761 Initial study of 1H-MR spectroscopy in obstructive sleep apnea hypopnea syndrome

Objective: To investigate the cerebral metabolic changes in patients with obstructive sleep apnea hypopnea syndrome(OSAHS) by 1H magnetic resonance spectroscopy(1H-MRS), and to explore the value of 1H-MRS on OSAHS. Methods: 1H-MRS was performed in 26 patients with OSAHS and 12 control subjects. The patients were classified into two groups based on clinical manifestations and the results of polysomnography(PSG): mild OSAHS(10 patients) or moderate to severe OSAHS(16 patients). Cerebral metabolism was studied by assessing ratios of N-acetylaspartate(NAA) to choline(Cho) and to creatine(Cr) as well as Cho to Cr was calculated from the peak areas in normal-appearing regions of cerebral white matter or cortex selected. Results: In the patients with moderate to severe OSAHS, the NAA/Cho ratios for the white matter were significantly decreased and lower than that of the control subjects as well as mild OSAHS(P<0.01). The ratios of NAA/Cr and Cho/Cr had no obvious changes(P>0.05). However, no significant difference in the three metabolite ratios was found between patients with mild OSAHS and control subjects for cerebral white matter, as well as among three groups for cerebral cortex(P>0.05). None of the subjects exhibited an obvious lactate signal in brain tissue. There was a negative correlation between the NAA/Cho ratios for the white matter and apnea hypopnea index(AHI)(r=-0.36, P<0.01). Conclusion: 1H-MRS was sensitive to demonstrate the metabolic impairment in patients with moderate to severe OSAHS, and can provide valuable diagnostic and therapeutic information.

765 Ultrasonographic manifestations and pathologic study of fat necrosis of the breast

Objective: To discuss mammographic findings and pathologic basis of fat necrosis of the breast. Methods: The ultrasonographic findings in 33 cases with pathologically proved breast fat necrosis were analyzed retrospectively, of which 4 cases were bilateral breast fat necrosis, and a comparison with pathological results was made. Results: The sonographic findings included: isoechoic masses(n=5, 13.5%), hypoechoic masses(n=16, 43.2%), anechoic nodules(n=10, 27.0%), complex with mural nodules(n=6, 16.2%). Doppler sonography did not show vascularity in all lesions, some demonstrated sonographic ill-defined margins and calcification. Posterior acoustic shadowing and posterior acoustic enhancement could also be found in hypoechoic masses and anechoic nodules. Seven lesions lying in subcutaneous fatty tissues of breast, and 30 lesions located in the breast parenchyma. The pathologic interpretations: ①There is local adipose cell destruction and the development of vacuoles filled with necrotic lipid material, which surrounded by phagocytic lipid-laden histiocytes(“foam cells”), with an accumulation of multinucleared giant cells at the edges of the lesion and accompanied by an inflammatory cell infiltrate; ②Fibroblasts proliferate at the periphery of the lesion, surrounding areas of necrotic fat and cellular debris, calcifications may appear; ③Fibrosis may replace the areas of necrotic fat and debris or leave persistent cavities. Loculated and degenerated fat may persist within a fibrotic scar. These histopathologic appearances also can occur simultaneously with various degrees of intensity. The imaging features of fat necrosis of the breast reflect the intensity of each of these histological events at the time of the imaging study. Conclusion: Sonography is very helpful for making the diagnosis of breast fat necrosis.

769 Application of total choline compound quantification in differentiating benign and malignant breast tumors

Objective: To investigate the value of tCho concentration in differentiating benign and malignant breast tumors. Materials and Methods: Thirty-six breast tumors(27 breast cancers and 9 fibroadenomas) were studied. MR spectra of 36 breast tumors were obtained and tCho concentrations were quantified with choline phantom as the external standard. tCho concentrations of benign and malignant breast tumors were compared. ROC curve was used to analyze the value of tCho concentration in differentiating benign and malignant tumors. Results: The tCho concentrations of breast cancers and fibroadenomas were (4.28±2.93)mM and (0.09±0.27)mM respectively. Significant difference was found between them. The area under ROC curve was 0.979±0.24. Taking 0.99mM as the threshold, the sensitivity and specificity were 96.3%, 100% respectively. Conclusions: Differentiation between benign and malignant breast tumors with tCho concentration was applicable and for tumors larger than 1cm, the sensitivity and specificity were relatively high.

772 Evaluation of right ventricular systolic function in patients with atrial septal defect usingreal-time three-dimensional echocardiography

Objective: To evaluate RV systolic function in patients with atrial septal defect(ASD) using real-time three-dimensional echocardiography(RT-3DE). Methods: Eighteen patients with ASD and twenty-two normal persons were studied by acquiring full volume images on RT-3DE, then were analysed by software(4D RV-Function 1.1). End-diastolic volume(EDV), end-systolic volume(ESV), ejection fraction(EF) of the global right ventricle and the regional volume and EF of the segments were acquired. Results: EDV and ESV of global and regional right ventricle increased in ASD patients, global and regional right ventricle EF were decreased in ASD patients(P<0.05). Meanwhile the systolic tendency of right ventricle in ASD patients was differed from the normal persons. Conclusion: The RT-3DE is a precise technology not only in evaluation of global and regional right ventricular systolic function but also in discrimination of segmental function simultaneously.

775 Comparison of the diagnostic value of quantitative CT pixel indices and pulmonary function test in chronic obstructive pulmonary disease

Objective: To assess the diagnostic value of quantitative CT pixel indices(PI), correlated with pulmonary function test(PFT) in chronic obstructive pulmonary disease(COPD). Materials and Methods: This study included 48 patients with COPD(slight-moderate group and severe group) and 20 normal controls. All subjects underwent CT scanning of the lungs with normal respiration and PFT within one week. PI were analyzed by pulmonary quantitative software Pulmo and the parameters PI included -1024~-960, -960~-910, -910~-800, -800~-700, >-700. Analyze the correlation between PI and PFT, draw histograms of lung attenuation. Results: There was correlation between PI and PFT, there was significant correlation between PI-960~-910 and PFT. PI-960~-910 significantly increased in slight-moderate group and severe group as compared to control group, while PI-800~-700 and PI>-700 were significantly decreased(P<0.01). PI-1024~-960 and PI-910~-800 were different in three groups(P<0.05). Conclusion: PI was correlated with those data of PFT in normal respiration, which can evaluate lung function, and was combined with image element histogram to reflect the degree of COPD paitent.

779 Dynamic observation of aortic restenosis in rabbits by contrast-enhanced ultrasonography and common ultrasonography

Objective: To compare the value of contrast-enhanced ultrasonography(CEUS) and common ultrasonography in follow-up of aortic restenosis in rabbits after balloon injury and to find a new method to observe restenosis. Methods: Thirty rabbits underwent CEUS and common ultrasonography instantly, on the 3rd, 7th, 14th, 21th and 28th days after abdominal aorta was injuried by balloon. Diameter and systolic peak velocity(PSV) of the injured segment and normal segment 2cm proximal to the abnormality were measured after injury and restenosis rate was calculated. Results: ①The diameter of the lesion increased instantly after injury and decreased on the 7th day. The restenosis rate reached the peak on the 14th day, and it didn’t change afterwards. ②Light restenosis was detected by CEUS on the 7th day, earlier than common ultrasonography. Stenosis rate measured with CEUS was well related with pathology, and the correlation coefficient of it was higher than that of common ultrasonography.③CEUS displayed turbulence more clearly than color Doppler. Conclusion: CEUS could detect restenosis early and accurately. Restenosis rate measured with CEUS is more accurate than common ultrasonography and it can do great help in follow-up of restenosis.

782 Study of the characteristics of normal prostate and prostatic carcinoma bytransrectal contrast enhanced ultrasonography

Objective: To observe the characteristics and difference of the inner zone and outer zone of prostate gland, with the inner zone cancer and outer zone cancer by transrectal contrast enhanced ultrasonography. To provide more valuable information for early diagnosing of prostatic carcinoma. Methods: The ultrasound contrast agent was SonoVue(Bracco, Italian). Instrument adopted was Esaote company Technos DU8. The starting time and the ending time of transrectal contrast enhanced ultrasonography in the prostate inner gland(16 cases), the prostate outer gland(16 cases), the prostate inner gland cancer lesion(8 cases), and the prostate outer gland cancer(11 cases) were observed. Results: There was no significant difference between the starting enhanced time of the prostate inner gland and the prostate outer gland, no significant difference between the starting enhanced time of the inner gland cancer lesion and the outer gland cancer lesion too, but the starting enhanced time of the inner gland cancer lesion and the outer gland cancer lesion was earlier than that of the prostate inner gland and the prostate outer gland, the wash out time of the prostate inner gland, the prostate outer gland, the inner gland cancer lesion and the outer gland cancer had no difference. Conclusions: Transrectal contrast enhanced ultrasonography had important value in distinguishing the normal prostate and prostatic carcinoma, and is helpful in diagnosing early prostatic carcinoma.

784 Technetium-99m pertechnetate scan for ectopic gastric mucosa in children with gastrointestinal bleeding

Objective: To assess the utility of technetium 99mTc-pertechnetate scintigraphy in diagnosing of the pediatric patient with gastrointestinal bleeding and suspected of having Meckel’s diverticulum. Methods: 99mTc-pertechnetate abdominal scintigraphy was undertaken in 39 children with history of gastrointestinal bleeding. The final diagnosis of all cases was proved by surgery and pathology or gastroscope or the follow-up visit. Results: In detecting Meckel’s diverticulum, the sensitivity, specificity, accuracy, positive predictive value and negative predictive value of 99mTc-pertechnetate abdominal scintigraphy was 90.5%, 94.4%, 92.3%, 95.0% and 89.5% respectively. Conclusion: Radionuclide scintigraphy may be the first procedure to explore Meckel’s diverticulum.

787 Advantage of contrast-enhanced ultrasound in quantitative analysis of chronic renal failure

Objective: To discuss the value of contrast-enhanced ultrasound(CEUS) in evaluating renal function in patients with compensated chronic renal failure. Methods: Forty-five patients with chronic renal failure were examined by CEUS and color Doppler flow imaging(CDFI). The study also included examination of blood biochemistry. Result: CEUS showed arriving time(AT), time to peak(TTP) and peak intensity(PI) was significantly different in all groups(P<0.05). With increasing renal function failure, AT and TTP delayed, and PI decreased. During the compensatory phase of renal function, the renal blood flow showed high velocity and low resistence index. At the decompensatory stage, the renal blood flow showed slow velocity and high resistence index. Those appearances were more significantly between interlobar artery and arcuate artery than major renal artery. Serum creatinine(Scr) and serum urea nitrogen(BUN) of the decompensatory group were more significantly different compared with the other two groups(P<0.05), and showed higher. Conclusion: CEUS is more helpful in evaluating renal function of the compensatory chronic renal failure.

790 Correlation of MSCT obstruction index and results of blood gas analysis

Objective: To investigate the relationship between the pulmonary artery obstruction index with MSCT pulmonary angiography and results of blood gas analysis in patents with acute pulmonary embolism(APE). Methods: Thirty-nine patients with proven pulmonary embolism by MSCT pulmonary angiography who were free of underlying cardiopulmonary disease before were included in this study. Ten volunteers were recruited in the control group. The severity of pulmonary embolism was assessed by the Qanadli’s pulmonary artery obstruction index using post-processing technology of MSCT workstation such as VR, MPR and CPR. We assessed the correlation of the MSCT obstruction index with the values of PaO2, PaCO2, SaO2 and P(A-a)O2. Results: The values of PaO2, PaCO2, SaO2 were significantly lower(P=0.02, 0.004, 0.003 respectively) in the patients of APE than those in control group. A significant correlation was observed between the obstruction index and results of blood gas analysis[PaO2, r=-0.643, P<0.05; PaCO2, r=-0.771, P<0.05; SaO2, r=-0.718, P<0.05; P(A-a)O2, r=0.735, P<0.01]. Conclusion: The MSCT obstruction index is correlated to the parameters of blood gas analysis significantly in patients with APE. It can be an effective index to evaluate the severity of APE.

795 Application value of spatio-temporal image correlation technique in the ultrasonic examination of fetal heart

Objective: To study the feasibility and clinical value of spatio-temporal image correlation(STIC) technique in fetal heart examination using ultrasound. Methods: Acquire heart volumes using 4D ultrasonography with STIC at the level of the four-chamber view in 30 normal fetuses between 20 and 34 weeks of gestation, then analyze the data offline. Results: After using multi-planner mode to process the datasets, all target cardiac planes were acquired in 26 volumes, and some target cardic planes didn’t display in the other 4 cases. Conclusion: Comparing with traditional echocardiography, STIC has some advantages, and it is an accessible and reproducible methodology for clinical application.

799 Value of MRI in diagnosis of lumbosacral superficial fasciitis

Objective: To assess the MR characteristics of lumbosacral superficial fasciitis. Methods: Eighty-nine cases of patients(53 males and 36 females) with clinical diagnosis of lumbosacral superficial fasciitis because of low back pain or(and) leg pain were included in this study. MR examination of low back was performed on a 3.0T or a 0.35T MR scanner for all the patients. The scanning sequences included spin echo T1WI(SE T1WI), fast spin echo T2WI(FSE T2WI), short time inversion recovery(STIR) or spectral attenuated inversion recovery(SPAIR). Result: Lesions of long T1 and long T2 signal intensity which were high signal intensity on STIR and SPAIR with clear border were seen in the lumbosacral superficial fascia of all the 89 cases. Conclusion: MRI has high sensitivity and specificity in diagnosis of lumbosacral superficial fasciitis and may help in avoiding misdiagnosis and missed diagnosis.
