Correlative study of MR perfusion weighted imaging with histopathologic grade in glioma

Objective: Relative cerebral blood volume(rCBV) is a commonly used perfusion MRI technique for the evaluation of glioma grade. Relative cerebral blood flow(rCBF) has less been studied. The goal of our study is to determine the usefulness of two parameters in evaluating the histopathologic grade of glioma. Methods: This study involved 36 patients(23 high-grade and 13 low-grade) who were identified as having histopathologically proven gliomas. MR perfusion imaging and MR conventional imaging were performed preoperatively in them. The perfusion data were deconvoluted to get a color map of CBV and CBF. The maximal rCBV and rCBF ratios of each lesion were calculated and were correlated with histopathologic grading of gliomas. Results: The maximal rCBV and rCBF ratios of high-grade gliomas were statistically different from those of low-grade gliomas(P<0.01). There was a strong correlation between the maximal rCBV and rCBF ratios(r=0.812, P<0.05). There was frequent mismatch(30%) between the qualitative assessment of the contrast-enhanced T1 weighted MR imaging and the perfusion maps. Conclusion: MR perfusion imaging is useful in the preoperative assessment of the histopathologic grade of gliomas. The maximal rCBF ratio in addition to the maximal rCBV ratio can be incorporated in MR perfusion analysis for the evaluation of cerebral gliomas.

CT diagnosis of epidemic encephalitis B and clinical value

Objective: To make a further understanding of the manifestations of epidemic encephalitis B on CT images, and to improve its diagnostic accuracy. Materials and Methods: We analyzed the clinic data of 27 cases, 4 cases in general type, 19 cases in heavier type, 4 cases in the heaviest type. Among 27 plain CT, enhanced CT in 5 cases. Results: Abnormalities located mainly in cerebral cortices, nuclei basales and thalamus, the lesions presented as low density on CT scan, 1 case was finding in truncus encephalicus and hemorrhage in 1 case. Conclusion: The findings accord with pathological change of epidemic encephalitis B and CT can provide important information for clinical diagnosis as a means of no trauma.

Multi-spiral CT in the diagnosis of gastric stromal tumors and correlation with pathology

Objective: To improve imaging recognition of gastric stromal tumors(GST) and appreciate the value of multi-spiral CT(MSCT) scan. Methods: The clinical and imaging data of 17 cases with GST proved by pathology were reviewed retrospectively. Results: The lesions included endo-luminal growth in 4 cases, extra-luminal growth in 6 cases and duplex-growth in 7 cases. MSCT showed homogeneous or inhomogeneous density, which associated with necrosis, cystis formation and calcification. In enhanced cases, lesions displayed different enhancement. Small vessels arranged in clumps or in line around the tumor during the arterial stage and the enhanced extent of venous stage was more obvious than that of arterial stage. Sign of gastric wall thickening close to lesions was abscent. The accurate rate of localization was 76.5%(13/17) compared with operation. Conclusion: Extra-luminal growth, duplex-growth and absence of gastric wall thickening are characteristic findings of GST in MSCT. The enhanced extent of venous stage superior to that of arterial stage is another feature. Reconstruction of MSCT defines the site, inner ingredients and adjacent tissues of lesion is beneficial to surgery.

CT findings of sacroiliac joints in ankylosing spondylitis

Objective: To assess the CT findings of sacroiliac joints changes in ankylosing spondylitis(AS), CT scanning were performed in 31 cases with AS proved clinically. Materials and Methods: CT findings of sacroiliac joints who were clinically diagnosed as AS were analysed. Results: Thirty of the 31 cases had abnormal CT findings of the sacroiliac joints. Five cases belonged to grade Ⅰ?(16.1%), 7 cases were grade Ⅱ(22.5%), 16 cases were grade Ⅲ(51.6%), 2 cases were grade Ⅳ(6.5%). In the early stage, sacroiliac joint of most cases had symmetric irregularity of articular surface, subcortical microcysts and mild sclerosis of iliac surface of joint. In advaced stage, sacroiliac articular surfaces were irregular with subcortical cystic destruction, showing serration or brush like deformity, sclerosis or hypertophic changes, and pseudo-widening or narrowing of joint space. Bony ankylosis of the joints with obliteration of joint space and osteoporosis or osteosclerosis were seen in the late or stable stage. Conclusion: CT scanning of sacroiliac joints could be helpful in diagnosis of AS, and helpful in clinical evaluation of the disease.

Curved M-mode strain rate imaging for noncompaction of the ventricular myocardium

Objective: To discuss the diagnostic value of noncompaction of the ventricular myocardium using tissue Doppler curved M-mode strain rate imaging(CMM-SRI). Methods: Consistency of the noncompactional ventricular myocardium contraction were observed and analyzed in 5 patients respectively in apical four-chamber view, two-chamber view of the left heart and apical long axis view of the left heart using CMM-SRI. Results: Curved M-mode strain rate profile recorded from left ventricular lateral wall and left ventricular posterior wall of 5 patients during 2 cardiac cycles. There were discrete paradoxical bands, adjacented to one another, of expansion and compression throughout both systole and diastole. There was no coordinated systolic compression or diastolic expansion wave form. Normal curved M-mode strain rate profile recorded from left ventricular wall of age-matched controls during 2 cardiac cycles. Here, uniform systolic compression or systolic strain rate was seen and 2 distinct expansion profiles were seen in early(E) and late(A) diastole. Conclusions: High frame-rate tissue Doppler CMM-SRI technique appears to be a promising tool to provide the phase information, and reveals unique appearances of paradoxical alternating bands of compression and expansion throughout systole and diastole in patients with noncompaction of the ventricular myocardium.

Clinical value of ultrasonography in lower extremity blood vessel lesion of type 2 diabetes

Objective: To evaluate the diagnostic rate and value of CDFI in the diagnosis of lower extremity blood vessel lesion in type 2 diabetes. Methods: Ninety-one cases of type 2 diabetes patients and 41 cases of nondiabetes patients simultaneously examinated by CDFI and ABPI were observated. Results: Diabetic arteriosclerosis was diffuse or multisegmental while simple arteriosclerosis showed focal in bifurcation of blood vessel. Blood vessel lesion incidence in type 2 diabetes was significantly higher than control group(P<0.01). The rate of arteriosclerotic plaques was highest in femoral artery, stenosis and occlusion were mostly appeared in anterior tibial artery(P<0.05). Conclusions: CDFI is an effective method in detecting lower exteremity vasculature in type 2 diabetes patients and evaluating location, character and length of the lesion, and has important clinical values.

Bcl-2 expression in osteosarcoma after chemotherapy: correlation with radiographic change and prognosis

Objective: To correlate bcl-2 expression after chemotherapy in osteosarcoma with radiographic change and prognosis. Methods: Large dose of MMIA chemotherapy was given to 23 patients with pathologically proved osteosarcoma preoperatively. Bcl-2 was examined using immunohistology chemistry before and after chemotherapy. Expression of bcl-2 was correlated with change of radiography and prognosis. Results: Bcl-2 was positive in 73.9% and 39.1% of patients before and after chemotherapy respectively. There was a statistically difference between the two groups. Mean score of radiography before chemotherapy was statistically higher than that after chemotherapy, which was 5.96 scores and 8.65 scores respectively. In 12 patients with good prognosis, bcl-2 was positive in 33.3% and 8.3% before and after chemotherapy respectively. While that in patients with bad prognosis was 44.4% and 33.3% respectively. Conclusion: Expression of bcl-2 after chemotherapy decreased compared to that before chemotherapy. Osteosarcoma with less expression of bcl-2 had a higher score of radiograph and better prognosis.

Comparison of CT and direct visualization in thoracic pedicle screw placement

Objective: To get the information of the ability of CT scanning as a tool to assess the accuracy of thoracic pedicle screw placement. Methods: Pedicle screws were inserted in five cadaveric spines. Speciments were scanned by CT and three observer graded the appearance of the screw based on CT scans. The results were compared with those of direct visualization group. Results: The authors arrived at a Kapper value of 0.603. For pedicle screw of grade Ⅰ, CT had a positive predictive value of 32%, a negative predictive value of 88%, a sensitivity of 33% and a specificity of 87%. Whereas, for pedicle screw of grade Ⅱ, CT had a positive predictive value of 60%, a negative predictive value of 100%, a sensitivity of 100% and a specificity of 88%. Conclusion: The authors conclude that CT scanning is a valid tool to assess the accuracy of thoracic pedicle screw placement, but in evaluating whether the screw has invaded the inner wall of the pedicle and its degree of invasion there has still some limitations.

Clinical value of double phase 99mTc-MIBI imaging in diagnosing hyperparathyroidism

Objective: To evaluate the clinical value of double phase 99mTc-MIBI imaging in diagnosing hyperparathyroidism. Methods: Forty-four patients with highly suspected hyperparathyroidism underwent double phase 99mTc-MIBI imaging. Abnormal increase radioactivity in delay imaging stood for positive index. Imaging material were evaluated according to final clinical results and compared with ultrasound or CT. Results: In 44 patients, 33 hyperparathyroidism were proven, including 31 adenomas(ectopic 1), 1 hyperplasia and 1 carcinoma. The sensitivity and specificity of diagnosis were 90.9%, 100%, respectively. The accuracy of double phase 99mTc-MIBI imaging was 93.2%. The sensitivity and specificity of ultrasound were 70%, 77.3%, respectively. The sensitivity of CT was 60%. The results showed that double phase 99mTc-MIBI imaging was superior to ultrasound and CT(P<0.05). Conclusion: Double phase 99mTc-MIBI imaging could accurately diagnose hyperparathyroidism. It is an important localizing method for preoperation which is worthy of generating.

Computer aided detection of pulmonary nodules

Objective: To determine the effect of computer aided detection(CAD) system on the diagnosis of pulmonary nodules. Methods: One hundred and fifty cases with solitary lung nodules up to 9~30mm in diameter were examined. All nodules had been verified by means of biopsy. One hundred and fifty cases of healthy individuals were selected as control group on the basis of chest CT. All chest radiographs in the two groups were obtained with a digital radiography system. Five experienced chest radiologists and five residents detected the chest radiograph with or without CAD output images. The scales of performance was evaluated with receiver operating characteristic analysis. Result: The average area under the curve value increased significantly from 0.766 without CAD to 0.833 with CAD output images. Individually, the use of CAD output images was more useful to radiology residents than to experienced radiologist. Conclusion: This CAD system for digital radiograph can assist radiologist and has the potential to improve the detection of lung nodules.

Clinical application of MSCT with VRT 3D reconstruction in the diagnosis of spinal trauma

Objective: To investigate the imaging methods of multi-slice spiral CT(MSCT?) with volume rendering technique(VRT) 3D renconstruction, and evaluate its clinical application in the diagnosis of spinal trauma. Material and Methods: There were 65 cases in this group. MSCT with volume rendering were performed in all cases. Then thin slice low and high constrast image were reconstructed. After that, all the source images were input into a 3D work station. High quality VRT-3D images of spinal trauma and after spinal internal fixation were obtained by selecting appropriate CT value. All the pictures were observed and analysed by two radiologist. Results: The optimal VRT images clearly showed alignment and displacement of fracture, and displayed space changes of spinal trauma. In observing spinal internal fixation with metal fixer, the optimal VRT images clearly showed the metal fixer and its surrounding structures. Conclusion: VRT-3D images of MSCT can directly, stereoscopically and multi-angularly show spinal trauma and after spinal internal fixation for the surgeons to decide clinical management.

Diagnosis and evaluation of tibial plateau fractures with MRI

Objective: To discuss the clinical value of MRI in tibial plateau fractures. Methods: Sixty-six patients of tibial plateau fractures associated with knee joint injuries were examined with MRI after X-ray plain film examination. Spin echo(SE) T1WI, fast spin echo(FSE) T2WI and short T1 inversion recovery(STIR) were acquired in knee joint of all patients. Each fracture was categorized according to the Schatzker classification. X-ray plain films were compared with MRI in diagnosing tibial plateau fractures. MRI appearances of injuried lesions of knee joints associated with fractures were assessed. Results: Sixty-six patients of tibial plateau fractures who had been diagnosed by X-ray film and 60 were found to have occult fracture. All of the 65 cases were found to have occult fracture by MRI. In our series, 7 cases(10.6%) of fractures were type I, 20 cases(30.3%) were type II, 6 cases(9.1%) were type III, 18 cases(27.3%) were type IV, 4 cases(6.1%) were type V, 10 cases(15.2%) were type VI. Within the 66 cases, it was found that in 57 cases there were meniscal injuries, 48 cases with ligamentous injuries, 58 cases with soft-tissue injuries, 34 cases with bone contusion, 61 cases with intra-articular effusion. Conclusion: MRI examination is a useful and comprehensive methods for evaluating tibial plateau fractures preoperatively.

MRI diagnosis of the acute achilles tendon rupture

Objective: To evaluate the diagnostic value of achilles tendon rupture by MR images. Methods: MRI findings in 21 patients with operatively proved acute achilles tendon rupture were analyzed, and manifestations of MR and operation were compared. Results: The achilles tendon rupture was diagnosed on MRI in 21 cases. Achilles tendon complete rupture was showed in 7 cases, partial rupture in 14 cases. MRI appearance of achilles tendon rupture represented achilles tendon thickening, hyperintensity in tendon, completely or partially discontinuous fibrous bundle. Operation showed achilles tendon complete rupture in 9 cases, partially rupture in 12 cases. Conclusion: MRI can accurately show the degree and portion of acute achilles tendon rupture, which is very helpful for treatment.

MRI findings of primary lymphoma of bone

Objective: To study the MRI findings of primary lymphoma of bone(PLB) and evaluate MRI scanning in the diagnosis of the disease. Methods: A retrospective analysis was made on imaging of 8 patients with pathologically proved PLB. The unenhanced and enhanced scanning was performed in all patients. Results: There was single bone involvement in 7 cases and multiple involvement in 1 case. Seven cases were B-lymphocytes, while 1 case was T-lymphocytes. Most of the lesions showed iso- or hypo-intense signal on T1WI and iso- or slightly hyperintense signal on T2WI. Lesions showed significantly homogeneous or inhomogeneous enhancement. Various size soft-tissue masses were present in all cases. Conclusion: PLB mainly involved vertebrae and pelvis. There was single bone involvement in most cases of PLB. The comparatively typical features of MRI were a large accompanying soft tissue mass, slight periosteal reaction, and isointensity on T2WI with significantly enhancement. The plain and enhanced MRI scanning were all very helpful in the diagnosing of PLB.

Clinical imaging manifestation and pathological analysis of chondroblastoma

Objective: To study the clinical and X-ray, CT appearance and pathological speciality of chondroblastoma. Materials and Methods: X-ray examination was performed in 16 cases, CT scanning was performed in 8 cases. Twelve cases were males and 4 cases were females. The youngest one was 6 years old and the oldest one was 47 years old. Results: Four cases were in tibia, 5 cases in femur, 3 cases in humerus, each of the acetabulum, calcaneus, talus and scapula had 1 case. Cystic and expansive damage of bone(9 cases), cyst-like destruction(5 cases), invaded destruction(1 case). Eccentric-type(12 cases), Y-aixs type(4 cases), glass-like density(2 cases), short bone protuberance(6 cases); sand-like or spotty sclerosis(11 cases), partial disappearce of bony cortex(6 cases), osteosclerosis(12 cases), periosteal reaction(1 case); articalas involvement(2 cases), mass of soft tissue(5 cases). Diameter of bone lesion was 5~9cm. Conclusion: Chondroblastoma had its specificity, but imaging finding, and pathology had many varieties. It’s should be differentiated from other relative lesions. The final diagnosis depends on pathology.

Comparative studies on MR enhancement of SA-DTPA-Gd after different routes of administration in nude mice bearing human colon carcinoma

Objective: To investigate MR enhancement of different administration routes in MR pretargeting immunology imaging. Methods: Two groups of mice were injected with SA-DTPA-Gd through tail vein(iv) and intra-peritoneal cavity(ip) respectively, and the MR signal intensity of tumor and liver was determined. Result: In iv group and ip group the maximum enhancement rate of tumor was 70.2% and 46.4% respectively, and there was significant difference. The maximum enhancement rate of liver was 23.7% in ip group and 20.4% in iv group, and there was no signifinant difference. Conclusion: Intravenous injection is in favor of SA-DTPA-Gd’s accumlation in tumor than intraperitoneal injection, and a more homogenious intratumoral distribution is archived.
