBV between high- and low-grade astrocytoma(P<0.05). But no obvious difference was found for MTT and PS between high- and low-grade astrocytoma(P>0.05). Discrete tumor cells were detected in peritumoral area for both high- and low-grade astrocytoma. Whereas more neoplastic cells and vascularity were found in peritumoral area of high-grade astrocytoma. Conclusion: Because of different pathologic basis, there was significant distinction of CTP parameters of peritumoral area between high- and low-grade astrocytoma. This distinction is due to their different findings on pathology.

689 Value of MR 3D-FIESTA sequence in displaying the cavernous segment of oculomotor nerve

 Objective: To asses the value of 3D-FIESTA sequence in displaying the cavernous sinus segment of oculomotor nerve in order to obtain more detailed MR anatomical data of this nerve for clinical application. Methods: Thirty-eight normal volunteers and 8 patients with oculomotor nerve palsy underwent conventional sequence and 3D-FIESTA sequence associated with multi-planar reformation(MPR) and curved planar reconstruction(CR) from different angle to show the pathway of the cavernous sinus segment of oculomotor nerve. Results: In the 38 normal volunteers, the cavernous segment of oculomotor nerve can be showed clearly in 40.8%(15 cases), can not be showed clearly in 57.8%(22 cases), only a part of the segment was showed, and the nerve can not be showed in 1.3%(1 case). In the 8 patients with oculomotor nerve palsy, the cavernous segment of the nerve can be showed clearly in 12.5%, only a part of the nerve can be showed in 62.5%, in 25% of cases the nerve was not showed. Conclusion: To some extent, MR 3D-FIESTA sequence with post-processing techniques can be used to show the anatomical detail of the canvernous sinus segment of the oculomotor nerve, but involvement of the cavernous sinus can not be showed clearly only provide indirect information for diagnosis and further surgery planning.

693 Imaging and differential diagnosis of lymphoma in Waldeyer’s ring

Objective: To study CT and MRI features of lymphoma in Waldeyer’s ring, in order to improve the accurate diagnosis and differential diagnosis. Methods: The CT and MRI data of 15 cases of non-Hodgkin’s lymphoma in Waldeyer’s ring, proven pathologically and clinically were studied retrospectively. Results: Pathology showed all cases were non-Hodgkin’s lymphoma, including 10 cases from B cells, and 5 cases from NK cells or T cells. Seven cases of palatal tonsil lymphoma showed round soft tissue mass with clear border in amygdaloid pit, and five cases involved both sides. Eight cases of palatal tonsil lymphoma grew aggressively within nasopharyngeal cavity, and some cases had turbinate or nasal sinus bone metastasis. CT imaging showed clear border between the mass and para-tumor tissues. The lymphomas were isointense to neighboring muscle on T1WI, and hyperintense on T2WI and DWI, and the signals were homogeneous with mild-to-moderate enhancement, without necrosis and cystic appearances. Ten of the fifteen cases showed bilateral swelling of cervical lymph nodes. Conclusion: Pharynx lymphatic loop lymphomas were mostly non-Hodgkin’s lymphomas, and they had characteristics of density, signal intensity, borders, and enhancement on CT or MRI, which need differentiation from tuberculosis, nasopharyngeal carcinoma, and tonsil cancer. Both CT and MRI could describe the position, shape, extent, and whether or not having neighboring involvement, and these information are helpful for the clinical diagnosis and treatment.

697 High frequency color Doppler ultrasonography in the diagnosis of ductal carcinoma in situ

Objective: To evaluate the diagnostic value of high frequency ultrasonography(HFUS) through summarizing the ultrasonic signs of ductal carcinoma in situ(DCIS). Methods: The ultrasonographic features in 40 cases of DCIS proved by post-operative pathology and 30 cases of hyperplastic disease of breast(HDB) including boundary, internal echogenicity, microcalcification, posterior echogenicity, peak systolic velocity(PSV), resistance index(RI) and blood distribution, were analyzed. Results: The ultrasonographic features of DCIS are mass-forming type, patchy hypoechoic type, duct dilatation type and microcalcification type, accounted for 57.50%(23/40), 27.50%(11/40), 10.00%(4/40) and 5.00%(2/40), respectively. There are 7 patients with microinvasion, including 1 patient with axillary lymph node metastasis. Thirty-one cases of DCIS and 24 cases of HDB were diagnosed correctly by preoperative ultrasonography, the accuracy, specificity and sensitivity were 78.57%, 80.00% and 77.50% respectively. The positive predictive value and negative predictive value were 83.78% and 72.73% respectively. Microcalcification and RI were statistically significant(P<0.05) in differential diagnosis between DCIS and HDB. Conclusion: The ultrasonic characteristics of DCIS has certain specific features, HFUS diagnosis for DCIS are of great value.

701 Speckle tracking imaging in early assessment of thoracic radiotherapy-induced left ventricular systolic function damage at short-axial view

Objective: To evaluate the value of speckle tracking imaging(STI) in detecting early thoracic radiotherapy-induced myocardial damage by using STI to quantitatively analyse the left ventricular systolic function at short-axial view. Methods: STI was performed in 46 patients with thoracic radiotherapy. The 46 patients were divided into three groups: group A: before radiation treatment(control group); group B: radiotherapy time was 2.5~3 weeks, radiotherapy dose was 26~30Gy; group C: radiotherapy time was 5~6 weeks, radiotherapy dose was 50~60Gy. Radial strain(RS) and circumferential strain(CS) were measured in the left ventricular short-axial view at the levels of mitral annulus and papillary muscle. Results: ①RS and CS of group C in anterior wall, anterior septal and posterior wall were significantly lower than those in group A(P<0.01); RS and CS of group C in anterior wall, anterior septal and posterior wall were lower than those in group B(P<0.05); RS and CS of group B in anterior wall, anterior septal and posterior wall compared with those in group A, had no significant difference(P>0.05); ②RS and CS of left lateral wall, posterior septal and inferior wall in all groups had no significant difference(P>0.05). Conclusions: RS and CS after the thoracic radiation treatment were significantly lower than those in the patients before radiation treatment, STI can indicate the change of heart disease in systolic function. STI is valuable to assess early thoracic radiotherapy-induced myocardial damage.

704 Autoimmune pancreatitis: study of CT manifestation

Objective: To investigate the CT imaging findings of autoimmune pancreatitis(AIP). Methods: The CT data of 11 patients with AIP were reviewed retrospectively. Plain CT scanning and dynamic contrast-enhanced scanning were performed in 11 patients. The location, shape of abnormal pancreas and the shape of boundary between normal and abnormal pancreas in segmental AIP were analyzed(in this text, the boundary is called “front-end” of abnormal pancreas, and the abnormal pancreas between the boundary and the most bulky abnormal pancreas is called “forepart”). The CT attenuation value of the pancreatic parenchyma of AIP and normal pancreas in plain scanning, the arterial phase, portal vein phase and delayed phase were calculated. The mean CT attenuation value of the pancreatic parenchyma in patients with AIP was compared with that in patients with a normal pancreas(n=25). Results: Four patients showed diffuse swelling of the pancreas and 7 patients showed segmental enlargement of pancreas. The tiny front-end and cone-shaped forepart was seen in 2 patients with segmental AIP, and the flat front-end and trapezoid forepart in 1 patient with segmental AIP, and the arcuate front-end and mimic bullet-point forepart in 3 patients with segmental AIP; the mimic bullet-point forepart at one side and the cone-shaped forepart at another side of abnormal pancreas in 1 patient with segmental AIP. Fusiform body of abnormal pancreas was seen in 3 patients of segmental AIP, irregular columnar body of abnormal pancreas with coarse edge in 3 patients with segmental AIP, and sausage body of abnormal pancreas in 5 patients. Abnormal pancreas showed homogeneous density, and the mean CT attenuation value of the abnormal pancreatic parenchyma was slightly lower than that of control group on plain scanning, significantly lower than that of control group in the arterial phase, slightly higher than that of control group in the portal vein phase. Conclusion: Abnormal pancreas of AIP may represent various shape, whereas there is some distinctive CT imaging manifestations. CT dynamic contrast-enhanced scanning play an important role in differentialing AIP and other massive lesions. AIP can be diagnosed correctly through CT examination.

711 Value of 3D-excretory contrast-enhanced MRU in the diagnosis of urologic diseases

Objective: To investigate the clinical value of 3D-excretory contrast-enhanced MRU(3D-ceMRU) in the diagnosis of urologic diseases. Methods: Forty-five patients with urologic diseases had MRI and MRU. 3D-ceMRU was performed by 3D SPGR, and 3D reconstruction MIP from source images were acquired. Results: 3D-ceMRU demonstrated the localization of urologic diseases was 100% in all cases, and in congenital abnormalies, urinary lithiasis, and stenotic urinary tract diseases if combined with plain and contrast-enhanced MRI. 3D-ceMRU could satisfactorily display the location, shape, size and quantity of diseases, and could roughly evaluate renal function and the degree of hydronephrosis. Conclusion: 3D-ceMRU is a safe and noninvasive technique. This technique has a high accuracy in the diagnosis of many urologic diseases and it is one of the best way for showing these diseases.

716 Value of preoperative MRI in prediction of pathological stage and circumferential resection margin in rectal cancer

Objective: To evaluate the accuracy of preoperative MRI in prediction of pathological staging and involvement of circumferential resection margin(CRM) in rectal cancer. Methods: Twenty-eight patients were biopsy-proven to be rectal cancer through preoperaitive colonoscopy. All patients were inserted with a triple cavity catheter with balloon inflated with air to the anus before MRI examining, Siemens Magnetom Avanto 1.5T were used. All patients underwent total mesorectal excision and were assessed prospectively using high-resolution MRI for tumour(T) and mesorectal nodal(N) staging as well as CRM status using the depth of tumour spread, tumour node metastasis and CRM involvement. Preoperative MRI assessment of these prognostic factors was compared with the histopathological findings by carefully correlated with pathologic specimen. Results: MRI correctly staged the tumor in 24 patients, had 4 mistakes, understaged in 1 case, and overstaged in 3 cases. The accuracy of T stage was 85.7%(24/28). There was an accurate correlation between pathologic and MRI tumor staging(Kappa=0.773, P<0.001). Node status was correctly staged in 22 patients, overstaged in 4 cases, and understaged in 2 cases. The accuracy of node staging was 78.6%(22/28), sensitivity was 89.5%(17/19), and specificity was 55.6%(5/9). The correlation between pathologic and MRI node staging was generic(Kappa=0.478, P=0.01). The CRM status was correctly reported in 26 patients, overstaged in 1 case, and understaged in 1 case. The accuracy of CRM status was 92.9%(26/28), sensitivity was 85.7%(6/7), and specificity was 95.2%(20/21). There was a good correlation between pathologic and MRI CRM involvement(Kappa=0.81, P<0.001). Conclusion: Preoperative MRI provides generic predictive data as to mesorectal node stage, but does produce reliable prediction of clear CRM and tumor staging.
