1 The study of brain proton magnetic resonance spectroscopy(1H-MRS) in patients with mild cognitive impairment1008-1062(2011)01-0001-05

Objective: To evaluate the roles of proton magnetic resonance spectroscopy(1H-MRS) in the diagnosis and differential diagnosis of amnestic mild cognitive impairment(aMCI), Alzheimer disease(AD) and normal control group. Methods: Normal scanning, 1H-MRS of VOI are acquired by Siemens Novus 1.5T MRI system. A 1H-MRS was performed in the bilateral medial temporal lobes(MITS) and left posterior cingulated contex(PCC) of the 12 aMCI patients, 12 AD patients and 12 healthy age and sex-matched controls using single voxel sequence mode(SVS-SE-30). The ratios of NAA/Cr, Cho/Cr, mI/Cr, NAA/mI and metabolite asymmetries were compared among the groups. Results: NAA/Cr of left medial temporal lobe in the aMCI and AD groups were lower than that of the NC group with statistical significance(P<0.05), AD group and the aMCI group had no statistically significant difference(P>0.05); NAA/mI in aMCI and AD group was lower than that in the NC group with statistical significance(P<0.05). There was no statistically significant difference in AD group and the aMCI group(P>0.05). NAA/mI of right medial temporal lobe in AD and aMCI group was lower than that of the NC group with statistical significance(P<0.05). There were no statistical significance of NAA/Cr, Cho/Cr, mI/Cr and NAA/mI located in left posterior cingulate gyrus of aMCI, AD and NC groups. Metabolites asymmetry index of two sides of medial temporal lobe of aMCI group and NC group had statistically significant difference(P<0.05), metabolites asymmetry index of other position had no significant difference. Conclusion: The changes of NAA/Cr in the left medial temporal lobe play an important role in differential diagnosis of aMCI, AD and NC; the reduction of NAA/mI in bilateral medial temporal lobes is earlier than NAA/Cr in aMCI due to the reason that hyperplasia of neuroglial is earlier than the loss of neurons. 1H-MRS is a useful method in monitoring, diagnosis and differential diagnosis of aMCI, AD and NC.

6 Study the relationship between the expression of Annexin A7 and CT ofnasopharyngeal carcinoma

Objective: To study the relationship between the expression of Annexin A7 and CT of nasopharyngeal carcinoma(NPC). Methods: Fifty-eight cases of NPC, 20 cases of lymph node metastatic NPC(LMNPC) and 24 cases of chronic rhinitis(CR) were all performed immunochemistry to detect the expression of Annexin A7. All patients of NPC had axial CT scan and some of the cases had coronal CT scan and contrast enhancement. Results: The positive ratio of Annexin A7 was 87.9%(51/58), 95.0%(19/20) and 41.7%(10/24) in NPC, LMNPC and CR, respectively. The expression of Annexin A7 were higher in LMNPC than NPC, in NPC than CR(P<0.05). There was correlation of expression of Annexin A7 to differentiation, region of metastasis, lymph node metastasis, distant metastasis and clinical stage(P<0.05), but no correlation to age and gender. There were correlation among the expression of Annexin A7 to invasion of posterior nares, pharynx oralis and bone destruction of base of skull, cervical lymph node metastasis showed in CT(P<0.05). Conclusion: There was relationship of the expression of Annexin A7 to differentiation, invasive depth and destruction of base of skull of NPC. And serve as the index of malignancy degree of CT and prognosis of NPC.

10 The value of coronal CT in the management of sinonasal malignancies

Objective: To explore the value of CT findings in the diagnosis and management of sinonasal malignancies. Methods: Fifty cases of coronal CT findings in sino-nasal malignancies were analysed retrospectively, and treatment planning were made depending on the CT manifestations. Results: All of the tumors were diagnosed under endoscopy. The rates of squamous cell carcinoma, melanoma, adenoid cystic carcinoma, olfactory neuroblastoma, lymphoma, and other malignancies were 30.0%(15/50), 14.0%(7/50), 14.0%(7/50), 8.0%(4/50), 14.0%(7/50) and 20.0%(10/50), respectively in which 66.7% of the squamous cell carcinomas invaded maxillary sinus; Most of the melanoma originated from nasal cavity, 71.4% of which invaded ethmoid sinus; Most of the olfactory neuroblastoma originated from nasal cavity, 100.0% of which invaded ethmoid sinus; 28.6% of the lymphoma invaded both nasal cavities, also can occur in each of the sinuses. To select treatment method based on coronal CT findings: 30.0%(15/50) of the tumors were removed under endoscopy; 26.0%(13/50) of the patients were treated with chemotherapy or radiotherapy after diagnosed under endoscopy; 14.0%(7/50) of the tumors were removed with Caldwell-Luc operation combined with endoscopy; 20.0%(10/50) of the tumors were removed through nasal lateral approach combined with endoscopy; 10.0%(5/50) of the tumors needed to remove the partial or complete maxillary bone. Conclusion: The lesions and invasions of sino-nasal malignancies could be shown clearly on coronary CT of the nasal sinus, which has great value in the diagnosis and treatment selection of sino-nasal malignancies.

13 Dual source CT angiography in the pre- and post-hybrid surgery of DeBakey type I aortic dissection

Objective: The purpose of this study was to evaluate the value of dual source CT angiography(CTA) in the pre- and post-hybrid surgery of DeBakey type I aortic dissection. Methods: The pre- and post-treatment CTA data from 11 patients with DeBakey type I aortic dissection undergoing hybrid surgery were retrospectively analyzed; CTA and intra-operative DSA data from 6 patients correlated with CTA. Results: For all 11 patients, DeBakey type I aortic dissection was definitely diagnosed at pre-treatment CTA. Crevasse was localized in the upper part of the ascending aortic root(n=6), the aortic arch(n=3). Organized thrombus in the false cavity was shown in 2 patients. All 11 cases were involved in the thoracic and abdominal aorta, of which 4 cases involved in the branches of aortic arch. Post-operative CTA clearly showed the location of the stents, organized thrombus in the false cavity, reconstructed aortic arch and bypass vessels. Occlusion of the proximal segment of left subclavian artery was demonstrated in two patients, endoleak in one patient; organized thrombus in the false cavity, enlarged true cavity and patent bypass vessels in the remaining 8 patients. Conclusion: Dual source CT angiography plays an important role in the evaluation of the pre- and post-hybrid surgery of DeBakey type I aortic dissection.

16 Assessment of left ventricular remodeling and systolic synchronicity inpatients with hypertensive heart disease by RT-3DE

Objective: Using real-time three-dimensional echocardiography(RT-3DE) to assess left ventricular remodeling in patients with hypertensive heart disease(HHD). Methods: RT-3DE data of 96 HHD(32 normal configuration, 28 concentric hypertrophy, 36 eccentric hypertrophy), 36 normal controls were acquired. Left ventricular end-diastolic volume(LVEDV), left ventricular end-diastolic epicardial volume(EDVepi) by RT-3DE and two-dimensional echocardiography(2DE) Simpson biplane method; left ventricular mass(LVM), left ventricular mass index(LVMI), left ventricular end diastolic epicardial volume index(EDVI), left ventricular remodeling index(LVRI) were calculated and compared. Result: ①LVRI, EDVI, LVMI measured by RT-3DE and 2DE were significantly different inter group, in eccentric hypertrophy groups(P<0.05), but no significantly different in normal control group, normal configuration and concentric hypertrophy(P>0.05); ②The LVRI, EDVI, LVMI had significant correlation between RT-3DE, 2DE in normal control group, normal configuration and concentric hypertrophy, the LVRI, EDVI, LVMI had no significant correlation between RT-3DE, 2DE in eccentric hypertrophy. Conclusions: RT-3DE LVRI can be a new approach for evaluating left ventricular remodeling and systolic synchronicity in clinic.

20 Discuss the value of vascular index in the diagnosis of chronic kidney diseases

Objective: To explore the reliability of three-dimensional color power angiography(3D-CPA) ultrasound parameter vascular index(VI) to assess the degree of renal damage of chronic kidney disease(CKD). Methods: Sixty patients suspected with chronic kidney disease underwent 3D-CPA and percutaneous biopsy, and 30 healthy adults served as control. The VI and pathological change scoring of renal were measured, and the correlation of VI and pathological findings as well as clasification of CKD were analyzed. Results: Different types of renal diseases had different VI; Thirty-three patients in the group I, the diseases limited to the glomeruli, VI(37.64%±9.97%) was closed to that of the control group(33.44%±4.31%)(P≥0.05). Twenty-eight patients in group II, the diseases positioned in the tubulo-interstitial and blood vessel, VI(23.34%±12.12%) was higher than that of group I and the control group(P<0.05). There was a significant correlation between the value of VI and classification of CKD. Conclusions: VI can reflect the types of renal diseases. In case of CKD with VI in normal limit, the diseases may be limited to the glomeruli; while the VI elevated, the diseases may locate in the tubulo-interstitial or blood vessel; The lower the VI, the higher the classification of CKD.

23 Accuracy of diagnosing para-aortic and pelvic lymph node metastasis in cervical cancer with PET/CT:a meta analysis

Objective: To review studies focused on diagnosing pelvic and para-aortic lymph node metastasis in cervical cancer with PET/CT, analyse sources of bias and variability and evaluate the diagnostic accuracy with meta analysis. Methods: Searched current literature in database without limiting time and language, evaluated the quality of included studies according to QUADAS(Quality assessment of diagnostic accuracy studies), MetaDisc version 1.40 software was used to pool sensitivity, specificity, positive likelihood ratio(+LR), negative likelihood ratio(-LR) and diagnostic OR, analysed source of heterogeneity with Q-test and drew SROC curves. Results: Nine studies met the inclusion criteria, eight literatures written in English and one in Chinese. 346 cases of cervical cancer(3979 lymph node metastasis) were included, we pooled data with random effects model on lymph nodes level: sensitivity, specificity, positive predictive value, negative predictive value, positive likelihood ratio, negative likelihood ratio, accuracy and AUC value were 0.58[95% confidence interval(CI) 0.51~0.65], 0.98 (95%CI 0.98~0.99), 0.686(0.464~0.852), 0.936(0.833~0.995), 2.15(7.20~64.24), 0.43(0.32~0.58), 0.900(0.75~0.993), 0.7724, respectively; pooled analysis with random effects model based on patient level: sensitivity, specificity, positive predictive value, negative predictive value, positive likelihood ratio, negative likelihood ratio, accuracy and AUC value were 0.54(95%CI 0.45~0.63), 0.86(95%CI 0.98~0.99), 0.725(0.471~0.923), 0.765(0.615~0.886), 4.58(1.86~11.31), 0.52(0.34~0.80), 0.756(0.620~0.0.897) and 0.8699, respectively. Conclusion: PET/CT has moderate diagnostic value for evaluating pelvic and aortic lymph node metastasis in cervical cancer, which can provide reliable guidance in clincal practice.

27 Value of plain radiography combine with ultrasonography in monitoring limb lengthening

Objective: To discuss the application value of X-ray and ultrasound in the evaluation of limb lengthening. Methods: An osteotomy was done on the tibia in 10 rabbits. After a 7-day latency period, distraction was commenced at a rate of 1mm daily. Lengthening was continued to a 30% increase over initial bone length and then left to consolidate for 5 weeks. At the osteotomy site the new callus formation and the blood circulation was monitored by X-ray and ultrasound. Results: X-ray: When the the limb was lengthened to 10%, there was obvious distraction occurred without any evidence of new bone formation within the distraction sites. When lengthened to 30%, much more callus at the lengthening area and the cortical bone appeared at both ends. Mineralized 3 weeks later, medullary cavity appeared. Ultrasound: In the early stage, the amount of the micrangium and blood flow increased and then decreased gradually. After 1 week, some speckled echogenic at the distraction site could be seen. When the limb lengthened to 30%, ultrasound can rarely penetrate the distraction gap. Conclusion: Traditional plain radiography and ultrasonography have their respective advantage and disadvantage. Evaluation of the newly formed bone with combination of the two methods is the cornerstone for terminating the limb lengthening successfully.
