361 The diagnostic value of CT in the orbit of Graves’ disease

Objective: To investigate CT features of the orbit of Graves’ disease(GD) and explore the value of CT in clinical diagnosis and treatment in patient with GD. Methods: The manifestations of CT scan in 68 cases(136 orbits) with GD were studied in respect to the size and density of the extraocular muscles, exophthalmos and other manifestations. Results: The extraocular muscles in patients with GD were significantly larger than those in normal person (all P<0.05). Among the enlarged extraocular muscles, medial rectus was more frequently involved(67.6%), inferior rectus(58.8%) and superior rectus(45.6%) were the next, the last was lateral rectus(30.9%). In all 136 eyes, there were 118 eyes with exophthalmos. Among the 118 eyes with exophthalmos, there were 97 eyes with degree I exophthalmos and 21 eyes with degree Ⅱ exophthalmos. The detecting rate of exophthalmos was 86.8% in CT, but 66.9% in clinical diagnosis. The density change of the extraocular muscles and other characteristic manifestations were found in patients with GD. Conclusion: Orbital CT scan can become a routine examination and can be useful to clinical diagnosis and treatment in patients with Graves’ ophthalmopathy(GO).

365 Assessment of right ventricular function in patients with pulmonary hypertension by Doppler tissue imaging

Objective: To assess systolic and diastolic function of right ventricle in patients with pulmonary hypertension by Doppler tissue imaging(DTI) technique. Methods: The patients with pulmonary hypertension were divided into three groups. The motion of tricuspid annular of patients were analyzed by DTI and the velocity of anterior, septal and posterior of tricuspid annular, systolic velocity(Sa), early diastolic velocity(Ea), late diastolic velocity(Aa), were measured. Meanwhile, these parameters were compared with control group. Results: Compared with control group, Sa of anterior and septal tricuspid annular were decreased in groupⅡ, Ⅲ(P<0.05). In group Ⅲ, Sa of anterior tricuspid annular was worse than groupⅠ(P<0.05). Sa of posterior tricuspid annular, Ea of anterior tricuspid annular were lower than those of control group(P<0.05). Meanwhile, Ea of septal tricuspid annular were lower than those of normal group and groupⅠ(P<0.05). In group Ⅲ, Aa and Ea/Aa of septal and posterior tricuspid annular were lower than those of normal group(P<0.05). In groupⅡ and Ⅲ, Ea/Aa of anterior tricuspid annular were lower than those of control group(P<0.05). Conclusion: Pulmonary hypertension lead to moderate or severe dysfunction of right ventricle, systolic dysfunction was involved earlier than those of diastolic function.

368 Evaluating left ventricular function in congestive heart failure before and after treatment assessed by Doppler tissue imaging

Objective: To analyse the correlation of parameters between left ventricular systolic function, functional class, and the systolic and diastolic function of left ventricle before and after treatment by Doppler tissue imaging(DTI). Methods: Thirty-four consecutive patients with a clinical diagnosis of congestive heart failure were studied by DTI. Four sites at mitral annuli were selected corresponding to the septal, lateral, anterior and inferior walls of the ventrical from apical 4- and 2-chamber views. We detected wall motion of mitral valve annulus and recorded DTI indexes included the peak systolic velocities(Sa), the peak early and late diastolic velocities(Ea and Aa). Sa/Ea and VMS were calculated. Thirty-four normal subjects were selected as control group. Results: Sa derived from the lateral site of mitral annulus from apical four-chamber view correlated well with functional class(rs=-0.65, P<0.0001), the 4-site average velocities of mitral annulus correlated better with functional class(rs=-0.69, P<0.0001). Compared with healthy controls, Sa and Ea of all 4 sites were significantly decreased(P<0.001), Ea/Sa significantly increased(P<0.01). After treatment, Sa and Ea of all 4 sites were significantly increased(P<0.001), Ea/Sa significantly decreased(P<0.01). Conclusions: Left ventricular systolic and diastolic function could be quantitatively analyzed by DTI, there are more close correlation between the 4-site average velocities of mitral annulus VMS and NYHA functional class.

372 Preliminary clinical study in side effect and diagnostic value of adenosine 201Tl myocardial perfusion imaging for detecting coronary artery disease

Objective: The aims of this study were to evaluate the side effect and diagnostic value of adenosine 201Tl myocardial perfusion imaging for detecting coronary artery disease(CAD). Methods: A total of 21 patients with suspected CAD were submitted with adenosine stress myocardial perfusion imaging. Adenosine was injected by minipump(140 microgram per kg body weight per minute). The total time of adenosine infusion was 6 minutes. At the third minute of adenosine infusion, a bolus of 1.295×108Bq thallium-201 was injected, and SPECT acquisition of burden imaging was obtained at 2 minutes after completion of adenosine infusion. After 2~4 hours redistribution imaging was obtained. Coronary angiography (CAG) was performed in all patients within 2 weeks. Results: In adenosine stress myocardial perfusion imaging, the sensitivity, specificity, positive predictive value, negative predictive value, accuracy were 92.86%, 57.14%, 81.25%, 80.0%, 80.95% respectively. The positive diagnostic rate of single, two and three branches lesions were 83.33%, 100% and 100% respectively. Total positive diagnostic rate was 92.86%. Conclusion: Domestic adenosine stress myocardial perfusion imaging is a safe and reliable diagnostic method for CAD. It has mild side effects, high sensitivity and accuracy in CAD diagnosis.

376 MRI diagnosis of spinal cord hemangioblastoma

Objective: To explore the MRI characteristics of hemangioblastoma of the spinal cord. Methods: MRI appearance of 12 spinal cord hemangioblastoma patients with surgically and pathologically proved were analyzed restrospectively. Results: In the 12 cases of spinal cord hemangioblastomas, 3 case had multiple lesions, one patient also had intracranial hemangioblastoma. The lesions were located in cervical spinal cord in 7 cases, thoracic spinal cord 3 cases, cervical thoracic segement in 1 case, thoracic lumbar segement in 1 case. MRI appearance: the signal on T1WI of the tumors showed iso-hypointense in contrast to the normal spinal cord, higher intensity(n=1) than blood; and mixed hyper-hypointense signal on T2WI. After enhancement, nodular tumors showed marked enhancement with distinct border on T1WI. The blood flow-void-sign(n=11) within and around the masses was seen on T2WI or on contrast scans. There were secondary syringomyelia or cystic changes, above or below the tumor in 10 cases. Conclusion: MRI is an efficient method in the diagnosis of spinal cord hemangioblastoma. It plays an important role in the diagnosis and differential diagnosis of hemangioblastoma.

379 Percutaneous closure of multi-fenestrated ASD under echocardiography

Objective: This study reports our clinical experience of interventional treatment of multi-fenestrated atrial septal defect(ASD) under echocardiography. Methods: From December 1998 to March 2005, 11 patients, 2 males, 9 females, with mean age of 26.1±4.2 years(ranged from 4 to 60 years old), underwent percutaneous closure of multi-fenestrated ASD. Out of the 11 patients, 8 had two defects, 2 had three defects, 1 had four defects. Two of the 11 patients associated with atrial septal aneurysm, one with atrial fibrillation and hydropericardium. The frequency of detecting probe used was 2.5MHz and 5.0MHz respectively. Diagnosis was determined with transthoracic or transesophageal echocardiography before procedure, procedure was performed under support of transesophageal echocardiography. Transthoracic echocardiography(TTE) was used on the second day after the procedure and during follow-up. TTE mainly observed in parasternal, cardiac apical, inferior xiphoid process four chamber view, and inferior xiphoid process couple atrium, great artery minor axial views, and TEE mainly reviewed in atrial septal long axis, superior vena cava long axis, inferior vena cava, great artery minor axis, four chamber view. Results: Nine of the 11 patients were successfully treated with single occluder(device size ranged from 12 to 40mm), two were implanted with two devices for effective closure of all defects(occluder diameter 18 and 8mm). During follow-up, residual shunting was observed in two of the 11 patients, and no other complications were noted. Conclusions: Multi-fenestrated ASDs should be closed under transesophageal echocardiographic guidance to determine number, diameter and distance of ASDs. We should try our best to close multi-fenestrated ASDs with one device.

381 Diagnostic value of MRI in hepatic hydatid disease

Objective: To study the MRI features of hepatic hydatid cyst, to evaluate MRI in its diagnosis. Methods: MRI features in 23 patients with surgically-proved hepatic hydatid disease were retrospectively analyzed, and compared with pathologic findings. Results: All twenty-two but one cases were correctly diagnosed. Preoperative diagnostic accuracy of MRI was 95.65%(22/23). Conclusion: MRI is of high value for the diagnosis of hepatic hydatid cyst. It should be regarded as an effective supplement to CT and sonography.

385 Relation between renal dynamic imaging and pathological changes in IgA nephropathy

Objective: To analyze the relation between renal dynamic imaging and pathological changes in IgA nephropathy (IgAN). Methods: Sixty-seven histologically confirmed patients with IgAN were included in this study. According to the extent of renal pathological changes, such as glomerular lesions, tubulo-interstitial lesions and vessel lesions, all patients were divided into several groups according to Katafuchi’s score system and Lee’s histological classification. All of the 67 patients underwent renal dynamic imaging with 99mTc-DTPA (diethylenetriaminepentaacetic acid). And the imaging parameters including perfusion index(PI), glomerular filtration rate(GFR) and 20min clearance(C20) were calculated. The relationship between pathological Katafuchi’s scores and imaging parameters among all Lee’s groups were analyzed. Results: The results indicated that there was a positive correlation between PI and the vessel lesion score (P<0.01), negative correlation between GFR and the glomerular lesion score (P<0.01) and tubulo-interstitial lesion score (P<0.05), and negative correlation between C20 and tubulo-interstitial lesion score (P<0.05). With the Lee’s grade increasing, GFR and C20 gradually decreased, and PI increased. The differences of these imaging parameters among Lee’s groups were significant(P<0.05). Conclusion: The results of renal dynamic imaging closely relate to the renal pathological changes of IgAN, and can better reflect the degree of pathological lesions. Renal dynamic imaging is of practical value in the decision of therapy procedure and evaluation of prognosis of IgAN.

388 CT diagnosis and staging of nephroblastoma in children

Objective: To evaluate the value of CT in diagnosing and staging of nephroblastoma in children. Methods: Clinical and CT manifestations of nephroblastoma in 27 cases with integrate imaging material proved operatively and pathologically were summarized. Results: Thirty-six primary tumors were found in 33 kindeys, 11 in the right kidney, 10 in the left kidney, and 6 in both kidneys. The pathological result of the 33 cases includes epithelial type(10 cases), blastemal type(7 cases), stromal type(9 cases), composite type(7 cases), anaplastic type(3 cases), The preoperative staging of the tumor: stage I in 9 cases, stage II in 7 cases, stage III in 3 cases, stage IV in 2 cases, and stage V in 6 cases. The accurate rate of tumor staging was nearly 88.89%. Conclusions: The CT inspection can show edge, site, size of the tumor, and accurately evaluate tumor invasion, metastasis. It has important value to determine the nature, staging, following-up of nephroblastoma.

392 The evaluation of transvaginal ultrasonography in the diagnosis of tubo-ovarian abscess

Objective: To diagnose tubo-ovarian abscess(TOA) by transvaginal ultrasound, improve the diagnostic veracity and guide the choice of treatment. Materials and Methods: 186 cases of TOA patients had transvaginal ultrasonography, morphous and blood flow of the mass were detected. Results: In 158 of the 186 cases transvaginal ultrasound clearly showed adnexal mass and made a correct diagnosis accordingly. Accuracy was 85%. Blood flow inside the mass was detected in 58 cases and blood flow arround the mass was detected in 72 cases. Treatment planning was determined according to the size of mass. Conclusions: Transvaginal ultrasound is a convenient, effective and non-invasive method in TOA diagnosing, it is one of the best choices in clinical work.

394 MRI findings of subacute combined degeneration of the cervical spinal cord

Objective: To analyse the MR manifestations of subacute combined degeneration of the cervical spinal cord(SCD), and to improve the understanding of SCD. Methods: The findings of MRI were analyzed retrospectively in 5 patients with clinically confirmed SCD. The clinical symptoms were also analyzed. Results: All five cases demonstrated high signal intensity on T2WI in the posterior and/or lateral column of the cervical spinal cord, and ribbon-like high signal intensity along dorsal aspect of the cervical spinal cord on sagittal T2WI. Patchy enhancement were noted in 2 cases. Conclusion: The characteristic appearance of SCD in cervical spinal cord on MRI is very helpful in diagnosis.

397 Imaging diagnosis in giant cell tumor of bone

Objective: To analyse the imaging appearances and to evaluate the diagnostic value of X-ray, CT and MRI in giant cell tumor of bone. Methods: Seventy-six cases of various giant cell tumor proved by pathology, their imagings features were analyzed. Result: Twenty-nine cases of cystic type had eccentric expanding destruction of bone at the end of long bone extremities. 39 cases of osteolytic type had discontinuous cortex of bone and blurred tumor margins. Eight malignant cases had moth eaten destruction of the bone with periosteal reaction and fast-growing soft tissue mass. Liquid-liquid level in MRI imaging were found in 19 cases with aneurysmal bony cyst. In 9 cases there were recurrence, the bony implant area was replaced by tumor tissues. Conclusion: Most of the GCT have typical radiological appearances and there is no difficult in making a correct diagnosis, CT and MRI supply more information and the capability of differential diagnosis may be improved.

401 CT diagnosis of giant lymph node hyperplasia

Objective: To study CT characteristics of giant lymph node hyperplasia and their pathologic basis. Methods: Seven cases of pathologically proved giant lymph node hyperplasia were examined by plain and enhanced CT scanning. Results: In 5 cases with localized type of giant lymph node hyperplasia, which belonged to hyaline vascular type pathologically, the lesion presented as a solitary round or oval, iso-density soft tissue mass with smooth border. After contrast administration, obvious homogeneous sustaining enhancement was observed. The 2 cases with disseminated type, belonged to plasma cell type pathologically, the lesion presented as many dissimilar size nodules, inhomogeneous density, indistinct border, the periphery fat plane was fuzzy, with mild moderate non-uniform enhancement, 1 case showed incomplete ring-like enhancement. Conclusion: CT findings of giant lymph node hyperplasia are closely associated with its pathology. For localized type(hyaline vascular type), it is possible to make a correct diagnosis preoperatively. For disseminated type(plasma cell type) carries no specific CT signs, its verification is mainly based on pathologic examination.

404 The technique and value of 16-slice spiral CT venography in lower extremity thrombosis

Objective: To evaluate diagnostic value of 16-slice spiral CT venography(MSCTV) in detecting venous thrombosis of the lower extremity. Methods: The imaging diagnosis of patients with venous thrombosis of lower extremity was performed on MSCT and processed the data on ADW4.1 workstation. The data of MSCTV were reconstructed with maximum intensity projection(MIP), multiplanar reformation(MPR), volume rendering(VR). Result: Twenty-six cases suspected of having venous thrombosis of the lower extremity were demonstrated successfully with MSCTV. It could clearly reveal the location and extension of the lesion. The pathological changes in MSCTV included swelling of lower extremity, filling defect of vascular lumen, single and/or multiple stenosis and occlusion, forming of collateralization. Conclusion: MSCTV was valuable to demonstrate venous thrombosis of the lower extremity. MSCTV was better than Doppler ultrasonography and DSA in diagnosing venous thrombosis of the lower extremity. MSCTV may be useed as the first choice technique in diagnosing venous thrombosis of the lower extremity.
