Analysis of the complications of malignant esophagus stenosis from stenting

Objective: To discuss the cause and treatment of the complications of malignant esophagus and gastric cardia stenosis by introducing the domestic-made nickel-titanium alloy coated mesh stent(plank) and the correlative plantation techniques. Methods: Twenty patients were treated with this method. The extent of stenosis were measured by esophagogram. The stents, about 4cm longer than the length of lesion, and 15%~20% larger than the normal esophagus above the lesion, usually 16~20mm were chosen. The stent was introduced to the chosen location under the guide of super-smooth wire, super-hard wire and catheter. The severe stenosis was expanded by balloon catheter before or after stent plantation. Results: Among the 20 patients, we successfully planted stent in 18 patients, and failed in 2 patients due to severe esophageal obstruction. The success rate is high up to 90%. Sixteen patients appeared pain behind the sternum for mass effect, 2 patients showed chest distress and dyspnea for tracheal bronchus compression, 2 cases had food incarceration, 1 case had bleeding, 1 had stent translocation, and 1 had restenosis. Conclusion: The stent plantation is an effective method for malignant esophagus stenosis, and can display the cause of complications. The correlative plantation technique is the key to reduce the incidence of complications.

CT findings of tuberculous lymphadenitis

Objective: To improve the understanding of CT imaging features of tuberculous lymphadenitis. Materials and Methods: Twenty-one cases of tuberculous lymphadenitis after pathologically confirmed, including 7 men and 14 women, the average age was 32 years(range from 18~67 years) were studied, all patients underwent enhanced CT scan. Results: Twenty-one patients with different parts of cervical, mediastinal and abdominal lymph node tuberculosis. The CT features could be divided into five types: ①iso-density enhancement; ②heterogeneous enhancement; ③corolla-like enhancement; ④separate ring-like enhancement; ⑤integration of multi-ring-like enhancement. Two or more features mixed together were seen in 16 cases. Eighteen cases showed various ring enhancement, accounting for 85.7%, a separate ring-like enhancement and/or integration of multi-ring-like enhancement were the most characteristic features. Conclusion: CT had certain characteristics of tuberculous lymphadenitis, and was the preferred method for diagnosing this disease.

Diagnosis of carcinoma of pancreatic head and ampulla with multi-slice CT

To investigate the imaging characteristics and diagnostic value of carcinomas of pancreatic head and ampulla with multi-slice CT. Methods: Thirty-nine patients confirmed by surgery and pathology were retrospectively studied, including 29 cases with carcinoma of the head of the pancreas and 10 with ampullary carcinoma, male patients 24, female 15, age ranged from 39~81 years; mean age 65 years. They all underwent the pre- and post-enhanced multi-phase CT examination before operation with Siemens Sensation 4 or 16 multi-slice scanner. On the base of transverse images and with multi-plannar reconstruction and other post-processing techniques, the imaging characteristics of the two groups were analyzed retrospectively and compared. Pre-operative diagnostic accuracy was evaluated statistically. Results: Main CT appearances of this group of cases: ①Ampullary carcinoma manifested as a small mass and was prone to protruding into duodenum, whereas the center of pancreatic head carcinoma was located in pancreas. ②Relationship between tumors and choledochopancreatic ducts and peripancreatic vessels were showed clearly. ③Getting together of the two ducts(distance between distal ends of the dilated common bile duct and the dilated main pancreatic duct≤5mm) was usually seen in ampullary carcinomas, while separation of the two ducts(distance between distal ends of the two dilated ducts>5mm) was mostly seen in carcinomas of pancreatic head. In our patients, 96.9% tumors of pancreatic head and 90% ampullary masses were displayed before operation by multi-slice CT. The pre-operative diagnostic accuracy for carcinomas of pancreatic head and ampulla with multi-slice CT was 93.1% and 90% respectively. Conclusion: Multi-slice CT can depict the imaging characteristics of carcinomas of pancreatic head and ampulla, and play an important role in pre-operative diagnosis and staging.

Preliminary clinical application of CT perfusion imaging in the evaluation of pre- and post-stenting in intracranial and extracranial arterial stenosis

Objective: To evaluate cerebral hemodynamics of pre- and post-arterial stenting for intracranial and extracranial artery stenosis by CT perfusion imaging. Methods: CT perfusion imaging was performed in 9 patients with intracranial and extracranial stenosis, the comparison of perfusion maps of MTT, CBF and CBV between pre- and post-stenting technique were observed. Results: Three of the 9 patients had single artery stenosis. In 1 case there was cerebral ischemic area, after stenting there was no improvement and in the other 2 cases no ischemic area was noticed either pre- or post-stenting. In 6 patients there were multi-arterial stenosis. In 4 patients the internal carotid artery was stenosed on one side with occlusion of the contralateral counterpart. Pre-stenting CT perfusion showed bilateral cerebral ischemia, 3 cases had ischemia on the occlusive side, no cerebral ischemia on the stenosed side. Post-stenting CT perfusion showed the case with bilateral cerebral ischemia had improvement on the stenting side but ischemia aggravated on the contralateral side. In the 3 cases with arterial occlusion, the ischemia aggravated in 2 cases and the other case had no change. There were no post-stenting complications. Conclusion: CT perfusion can show early hemodynamic changes after stenting. It is a simple and convenient technique for evaluating the cerebral hemodynamics after arterial stenting.

Value of the diagnosis of total abnormal pulmonary venous connection by echocardiography

Objective: The significance of the diagnosis of total abnormal pulmonary venous connection(TAPVC) by echocardiography is investigated. Methods: The echocardiography data of TAPVC were analyzed, Philips IE33 and ATL 3000 Ultrasonic diagnostic systems were used. Results: The characteristic changes were observed in all the cases where the right atria and right ventricles were enlarged, and the left atria(LA) were smaller than normal, there was not any entrance of pulmonary veins detected on the wall of left atria, and the common pulmonary veins were seen in the upper left of the LA. In all cases atrial septal defects(in 10 cases) and/or patent foramen ovale(in 6 cases) existed. Among the 15 cases, there were 8 cases(53.3%) with the supracardiac connection, 6 with cardiac connection, and the other 2 cases(13.4%) were undetermined. There were 2 cases accompanied by pulmonary venous obstruction in the supracardiac cases. Conclusion: There were characteristic signs in the echocardiography in the patients with TAPVC. The accurate diagonosis can be made. Echocardiograpy is significant to determine the sites of drainage of pulmonary veins and pulmonary obstruction.

Evaluation of SENSE MRV technique in lower limb veins

Objective: To explore the application value of the technique of SENSE MRV in lower limb veins. Methods: Without SENSE technique and with SENSE technique were performed in 34 patients suspected with venous diseases of the lower limb. Contrast enhancement were not used in all examinations. Results: All cases acquired excellent image and anatomic changes in the lower limb veins. The scan time with SENSE technique of reduction factor 2.0 in lower limb veins imaging decreased about 50% of the counterparts without SENSE technique and the contrast resolution on SENSE imaging almost kept the same as that on conventional sequence. Changing the parameters which enhance resolution as the same as scanning scope and time may obviously improve the picture quality. Conclusion: It offers a new approach to reduce the scan time at preserved spectral resolution, SENSE technique shows a widen promising in fast MRI and clinical application.

Ultrasound diagnosis and clinical value of single umbilical artery

Objective: To study the ultrasound and clinical diagnosis of single umbilical artery(SUA). Methods: 7623 pregnant women were examined by color Doppler ultrasound. If SUA was found, fetus was observed more carefully. Meantime, umbilical artery resistance indexs(RI) were measured, fetal weight was estimated. Results: Of the 7623 pregnant women, SUA was present in 10 cases, 5 of them were combined with fetal abnormality. RI enhancement was present in 3 of them. Lower fetal weight was present in 2 of them. All the diagnosis was confirmed by pathology. Conclusions: Ultrasonography can diagnose SUA correctly. SUA was relevant to many fetal abnormalities. A ultrasonic diagnostician should take more attention to umbilical cord vessels and SUA to give more message to clinical physicians.

Color Doppler ultrasonography in the diagnosis of common complications following puncture of femoral artery

Objective: To analyze the ultrasonic characteristic and its clinical application in the diagnosis of common complications including hematoma, false aneurysm and arteriovenous fistula following puncture of femoral artery. Methods: Forty-six cases with vascular murmur and/or a palpable mass in puncture site were observed by color Doppler ultrasonography. The echo characteristic and its flow state in the mass, the relation between the mass and the femoral artery were analyzed, the condition about the femoral artery and veins were also evaluated. Results: In the 46 cases, 21 cases were hematoma, no flow was found in the mass without clinical treatment; 16 cases were false aneurysm, the blood flow in the mass was coming from the femoral artery and the tear in femoral wall was found, 15 cases were pressed to close the aneurysm and 1 case was operated to repair the aneurysm; 9 cases were arteriovenous fistula, the fistula between the femoral artery and vein was detected, the shunt flow from the femoral artery to vein was detected during systole and diastole, 7 cases were pressed to close the fistula and 2 cases were operated to repair fistula. Conclusions: Color Doppler ultrasonography is a simple and accurate method for common complications following puncture of femoral artery, now it has been the preferred examination and offers an accurate information for clinical treatment.

Comparison of ovarian volume and follicle number between adolescent and adult patients with polycystic ovary syndrome

Objective: To compare ultrasonographic data(ovarian volume and follicle number) of adolescent and adult patients with polycystic ovary syndrome(PCOS). Methods: 358 PCOS patients(85 adolescents and 273 adults) were involved in this study. They underwent transrectal or transviginal ultrasound during the early follicular phase(dominant follicle excluded by ultrasonography) with a Toshiba Sonolayer SSA-220A(Toshiba, Tokyo, Japan) real-time sonography fitted with a mechanical 6MHz probe. Ovarian volume was calculated using the simplified fomula: 0.5×length×width×thichness and follicle number of bilateral ovaries were counted, t test was used for statistical analysis. Results: Compare adolescent PCOS group with adult PCOS group, the mean ovarian volume(MOV) and maximal ovarian volume(MaxOV) and mean follicle number(MFN) were (9.33±3.87)cm3 versus (10.00±4.97)cm3, P=0.83; (11.04±4.82)cm3 versus (11.89±7.42)cm3, P=0.86; (12.93±2.90) versus (13.60±2.99), P=0.36 respectively. Conclusion: Ovarian volume and follicle number by ultrasonic inspection between adolescent and adult patients with PCOS showed no difference.

Percutaneous excision of multiple breast fibroadenomas using ultrasound-guided Mammotome vaccum-assisted hand-held device

Objective: To explore the clinical value of minimally invasive treatment of multiple breast fibroadenomas using high frequency ultrasound-guided Mammotome vaccum-assisted hand-held device. Methods: One hundred and fourteen breast fibroadenomas in 37 patients were removed by Mammotome biopsy system. The results of high frequency ultrasound guidance and treatment were evaluated. Results: The needle position in 114 lesions was all visualized and real-time ultrasound guidance was successful. Fibroadenomas(5~18mm) were completely excised in all 37 patients. No other complications occurred except 1 case of subcutaneous ecchymosis and 1 case of small hematoma. Conclusion: For breast fibroadenomas 5~18mm in size, high frequency ultrasound-guided Mammotome excision is minimally invasive and significantly effective in suitable patients.

Clinical application of brown algae-microballs particles and gelfoam in partial splenic embolization for hypersplenism in cirrhosis

Objective: To evaluate the efficacy and complications of partial splenic embolization(PSE) using brown algae microballs and gelfoam as embolic materials for hypersplenism in cirrhosis. Methods: Seventy-four patients with hypersplenism and cirrhosis were treated by PSE and followed up for 1 year. According to the difference of the embolic material in PSE, 60 patients were divided into 2 groups. Group A: 22 patients with gelfoam cubes as the embolic material of PSE and group B: 38 patients with brown algae microballs. The follow-up indices included peripheral blood cell counts(WBC, PLT, RBC) and the complications associated with PSE. Results: In group A and B, WBC and PLT counts kept significantly higher in number than pre-embolization from 2 weeks to 1 year after PSE(P<0.001). Though WBC and PLT counts reached higher level after PSE in group B than that in group A but without significant difference(P>0.05). RBC counts did not show significant changes after PSE within 1 year follow-up(P>0.05). The most frequent side-effects were belly ache and fever. The degree and duration of belly ache were higher and longer in group B than those in group A, but the occurrence rate of fever was lower in group B. The occurrence rate of severe complications showed no significant differences for the two groups. The increase of embolization degree may aggravate post-embolization side-effects and raise the incidence of complications. Conclusion: Brown algae microballs may be more safe than gelfoam as the embolic material for PSE.

Multi-slice CT enterography in the diagnosis of small intestinal diseases

Objective: To investigate the value of multi-slice CT enterography(MSCTE) in intestinal diseases. Methods: Helical CT scans were performed in 36 patients before and after infusion of contrast medium. Before CT examination, all patients were injected 10~20mg 654-2 to minimize bowel movement then took administered iso-osmotic mannitol to fill the bowl. Multiplanar reformation(MPR), maximum intensity projection(MIP) were carried out. Results: Among 14 cases of intestinal neoplasms, there were 4 cases of adenocarcinoma, 6 cases of stromal tumor, 2 cases of lymphoma, 1 case of adenoma and 1 case of carcinoid tumor, 3 cases of Crohn’s diseases, and 3 cases of postoperative adhesion. The others were normal. Conclusion: MSCTE using biphasic enhancing and 3D-image reconstruction techniques was valuable for intestinal diseases.

Analysis of MR findings and differential diagnosis in spinal postoperative complications

Objective: To analyse MR findings in spinal postoperative complications and investigate the clinical applicative value of MRI in the diagnosis and differential diagnosis. Methods: MRI data of 114 cases of spinal postoperative complications were analyzed retrospectively and compared with the clinical data. Results: The main spinal postoperative complications includes: spinal stenosis(n=33, 21 cases of epidural fibrosis, 13 cases of epidural hematoma or epidural abscess); lack of stability(n=43); infection(n=23, 7 cases of para-spinal soft-tissue infection, 5 cases of vertebral discitis, 4 cases of vertebral and appendix infection, 3 cases of epidural abscess, 2 cases of myelitis, 2 cases of spinal arachnoiditis) and other(n=28, 12 cases of inner fixation failure, 9 cases of epidural hematoma, 7 cases of cerebrospinal fluid gusher). Conclusion: MRI can specifically display all kinds of spinal postoperative complications, and has a significant value in diagnosis and differential diagnosis.

Synthetic methods research of 18F-FLT as phase transfer calalyst by ionic liquid with one pot methodology

Objective: The synthesis 18F-FLT with one pot method used 1-Butyl-3-methylimidazolium trifluoromethanesulfonate(BMI) ionic liquid(IL) as phase transfer catalysis(PTC). To develop the simple, quickly, stability synthesis 18F-FLT method for clinical application. Materials and Methods: To separate 18F ionic transferred from cyclotron first. We study 18F-FLT productivity with the different K2CO3 volume, heat temperature and precursor quantity used 3-N-tert-Butoxycarbonyl-[5’-O-(4, 4’-dimethoxytripphenylmethyl)-2’-deoxy-3’-O-(4-nitrobenzenesulfonyl)-b-D-threopentofuranosyl] thymine as precursor and BMI as PTC. Results: We can get the highest yield of 18F-FLT synthesis with 0.1ml 1M, 130℃ and 30mg precursor. Conclusions: BMI IL can be used PTC to synthesize 18F-FLT which has fast higher yield and high stability.
