Comparison of 1H MRS, PET/CT and postoperational pathology of the patients with temporal lobe epilepsy

Objective: To compare the results of the preoperational 1H MRS with PET/CT, electroencephalogram(EEG) and postoperational pathology of the patients with temporal lobe epilepsy(TLE) and to determine the relative utility of the preoperational 1H MRS for the diagnosis and presurgical lateralization of temporal lobe epilepsy. Methods: EEG and PET/CT were performed in all of the 16 patients. 1H MRS was performed in 16 patients with TLE and 28 healthy volunteers. Signals from N-acetylaspartate(NAA), creatine(Cr) and choline-containing compounds(Cho) were observed. The focus of 3 patients was resected with the intraoperative monitoring of EEG and the pathological examination were performed. Results: Analysis demonstrated the difference of NAA/Cr, Cho/Cr and NAA/(Cho+Cr) among the abnormal side, contralateral side and control group had statistical significance (P<0.05). On the basis of NAA/(Cho+Cr) ratio, lateralization was achieved in 14 cases. Bilateral abnormalities were showed in 11 cases with the ipsilateral side more severe than that of contralateral side. Unilateral abnormalities were showed in 5 cases. 1H MRS had a sensitivity of 87% for lateralization of seizure in TLE. Conclusion: 1H MRS could contribute to the location of seizure focus and detection of bilateral abnormalities, consisting with the findings of EEG and PET and pathologic alterations very well. 1H MRS is a useful noninvasive diagnostic technique for preoperational evaluation of patients with TLE who are eligible for surgery.

The imaging characteristics of MELAS syndrome

Objective: To investigate the imaging characteristics of MELAS syndrome and assess the diagnostic value of MRI. Methods: The clinical and imaging findings of 6 patients with MELAS syndrome were analyzed retrospectively. Result: Abnormal calcification on CT was not found in 3 patients. 10 stroke-like episodes presented long T1 long T2 signals in the cortex and subcortical white matter of temporal, parietal, and occipital lobes, and 2 presented high signals in diffusion weighted images in periods of attacks. In convalescent periods, local atrophy and laminar necrosis were showed. Muscle biopsy showed ragged red fibers in 5 patients. Conclusion: Recurrent lesions in the cortex and subcortical white matter of temporal, parietal, and occipital lobe on MRI might provide clues for diagnosis of MELAS syndrome. DWI might show lesions earlier than T2WI in MELAS syndrome similar to cerebral infarction.

Comparison of MRI sign and pathological findings in intracranial hemangiopericytomas and angiomatous type meningioma

Objective: To analyse MRI features of intracranial hemangiopericytomas and angiomatous type meningioma, and correlate with their pathology. Materials and Methods: Ninteen cases of intracranial hemangiopericytomas and 25 cases angiomatous type meningioma were verified by pathology. MRI with and without contrast medium were performed in all 44 cases. Results: All patients were of extraaxial tumors. Fifteen of the 19 cases were heterogeneous in intensity isointense to cortical gray matter in 4 cases on T1WI; 15 of 19 cases were heterogeneously iso-hyperintense signal and the other 4 cases were isointense on T2WI. Fourteen of the 19 cases were heterogeneously enhanced on contrast-enhanced T1WI. Conclusion: Intracranial hemangiopericytomas is similar to meningiomas on MRI, but they can be distinguished according to MRI features of intracranial hemangiopericytomas: multiobulated, rich in vascular flow in the tumor, heterogeneous signal intensity, hyperostosis and no intratumoral calcification, the tumor used to have adjacent bone destruction.

CT diagnosis of orbital lymphoma

Objective: This article explores how to diagnose lymphoma in the orbit by CT in order to improve the ability of diagnosing. Methods: In this paper, the CT features of orbital lymphoma in 12 cases proved by operating pathology were analyzed retrospectively. Results: In these 12 cases, only 1 is Hodgkin’s disease, whose pathological changes are in extraconal compartment, and the other 11 are non-Hodgkin’s lymphoma, in which merely one involved both eyes, and others on one side. Seven are of confined-type whose pathological changes exist around the eyes, and 5 are of diffused type, on whose picture there are shapes of soft-tissue in extraconal orbital compartment. Conclusion: Lymphoma has certain features on CT. The important clue of lymphoma is that the soft-tissue mass grows around the eye ring. But the final decision is made according to biopsy and immunohistochemistry.

The CT diagnosis of adenolymphomas in parotid gland

Objective: To discuss the CT and clinical appearance of adenolymphomas in the parotid gland. Methods: The CT appearances of adenolymphomas(n=12) in the parotid gland confirmed pathologically after operateration or biopsy were analysed retrospectively. Results: In 12 cases adenolymphomas of the parotid gland, 11 cases were found in unilateral side and bilateral side in 1, altogether 13 sides; male (n=10, 83.3%), female(n=2, 16.7%). Solitary lesion was seen in 9 cases (75%) and multiple lesions in 3(25%), altogether 16 lesions, 9 lesions were located in the posterior-inferior part of the parotid gland(n=9, 56.3%). CT findings: lesions were round,oval or lobulated soft tissue mass with well-defined, homogeneous density(n=10), and inhomogeneous density(n=6) with necrosis or cystic formation. Lesions showed homogeneous enhancement except the area of cystic changes, mostly with moderate enhancement. Conclusion: Fully analysing sex, age, localization of lesions and appearances of CT is useful in diagnosis and differential diagnosis.

Application of carotid MRA in the diagnosis and surgical treatment of carotid body tumor

Objective: To discuss the clinical value of carotid magnetic resonance angiography(CMRA) in the diagnosis and surgical treatment of carotid body tumor(CBT). Methods: The imagining materials of 24 cases with CBT admitted from 1998 to 2005 were analyzed retrospectively. All patients were examined by ultrasound and carotid magnetic resonance angiography routinely before the operation, and the results were compared with that of surgeries, and the accuracy rate of the two examinations were analyzed. Results: The ultrasound showed rich in blood supply of CBT but were not accurate in judging the source of the tumor, and the accuracy rate of ultrasound was 95.8%. Meanwhile, all 24 cases were classified as Shamblin I and II type on the imagining of CMRA, which revealed definitely a solid vascular mass and its relation with the surrounding arteries in the carotid bifurcation. The diagnosis of CMRA were confirmed by surgery as well as post-operation pathological sections. The accuracy rate of diagnosis of CMRA was 100%. Conclusions: CMRA plays an important role in the diagnosis and clinical classification as well as surgical treatment of CBT. When CBT was suspected, CMRA should be examined routinely before surgery.

CT diagnosis of hepatic invasion from gallbladder carcinoma(analysis of 25 cases,compared with B ultrasonography)

Objective: To explore the value of CT diagnosis of hepatic invasion from gallbladder carcinoma. Methods: CT manifestations of hepatic invasion from gallbladder carcinoma in 25 cases examined by CT and B ultrasonography and have been proven by clinic were analysed retrospectively. Results: Of the 25 cases with hepatic invasion from gallbladder carcinoma, 21 were diagnosed accurately by CT and 11 were diagnosed accurately by B ultrasonography. There is no interface between liver and gallbladder tumour in most cases with hepatic invasion from gallbladder carcinoma, and there is abnormal low density in the liver closed to gallbladder tumor, which has enhancement after injection of contrast medium. Conclusion: CT is valuable in diagnosing hepatic invasion from gallbladder carcinoma, the diagnostic accuracy of CT is significantly superior to that of B ultrasonography(P<0.005).

Spiral CT with curved planar reformations in the evaluation of pancreatic vessels invasion by pancreatic carcinoma

Objective: To evaluate multislice spiral CT(MSCT) with curved planar reformations(CPR) in detecting invasion of pancreatic vessels by pancreatic carcinoma. Methods: Twenty-seven cases of pancreatic carcinoma verified by surgery underwent contrast material-enhanced triphasic multi-slice spiral CT. CPR were generated along peripancreatic major vessels. Three readers together assessed the transverse images and the transverse images combined with CPR for detecting the presence of vascular involvement. Results: Totally, 216 vessels of 27 cases were analyzed, 72 vessels were invaded and 144 vessels were not-invaded verified by surgery. Sixty-four vessels were invaded(6 vessels were erroneously diagnosed), 152 vessels were not-invaded(14 vessels were misdiagnosed) by the transverse images. 69 vessels were invaded (3 vessels were erroneously diagnosed), 147 vessels were not-invaded(6 vessels were misdiagnosed) by the transverse images combined with CPR. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of the transverse images and the transverse images combined with CPR for vessels invasion were 80.6%, 95.8%, 90.6%, 90.8%, 90.7%;91.7%, 97.9%, 95.7%, 95.9%, 95.8% respectively. Conclusion: CPR help to display the whole view of peripancreatic major vessels and relation to tumors, and help to improve the detection accuracy of vascular involvement by tumor.

Primary ureteral carcinoma: comparison of CT findings with pathology

Objective: To explore the characteristics of the primary ureteral carcinoma and discuss the value of spiral CT(SCT) in diagnosis of it. Methods: The SCT findings of primary ureteral carcinoma in 16 cases were analysed and compared with histopathological diagnosis. Results: The transverse diameters of the lesions were 1.0~2.1cm, and the longitudinal involvement lengths were from 1.5~15.2cm. There were no statistically significant differences(P>0.1) in diameters and lengths among the low staging group(pT0~T2) and the high staging group(pT3~T4). The average CT value of the lesion was 43HU on plain scanning, and 73HU on CE scanning. The increasement of CT value was 30HU. The lesions were clearer on CE scanning. Curved planar reconstruction(CPR) can show the entire course of the urinary tract. Among 6 cases of pT3 stage, CT diagnosed accurately in 1 case. For 2 cases of pT4 stage, CT diagnosis was right. Conclusion: The carcinomatous lesions spread along the ureter. The longitudinal length were longer than the transverse diameters, but tumor cannot be staged merely according to the diameter and length. For stage T0~T3, CT differentiation was difficult, for stage T4, all CT diagnosis were accurate. Contrast enhancement CT scanning plays an important role in confirming and differentiating diagnosis. CPR offered a direct and easy observing images for clinical doctors.

Evaluation of MR venography of lower extremity in 38 cases

Objective: To evaluate the clinical value of magnetic resonance venography in diagnosing venous disease of lower extremity. Method: Thirty-eight cases were examined with 2D TOF MRV. Contrast enhancement MRV was performed from the distal part of inferior vena cava to the shank. Result: In 38 cases, MRV showed normal deep veins in 395 segments, venous thrombosis in 5 segments, stenosis in 5 segments and occlusion in 13 segments. With the X-ray angiography(XRA) as a standard of reference, the sensitivity, specificity and the accuracy of MRV in detecting deep vein abnormalities were 100%, 66.67% and 94.44%. Conclusion: As a new repeatable noninvasive imaging examination, MRV is accurate in showing the site, range and degree of venous stenosis and thrombosis of the lower extremity.
