Comparative study of SPECT between patients with depressive disorder and anxiety disorders

Objective: To study the characteristics of regional cerebral blood flow(rCBF) and the diagnostic value of brain SPECT in patients with depressive disorder and anxiety disorders. Methods: SPECT were performed in 21 depressive patients, 17 anxiety patients to compare the difference in cerebral perfusion. Region of interest(ROI) in cross-sectional images of the brain lobes were drawn, to semi-quantitatively analyse the rCBF changes of depression and anxiety disorders in patients. The t test was used to analyze the quantitative data. Results: Lower radioactivity rate in bilateral prefrontal lobes, temporal lobes, occipital lobes, bilateral parietal lobes, cingulated gyrus, caudate nucleus were demonstrated in the depressive patients(P<0.05 or 0.01). Lower radioactivity rate in bilateral frontal lobes, anterior temporal lobes and cerebral ganglion were demonstrated in the anxiety patients(P<0.05 or 0.01). Conclusion: The depressive disorder and anxiety disorders have obvious effect on cerebral blood flow. Radionuclide imaging have important value in the diagnosis and differential diagnosis of depressive disorder and anxiety disorders.

Voxel-based analysis of white matter during adolescents and young adults

Objective: To evaluate differences in white matter by voxel-based analysis in healthy adolescents and young adults. Materials and Methods: Echo-planar diffusion-tensor magnetic resonance(MR) imaging was performed in 3 groups of healthy subjects aged 11~13 years, 16~18 years and 23~25 years. Linear correlative analysis was performed for fractional anisotropy(FA) and mean diffusivity(MD) with age, as well as an intergroup t-test comparison for FA and MD maps. All tests on FA were performed using an absolute threshold of FA>0.15, and set all MD>0.001 to zero. Results: Significant positive correlation of FA with age was found in the internal capsule, the frontal and parietal white matter, the genu and body of the corpus callosum. Statistically significant negative correlation of the MD with age was found in the peripheral frontal and parietal white matter. The minimal overlap in decreases of the MD and increases of FA may indicate that they mainly reflect different physiologic processes. Conclusion: Diffusion-tensor MR imaging results indicate that white matter maturation assessed at different ages involves increases in both white matter density and organization during adolescents and young adults. The FA and MD may reflect different physiologic processes in healthy adolescents and young adults.

The value of 16-slice spiral CTA in the therapy of intracranial aneurysm

Objective: To evaluate the value of 16-slice CT angiography technique in the treatment of intracranial aneurysm. Materials and Methods: Thirty-eight cases of patients with intracranial aneurysm during June 2007 to June 2009 were reviewed, all cases were performed MSCT angiographies before and after surgery, DSA were performed in all cases before surgery and 4 cases after surgery. Pre- and post-operation MSCT angiography were performed within 10 days in 32 cases, and within 11~27 days in 6 cases. Result: There were 46 intracranial aneurysms in 38 patients; Clipping 40 aneurysms in 34 patients; embolysing aneurysms in 4 cases; another two were not treated. Forty aneurysms were diagnosed correctly by CTA, but 5 lesions were suspicious, and 1 lesion was missed. Two incomplete occlusions were found by post-operative CTA, residual neck was found in 1 case. Conclusion: 16-slice CTA was a non-invasive, effective and accurate method to evaluate the intracranial aneurysm and it could be easily accepted by patients. It has advantages not only in diagnosis of the intracranial aneurysms, preoperative selection of treatment, also in the postoperative follow-up of the intracranial aneurysm.

Relative study on ventricular asynchrony and left ventricular function in experimental acute myocardial ischemia of canines

Objective: To evaluate the relationship between ventricular asynchrony and left ventricular function in acute myocardial ischemia of canines. Quantitative tissue velocity imaging(QTVI) was used. Methods: Twenty-one canines were studied on QTVI, left coronary artery(LCA) was carefully dissected and occluded. Before LCA occlusion, the canines were defined as the normal control group and after occlusion defined as the experimental group. The regional velocity profiles in 12 segments of left ventricle(LV) were obtained. The time from onset of QRS to peak velocity during systole and early diastole(Ts and Te) were measured. The maximal difference in Ts and Te within the same wall(Intra-△Ts and Intra-△Te) and all 12 segments(Max-△Ts and Max-△Te) of the LV were calculated. The time from onset of QRS to beginning time of flow velocity spectra in aortic valve(Q-A) and pulmonary value(Q-P) and their differences(Q-AP) were measured. The parameters of LV pump function include the left ventricular ejection fraction(LVEF) and Tei index were calculated. Results: Intra-△Ts, Intra-△Te, Max-△Ts, Max-△Te, Q-A, Q-P and Q-AP were significantly prolonged compared with the control group. Max-△Ts was positively correlated with Tei index(P<0.05) and negatively with LVEF(P<0.05), Max-△Te was positively correlated with QRS duration(P<0.05). Conclusions: The inter- and intraventricular asynchrony exists in canines with acute left ventricular ischemia and it has relation with LV function.

Clinical research of dual source CT perfusion imaging in evaluating biological behavior of gastric carcinoma

Objective: To discuss the value of dual source CT perfusion imaging parameters in evaluating the biological behavior of gastric carcinoma. Materials and Methods: The perfusion parameters were obtained from patients who experienced the dual source CT scan. And they were divided into several groups according to the extent of cell differentiation of tumor after operation, invasive depths in the gastric wall and whether generating lymph node metastasis. Results: The BF and BV value in the distant metastasis and in the recent distant metastasis as well as the recent distant metastasis group were higher than that of no distant metastasis, meanwhile the evident difference was observed, P<0.05(F=6.555, 5.972). Besides, the value of Patlak P in the muscularis infiltration group was lower than that in the serosa external group and it had significant difference, P<0.05(F=5.460). And we concluded that PBV value was negatively correlated to the degree of cell differentiation, besides, positive correlation was observed between BV, BF, Patlak P, PBV value and invasive depths and the extent of distant metastasis. Besides, there were the positive relationship between BV, BF, Patlak P value and distant metastasis. Similarly, Patlak P value was positive related to the degree of lymph node metastasis. Conclusions: The dual source CT perfusion imaging parameters were able to demonstrate biological behaviors of gastric carcinoma. BF and BV value could evaluate distant metastasis and recent distant metastasis after operation or not objectively. Similarly, BV and Patlak P values were able to estimate the depth of the cancer tissue infiltrating the gastric wall objectively.

CT diagnosis of intestinal stromal tumors

Objective: To investigate the CT features of intestinal stromal tumors and its diagnostic value. Materials and Methods: The CT imaging data of 15 cases with intestinal stromal tumors confirmed by operation and pathology were analyzed retrospectively, the gross pathologic findings of the tumors were compared with the manifestations of CT. Results: Among the 15 cases, 5 cases located at duodenum, 4 cases at jejunum, 3 cases at ileum, 2 cases at colon, 1 case at multisegments of intestine and colon. All cases presented as celiac or pelvic soft tissue tumors with diameter of 2.7~24cm, 5 cases were less than 5cm and 2 cases of them were benign, 10 cases were larger than 5cm which were all malignant; Seven cases presented with a irregular margin; Twelve cases showed heterogeneous density on plain scan with slightly and moderately heterogeneous enhancement; Two cases had liver metastasis, 2 cases recurred; Four cases were preoperatively misdiagnosed, 6 cases were localized precisely and 5 roughly. Conclusion: Although precisely preoperative localization of intestinal stromal tumors is difficult, CT findings of tumor features and the relationship with adjacent tissues or organs can be of great help and also for evaluation of malignant or benign lesions.

Study of CT perfusion imaging and angiogenesis in pancreatic adenocarcinoma

Objective: To investigate the possibility of CT perfusion imaging applied to evaluate angiogenesis of pancreatic adenocarcinoma. Methods: CT perfusion imaging was performed in 21 cases of pancreatic adenocarcinoma and 11 healthy volunteers without pancreatic disease and perfusion parameters including BF and BV were obtained. The 21 cases of pancreatic adenocarcinoma and pancreatic normal tissue(normal tissue around adenocarcinoma) were all stained with streptavidin/peroxidase(SP) method. The VEGF and MVD of each sclice were quantified. Results: The mean CT values of pancreatic adenocarcinomatic perfusion including BF and BV were lower than that of normal pancreatic perfusion, and there was statistical significance(P<0.01). The expression of MVD and VEGF in pancreatic adenocarcinoma was higher than that in pancreatic normal tissue, and there was statistical significance(P<0.01). Conclusion: CT perfusion imaging of pancreatic adenocarcinoma could not truly reflect angiogenesis of pancreatic adenocarcinoma.

The application value of score by ultrasound elastography at fatty liver graduation

Objective: The aim of this study was to investigate the application value of score by ultrasound elastography in grading fatty liver. Methods: Seventy-seven patients with fatty liver were enrolled, and 30 patients with proven healthy livers were used as control group. After all patients with conventional ultrasound scanning, and following ultrasound elastography scan, we scored patients with fatty liver in accordance with ultrasound elastography score criteria. Results: There was a high consistency between ultrasound elastography score and traditional ultrasound score(Kappa value=0.833). The sensitivity, specificity of fatty liver evaluated by ultrasound elastography were 96.1%, 93.3% respectively. Conclusion: Ultrasound elasticity imaging technique can be used as a more objective auxiliary examination method in grading fatty liver.

An influence of biliary internal environment after metallic stent implanted through ampullae: a clinical study

Objective: To investigate the relationship between the stent implatation and the changes of microenvironment of the bile duct. Methods: Fifteen cases with low malignant obstruction of bile duct were implanted with metallic stent. The bile was gathered before and after the stenting, and then the volume of AMS, LPS, pH value was measured, and the germiculture was performed. Oral 99mTc was also performed after the biliary stenting. Results: The AMS/LPS was higher than before biliary stenting, and pH value was lower than before as well. The result of germiculture was positive. Oral 99mTc proved that the enteric bile reflux existed. Conclusion: Biliary internal environment was changed apparently after metallic stent implanted through ampullae. The enteric bile reflux and the pancreatic bile reflux should be considered when the biliary stent has to be implanted through ampullae.
