77 Preliminary study of MR susceptibility-weighted imaging in cerebral vascular malformation

Objective: To discuss the value of susceptibility-weighted imaging(SWI) in the cerebral vascular malformations and the features of SWI. Materials and Methods: Forty-one patients(with cerebral vascular malformations including 23 cases of cavernous angioma, 13 cases of arterio-venous malformation, 5 cases of venous malformation) were examined with MR scanner. The sequences included SWI, T1WI, T2WI and post-contrast T1WI. The comparison of SWI and conventional MR sequences was done according to surgical findings and the imaging of DSA, then discuss the value of SWI in the diagnosis of cerebral vascular malformations. Results: SWI showed more lesions of cavernous angioma than conventional sequences, and SWI detected 3 cases of wide spread hemangiomas and 5 cases with multiple nidi. SWI displayed the same lesion larger than the conventional sequences. Five cases of venous malformation, 6 lesions are shown by SWI, and 5 shown by conventional sequences, and the medullary veins showed clearly, according to the post-contrast T1WI; SWI revealed 13 feeding arteries and 18 draining veins, and the conventional sequences showed the same number of feeding arteries and 13 draining veins. Conclusion: SWI is the best sensitive sequence for detecting cerebral vascular malformation, and the ideal method to screen the cerebral vascular malformations, it will provide more complete and detailed information combining with other sequences.

80 Preliminary study of susceptibility weighted imaging in venous hemorrhagic cerebral infarction on3.0T magnetic resonance

Objective: To explore the value of susceptibility weighted imaging(SWI) in the diagnosis of cerebral venous/venous sinus thrombosis combined with venous hemorrhagic cerebral infarction(CVST-VHCI). Methods: The imaging data of the 18 patients with CVST-VHCI proved by DSA or pathology were studied retrospectively on GE 3.0T MR system, and were compared with those of 12 patients with simple cerebral hemorrhage and 8 patients with brain tumor which located in nearly the same region. Result: In the 18 patients with CVST-VHCI, SWI showed patches of hypo-signal intensity in parietal cortex or parietal subcortex. There were wide spread or earthworm-like mesh cortex veins or pulp veins around the cortex or parietal subcortex. In the 12 patients with simple cerebral hemorrhage, SWI revealed lesions of low signal intensity. The cortex veins were normal or reduced slightly. In the 8 patients with brain tumor, SWI expressed clumping mixed-signal and linear hypo-signal small veins and patchy hypo-signal hemorrhage inside the tumor. Conclusion: For the hemorrhagic disease in parietal cortex or parietal subcortex, SWI can reveal the specific feature by showing expanded cortex vein or medullary vein to improve the diagnosis of CVST-VHCI. So SWI is an important supplement of cMRI and MRV.

84 Evaluation of MSCT application in prognosis and follow-up studies of 60 patients with active pulmonary tuberculosis

Objective: To investigate MSCT manifestations after the healing of active pulmonary tuberculosis, and estimate the value of CT in detecting prognosis and follow-up of pulmonary tuberculosis. Material and Methods: Sixty bacteriologically-cured patients with smear-positive pulmonary tuberculosis were included in this study, whose CT manifestations of pre-treatment, post-treatment and 6 months follow-up were observed. CT images were also acquired at the end of expiration. The small airway obstruction and air trapping signs were observed, and explored the correlation between the air trapping volume and pulmonary function parameters. Results: In pre-treatment, post-treatment and 6 months follow-up, the detection rate of activity signs in CT was 95.0%, 31.7% and 1.7%, respectively. The accuracy of CT diagnosing the activity of pulmonary tuberculosis was 95.0%, 68.3% and 96.7% in pre-treatment, post-treatment and 6 months follow-up respectively. The residual lesions were found to be reduced in size and absorbed after drug withdrawal for 6 months in 39 cases(76.5%). Small airway lesions still existed after treatment. The air trapping volume had an obvious correlation with FEF50% and MMEF(χ2=10.48, P<0.01). Conclusions: The CT manifestations of prognosis are demonstrated later than bacteriology. There is a limitation in assessing the activity of pulmonary tuberculosis only by CT signs. CT plays a role in demonstration of secondary small airway changes after bacteriologically cured.

89 Study of regional cerebral blood flow in alcohol dependants using statistical parametric mapping

Objective: To investigate the regional cerebral blood flow(rCBF) alternations in alcohol dependants, and to compare with normal subjects using statistical parametric mapping. Methods: Alcohol dependant group: 14 males, age 40~54 years old, diagnosed with SCID-AD, all of them come from mental health centre of Putuo district Shanghai. All people have neither body nor other mental diseases, and have never used any psychotropic drug or ceased for using such drugs for 4 weeks; control group: 20 males, age 38~54 years old, do not have any body or mental diseases. 99Tcm-ECD SPECT brain imaging were performed, all image transformed to DICOM or analysis 7 format with special program. Then the comparison situation of rCBF was analyzed by SPM. Results: The rCBF of bilateral frontal lobe, right cingulate gyrus, the body of right caudate nucleus were significantly decreased in alcohol dependants, and the rCBF of left inferior temporal gyrus, left middle temporal gyrus, bilateral precentral gyrus, left thalamus, left parahippocampal gyrus, right lentiform nucleus was significantly increased(P<0.05). Conclusion: SPM analysis showed that significant rCBF change in alcohol dependants.

91 Diagnostic value of contrast-enhanced ultrasound in the diagnosis of breast benign and malignant lesions

Objective: To evaluate the diagnostic value of contrast-enhanced ultrasound in the diagnosis of breast lesions. Methods: Sixty-two cases of breast lesions, in which 33 cases were benign, and 29 were malignant, were examined with contrast-enhanced ultrasound. The enhancement pattern, time-intensity curve and contrast parameters were analyzed to compare the differences between benign and malignant lesions. Results: Significant differences were found in homogeneity of the intensity, the initial enhancement time, peak time and the slope rate(all P<0.05). No significant difference were found in peak intensity, average intensity and areas under peak~60s of the time-intensity curves. The inhomogenity of the intensity, the time from initial enhancement<10s, peak time <20s and the slope rate<5 were set as the indicators for malignancy. The diagnostic accuracy of B-ultrasound and color Doppler flow imaging is 88.7%, when combined with contrast-enhanced ultrasound, the diagnostic value can be improved to 93.6%. Conclusions: Contrast-enhanced ultrasound can improve the accuracy of diagnosis for breast lesions.

94 Study of cardiac function on immature myocardium by high-frequency echocardiography in infant rabbit

Objective: To study the cardiac function of immature myocardium by high-frequency echocardiography in infant rabbit. Methods: Indices of left ventricular diastolic and systolic function were measured using M-mode and Doppler echocardiographic examinations. Stroke volume(SV), ejection fraction(EF), fraction shortening(FS) were measured and calculated. Mitral valve orifice flow velocity was recorded by Doppler flow imaging, peak early velocity(E) and late filling wave(A) were measured. DTI was applied in the pulsed wave Doppler mode to allow for a spectral display of the mitral annulus velocities, peak E wave of early diastolic(Ea) and peak A wave of late diastolic(Aa) at annulus were measured. Index myocardial performance(IMP) was calculated. Results: Left ventricular diastolic function was abnormal and systolic function was normal in infant rabbit. Conclusion: High-frequency echocardiography may be used to study the cardiac function on immature myocardium in infant rabbit.

97 Diagnosis of Ebstein’s anomaly with transthoracic echocardiography and multiplane transesophageal echocardiography

Objective: To explore the application value and the guidance for surgical operation of transthoracic echocardiography(TTE) and multiplane transesophageal echocardiography(MTEE) in the diagnosis of Ebstein’s anomaly. Methods: Preoperative TTE features in 13 cases of Ebstein’s anomaly and intraoperative MTEE features in 5 of them were analyzed retrospectively, comparison to the operation results was carried out. Results: Among the 13 cases, 1 case was diagnosed as Ebstein’s anomaly with solitary displacement of septal leaflet, 12 cases with displacement of septal and posterior leaflets by TTE. Among the 5 cases, 2 cases were diagnosed as severe displacement of the leaflets with functional right ventricle hypoplasia and thinning of the wall, 3 cases with severe abnormalities of the leaflets by MTEE. Among the 13 cases with different degree of abnormalities, 6 cases received valvuloplasty, 5 cases received valvular replacement procedure, 2 cases received bidirectional Glenn shunt procedure. Conclusions: TTE can diagnose Ebstein’s anomaly, MTEE can evaluate the function of tricuspid leaflets and right ventricle more accurate, if the two examination methods were combined together, they can help surgeon to choose the better operation methods for treating various degree of anomalies and improve the success rate.

100 Clinical value of prostatic biopsy under the guidance of transrectal power Doppler ultrasonography

Objective: To evaluate the clinical value of transrectal power Doppler ultrasonography(PDU) and PDU-directed prostatic biopsy to improve the diagnosis of prostatic cancer. Methods: Fifty-four patients suspected to be prostatic cancer underwent routine gray-scale TRUS, transrectal PDU of the entire gland before biopsy. According to the result, select the protocols of gray-scale TRUS, transrectal PDU and combined use of six-score transrectal biopsy protocols. The sensitivity, specificity, positive predictive value, negative predictive value were compared and analyzed. Results: Abnormal flow was detected by power Doppler scanning in 37 patients, among them 23 cases were of prostatic cancer(two of them were normal on gray-scale image), BPH in 11 cases, prostatic cancer cannot be ruled out in 1 case, prostatic mass of undetermined nature in 1 case, metastatic prostatic cancer of bladder origin in 1 case. Conclusions: Transrectal PDU can improve the sensitivity of cancer detection, and can determine the correct site for PDU-directed biopsy, the Gleason score is significantly higher.

103 Application of contrast-enhanced ultrasound in the diagnosis and differential diagnosis of renal diseases

Objective: To evaluate the value of contrast-enhanced ultrasound in differential diagnosis of renal occupied lesions. Methods: Thirty-one renal occupied lesions underwent conventional and contrast-enhanced ultrasound examinations with a bolus injection of SonoVue. The perfusion characteristic of ultrasound contrast agent was assessed and the results were compared with that of pathology. Results: The renal occupied lesions included 17 malignant lesions and 14 benign lesions. Enhancement patterns of contrast-enhanced ultrasound were showed in 11 malignant renal lesions characterized by fast washing-in and fast washing-out. The perfusion characteristic of benign renal lesions was slow(or simultaneously) washing-out. There were no perfusions of contrast agent during the whole procedure in the 3 cases of renal atypical cyst. Conclusion: The contrast-enhanced ultrasound is valuable in differenting benign and malignant renal occupied lesions.

106 Imaging analysis of synovial sarcoma of extremities

Objective: To discuss the imaging findings of synovial sarcoma of extremities and evaluate the value of X-ray, CT, MRI and DSA. Methods: Imaging data of 13 cases of synovial sarcoma of extremities confirmed pathologically was analyzed retrospectively. Results: Most synovial sarcomas located in the extremities close to joints. Synovial sarcomas detected by radiography typically appeared as round, oval or multilobulated juxtaarticular soft-tissue masses with calcification in some cases. The most common CT appearance of synovial sarcoma was heterogeneous deep-seated soft-tissue mass with attenuation similar to or slightly lower than that of muscle. On T1WI, synovial sarcomas typically appeared as prominently heterogeneous multilobulated soft tissue masses with signal intensity similar to or slightly higher than that of muscle. On T2WI, synovial sarcomas appeared as prominent heterogeneity with predominantly high signal intensity. Angiogram showed dense tumor staining and neovascularity. Conclusion: Synovial sarcoma has some specific image features. Comprehensive analysis of multiple imaging examinations and clinical features are helpful for the diagnosis of synovial sarcoma.

110 Imaging findings of chondromyxoid fibroma of bone

Objective: To explore the imaging features of chondromyxoid fibroma of bone and evaluate their clinical value. Methods: Eleven cases with chondromyxoid fibroma of bone confirmed by surgical pathology underwent radiography, CT plain scanning and MR SE-T1WI, SE-T2WI and SE-T1WI enhanced scanning before operation, the imaging data was reviewed and analysed retrospectively in comparison with surgical and pathological results. Results: The lesions in seven cases of 11 chondromyxoid fibroma with well defined osteolytic destruction were located at the metaphysis of the long tubular bone, in 2 cases the lesion was in pelvis, 1 case in skull, the other case in vertebrae. The diameter of the tumor ranged from 3.2cm to 16.9cm, with a mean diameter of 4.7cm. When located in a long tubular bone, chondromyxoid fibroma were generally eccentrically situated at the metaphysis, radiolucent and elongated in shape, cortical expansion, exuberant endosteal sclerosis, and coarse trabeculation, with small area of ring- and arc-like calcification, thin sclerotic rim separated the tumor from adjacent normal bone, there may be little periosteal reaction and small soft tissue masses. The CT value was similar to muscle and CT is optimal to detect the matrix mineralization. MR imaging depicted low signal intensity with well defined rim on T1WI, high signal intensity on T2WI. Conclusion: The imaging manifestations of chondromyxoid fibroma were specific to some extent. Combined utilization of plain X-ray, CT, and MRI is helpful for the diagnosis and differential diagnosis of chondromyxoid fibroma.
