Resting-state function MRI study on the effectiveness of aerobic exercise in patients with Alzheimer’s disease

Objective: To evaluate the therapeutic effect of aerobic exercise in patients with Alzheimer’s disease(AD) quantitatively by functional magnetic resonance imaging(fMRI). Methods: Seventeen cases of patients with AD were randomly divided into training group(9 cases) and control group(8 cases). The fMRI data of baseline and 3 months later were got for the amplitude of low frequency fluctuation(ALFF) analysis and analysis of brain functional network. Results: The training group showed increased ALFF value of bilateral prefrontal and anterior cingulate cortex and brain network between centrality(BC) value compared with baseline after 3 months of training, while the BC value of training group was higher than those in control group after 3 months. The difference was statistically significant(P<0.05). Further analysis of the network hubs showed bilateral distribution after training, which was different from the asymmetric distribution before. Conclusion: Resting-state fMRI data suggest that aerobic exercise is helpful to recovery of cognitive function in patients with AD, providing a quantitative evaluation method of effectiveness.

The MRI diagnosis of cerebral cortical laminar necrosis

Objective: To investigate the MR imaging features of cerebral cortical laminar necrosis(CLN). Methods: The MR imaging materials of 17 CLN patients diagnosed in our hospital ranged from January 2012 to October 2014 were retrospectively analyzed. The MRI characteristics were studied. Results: CLN was detected from 12 to 32 d since the patients were admitted. The affected regions were bilateral cerebral hemisphere(3/17), unilateral cerebral hemisphere(14/17), then the single lobe(11/17), double lobes(4/17), multiple lobes(2/17). On MRI, the gyral shape lesion showed high intensity on T1WI and fluid-attenuated inversion-recovery(FLAIR), lesions showed more clearly on original three dimensional time of flight magnetic resonance angiography(3D-TOF MRA) images, ill defined on T2WI, 12 patients manifested as high intensity on DWI. No hemorrhagic focus showed on SWAN in 6 patients. Conclusion: Typical imaging features could be revealed in CLN, which is high intensity on MRI T1WI. Accurate diagnosis could be obtained with imaging examination in combination with clinical history.

Diagnosis value of contrast-enhanced ultrasound combined with VTQ technology in the thyroid solid nodules

Objective: To assessment the diagnosis value of thyroid nodules by the contrast-enhanced ultrasound and the virtual touch tissue quantification(VTQ) of acoustic radiation force impulse. Methods: Sixty-two patients with 96 thyroid solid nodules were enrolled into this prospective study(the final pathology diagnosis was confirmed by thyroidectomy). Analysis of contrast enhancement characteristics, intensity-time curve parameters combination VTQ parameters for differential diagnosis of thyroid benign and malignant nodules. Results: Fifty nodular goiter, 15 adenoma, 31 papillary carcinoma, according to Chi-square test analysis with three types, enhanced degree, enhanced shape, peripheral enhancement had significant statistical difference(P<0.01). In differential benign and malignant nodules aspect, the parameters of RS-Ratio, IMAX-Ratio AUC was relatively larger than others in the time-intensity curve(P<0.01), VTQ parameter SWV and SWV-Ratio AUC was larger than the RS-Ratio, IMAX-Ratio. Conclusion: The contrast-enhanced ultrasound pattern has great value in identification of thyroid nodule pathological type. The VTQ technology has greater value than the intensity-time curve in determination of whether a thyroid nodule was benign or malignant.

The utility of shear wave elastography and micropure imaging in the differentiation of#br# benign and malignant thyroid nodules

Objective: To evaluate the utility of shear wave elastography(SWE) and micropure imaging in the differential diagnosis of benign and malignant thyroid nodules. Methods: A total of 109 consecutive patients with 186 thyroid nodules, who were referred for biopsy results underwent thyroid surgery, were prospectively examined using B-mode ultrasound, ultrasound SWE, and micropure imaging. Results: Final histology revealed 102 benign and 84 malignant. The sensitivity of using SWE, micropure imaging, SWE combined with micropure imaging were 73.8%, 85.7%, 91.7% respectively, the specificity were 86.3%, 83.3%, 89.2% respectively, and the accuracy were 80.6%, 86.0%, 90.3% respectively. SWE combined with micropure imaging had a high correlation with histology. Conclusion: This preliminary study indicates that SWE combined with micropure imaging can be used for the differentiation of benign and malignant thyroid nodules, which could provide an available method in the diagnosis of thyroid nodule.

Study on the angiography of the pulmonary arteries in different threshold using low dose contrast with gemstone CT

Objective: To study the effect of angiography of the pulmonary arteries using the intelligent technology in the same trigger position, but different threshold value of low dose contrast agent, in order to improve the pulmonary CTA image quality. Methods: Seventy-two patients accepting pulmonary artery CTA were randomly divided into A, B, C group. The superior vena cava in the level of the aortic arch was the monitor position. 50, 80, 110 HU were the monitoring threshold under which the intelligent trigger scan was administered respectively. Both the difference of CT value of main pulmonary artery, segmental pulmonary artery, vein, aorta, and the difference between the subsegmental pulmonary arterial and the pulmonary venous enhancement CT value, and the differences of image quality in different monitoring threshold levels were compared finally. Results: The differences of the enhancement CT value of the main pulmonary artery, segmental pulmonary artery and pulmonary vein, aorta and subsegmental pulmonary artery and pulmonary vein and the image quality in different monitoring threshold level were significant. The 50 HU group had good image quality and superior to the other two groups(P<0.05). There were no significant difference between the 80 HU and the 110 HU groups(P>0.05) in the segmental pulmonary artery and vein. Conclusion: With superior vena cava for monitoring points in the level of the aortic arch, and by using 50 HU as the trigger threshold can obtain more ideal images.

Comparison of image quality among different low dose contrast materials injection protocols in#br# coronary CT angiography

Objective: To compare the image quality(IQ) of different contrast materials(CM) injection protocols with low identical iodine delivery rate(IDR) for low-dose coronary CT angiography(CCTA) with iterative reconstruction(IR) technique. Methods: Ninety consecutive patients who underwent prospective electrocardiogram-CCTA on a 256-MDCT scanner were randomized to three groups: group A, which received CM(350 mgI/mL), was scanned by using the conventional tube output(120 kV) with filtered back projection(FBP). Group B which received CM(350 mgI/mL) and group C which received CM(270 mgI/mL) had identical IDR adapted to body mass index and weight, and scanned by 100 kV with iDOSE4 level 4. The subjective IQ, objective IQ, radiation dose and iodine load were compared among the three groups. Results: There were no significant differences in the image noise, contrast-to-noise ratio and attenuation of coronary among these three groups. Group B and C had good scores of the subjective IQ(≥3), and it could satisfy the clinical use. There was no significant difference of lumen contrast between group B and C. No significant differences of sharpness, subjective noise, and acceptability of images were observed, but the contrast dose of group A was significantly higher than group B and C. Mean iodine load of group A were 36.9%~39.6% higher than group B and C, and there was no difference between group B and C. The effective radiation dose of group B and C was 37.9% lower than group A, and there was no difference between group B and C. Conclusions: Equivalent and adequate diagnostic IQ can be achieved by using different iodine-concentration contrast materials(270 mgI/mL and 350 mgI/mL) when the IDR were kept identical in low-dose 256-MDCT coronary angiography with iDOSE4 level 4, compared with the IQ by using conventional tube output and FBP.

CT findings of congenital variation and malformation of coronary artery

Objective: To research the CT findings of congenital variation and malformation of coronary artery. Methods: There were 1 402 patients who had underwent prospective ECG-triggering coronary CTA in our hospital were enrolled this study. We reconstructed the raw data by several post-processing techniques and summarized the CT findings of congenital variation and malformation of coronary artery. Results: We found 364 cases of variation and malformation of coronary artery in this study, including one case of absence of left main coronary artery, 3 cases of short left main coronary artery and one of them has accessary coronary artery, 3 cases of accessary coronary artery or third coronary artery, 7 cases of right coronary originated from the left coronary sinus, one case of single coronary artery, one case of abnormal coronary artery anastomosis, one case of the right posterior descending coronary artery aneurysm with left atrial diverticulum, three cases of coronary artery aneurysm, 20 cases of coronary artery tortuosity, 325 cases of myocardial bridge-mural coronary artery. Conclusion: Prospective ECG-triggering coronary CTA can significantly reduce the radiation dose of coronary CTA and can accurately and clearly show the variation and malformation of coronary artery.

The echocardiographic features of multiple foramen atrial septal defect

Objective: To summarize the echocardiographic features of multiple foramen atrial septal defect(ASD) by observing 65 cases of multiple foramen atrial septal defect. Methods: We observed the ASDson multiple sections and angles by iE33 or iU22 diasonograph and analyzed the number, shape and size of ASD. According to the ultrasonic inspection results, different clinical intervention opinions were given according to different types of multiple foramen ASD. Results: There were 33 double foramen and 32 multiple foramen which were often associated with atrial septal aneurysm. The characteristic echocardiographic appearance included mesh-like echo interruption and different shunts from left to right in color modes. Conclusion: It is important to observe multiple foramen ASD on multiple sections and angles. Echocardiography has important clinical significance for multiple foramen ASD.

To evaluate the left ventricular mechanics changes in patients with acute heart failure after#br# treatment by rhBNP using two-dimensional speckle tracking imaging

Objective: To investigate the left ventricular twist in patients with acute heart failure after treatment by rhBNP using two-dimensional speckle tracking imaging. Methods: Forty cases of acute heart failure patients were given conventional therapy. Respectively, in the application of the rhBNP before and after treatment, the left ventricular mechanics parameters were obtainted, the parameters included peak apical rotation(Par), peak basal rotation(Pbr), peak twist(Ptw), peak untwist velocity(PutwV), time to peak twist(TPK). The above data of another 40 healthy volunteers were measured as control. Results: Compared with control group, Par, Pbr, Ptw and PutwV of patients with acute heart failure all obviously decreased before treatment by rhBNP(all P<0.001) among them, Par relatively obvious decline compared Pbr, the twist parameters improved in some degrees after treatment by rhBNP, but were still lower compared to those of control group(all P<0.05). Conclusion: rhBNP treatment can effectively improve the left ventricular function in patients with acute heart failure, 2D-STI can assess left ventricular myocardiac mechanics changes after rhBNP treatment quantitatively and objectively, and provide an effective and noninvasive method for clinical evaluation of left ventricular function.

The appilication of ultrasound elastography scoring and strain ratio in#br# differentiating benign and malignant focal liver lesions

Objective: To evaluate the value of ultrasound elastography(UE) scoring and strain ratio(SR) in differentiating benign and malignant focal liver lesions(FLL). Methods: Conventional ultrasound, ultrasound elastography scoring and SR were performed in 81 patients with 89 FLL, the results were compared with clinical diagnosis. Receiver operating charcteristic(ROC) curves of UE scoring and SR were portrayed, the cut-off points and the areas under the ROC curves(AUCs) were calculated. The sensitivity, specificity, accuracy of conventional ultrasound, UE scoring and SR were calculated, the accuracy of these three methods were evaluated. Results: ①The sensitivity, specificity, accuracy of conventional ultrasound in differentiating benign and malignant FLL was 76.2%, 72.3%, 74.2%. ②The AUC of UE scoring in differentiating benign and malignant FLL was 0.875, the cut-off point was 2.5, the sensitivity, specificity, accuracy of UE scoring was 90.5%, 78.7%, 84.3%. The accuracy of UE scoring in differentiating benign and malignant FLL was higher than conventional ultrasound(P<0.05). ③The AUC of SR in differentiating benign and malignant FLL was 0.892, the cut-off point was 2.71, the sensitivity, specificity, accuracy of SR was 83.3%, 89.4%, 86.5%. The accuracy of SR in differentiating benign and malignant FLL was higher than conventional ultrasound(P<0.05). ④The accuracy of UE scoring and SR in differentiating benign and malignant FLL showed no significant difference(P>0.05). Conclusions: The accuracy of UE scoring and SR in differentiating benign and malignant FLL are higher than conventional ultrasound, they provide new ways to differentiate malignant from benign FLL, UE scoring is in good agreement with SR, there is no significant difference between UE scoring and SR.

The critical point of the peripancreatic blood vessels by pancreatic cancer invasion can be removed

Objective: To explore the critical point of imaging findings, in which the invaded peripancreatic blood vessels could be operability by surgery. Methods: A total of 76 cases of patients with distant metastasis-free pancreatic cancer confirmed by operative pathology or clinical treatment were enrolled, all of them underwent contrast-enhanced CT before operation. Among them, 66 patients underwent enhanced MRI scanning and three-dimensional vascular reconstruction. According to the images, the extents of pancreatic cancer invasion of peripancreatic vessels were divided into grade 0, grade 1, grade 2, grade 3, and grade 4. The numbers of vessels in each grade were counted and compared to the results of operation. Results: There were 138 vessels in 39 cases with invasion extent higher than grade 2, so the operations were given up. A total of 37 cases were assessed with the possibility of operative excision before operation. Among them, 25 cases accepted radical excision of pancreas, however, 12 cases accepted tumor resection or palliative therapy alone because of preoperative underestimate of the invasion extent of one or more vessels. In these 12 cases, the invasion extents of 16 arteries and 21 veins were underestimated. Operative excision could not be executed if the invasion extent was assessed as grade 3 and 4 during operation, while it could be executed if the invasion extent was assessed as grade 0 and 1 during operation. Moreover, operative excision depended on the condition of patients if the invasion extent was assessed as grade 2. Conclusion: Preoperative multi-phase CT, MRI scanning and 3D vascular remodeling, it is very important to evaluate the operability before operation. Level 2 arterial invasion and level 1 vein invasion can be the critical point of resection.

The performance of solid-pseudopapillary tumors of pancreas of the men in dynamic contrast-enhanced MSCT

Objective: To investigate the dynamic contrast enhancement multi-slice CT(MSCT) appearances of solid pseudopapillary tumor of pancreas(SPTP) in men, and improve the diagnostic accuracy. Methods: Twenty cases of SPTP were collected respectively from June 2008 to December 2014, each case was confirmed by pathology, 8 cases were male, 12 cases were female. The difference of the performance of MSCT between male and female was analyzed, including the location, size, capsule, calcification, composition, CT enhancement of the mass. The Chi-square test was used to statistical analysis. Results: The median age was 41.0 in men, that was elder than the women, the difference was statistically significant. The differences of the location, calcification, CT enhancement of the mass between male and female were not statistically significant(P>0.05). The maximum diameter of the mass of male was less than the one in female(P<0.05). Solid mass was more common in women(P<0.05), the difference was statistically significant. Conclusion: The performance of MSCT of male is significant partly. When the pancreatic lesion expressed as a small, with many solid component and enhanced as SPTP typically, it should be diagnosed as SPTP.

The value of DWI in the early-term effectiveness of high-intensity focused ultrasound ablation of adenomyosis

Objective: To retrospectively analyze the early-term value of diffusion weighted imaging(DWI) features of adenomyosis after high intensity focused ultrasound(HIFU) ablation. Methods: Fifty-five patients(44 cases of focal uterine adenomyosis, 11 cases of diffuse uterine adenomyosis) underwent HIFU ablation, DWI and CE-MRI was performed one week before and 5 days after HIFU. The feature of DWI signal and ADC value of the residual part of adenomyosis was analyzed, and then calculated the difference of ADC values among different areas. Results: The residual part of adenomyosis was found in DWI. The signal intensity of the residual part of adenomyosis in DWI sequence increased at 5 days after HIFU ablation. The ADC value of the residual part of adenomyosis was (1.49±0.23)×10-2 mm2/s at 5 days after HIFU ablation, was higher than the ADC value((1.36±0.21)×10-2 mm2/s) before operation, and the t value of T-test was 3.10, the value of P is 0.003, there was significant difference between before and after surgery. And the ADC value of the focal ablation part of adenomyosis was (1.22±0.24)×10-2 mm2/s when was HIFU ablation after 5 days, was lower than the ADC value((1.36±0.21)×10-2 mm2/s) before operation. The t value of T-test was 6.02, the value of P is 0.000, the significant difference was found between the ADC value of the residual part and focal ablation part(P<0.005). Conclusion: DWI and ADC value are very useful means for monitoring the early term effectiveness of ablation of adenomyosis after HIFU ablation.

Segmental analysis for prenatal ultrasound in diagnosis of fetal heterotax syndrome

Objective: To explore clinical application value of prenatal ultrasound in diagnosis of fetal heterotax syndrome with segmental analysis, to compare the results of pathological anatomy. Methods: We compared relevance ratio between February 2011 to December 2012 without using segmental analysis and January 2013 to March 2015, 21 fetus with heterotax syndrome confirmed by prenatal ultrasound were analyzed retrospectively through segmental analysis. We also compared the prenatal ultrasonographic characteristics and pathologic autopsy after induced labor. Results: During the period of January 2013 to March 2015, 21 fetuses with heterotax syndrome were diagnosed, and proved by autopsy. Sixteen fetuses had right isomerism with complex cardiac anomalies, juxtaposition of inferior vena cava and abdominal aorta, abnormal position of the stomach, 15 fetuses with asplenia, 1 fetus with spleen dysplasia; 5 fetuses had left isomerism with interruption of inferior vena cava with azygos continuation, 3 fetuses with complex cardiac anomalies, 2 fetuses with normal cardiac structure, 2 fetuses with atrioventricular block. Eighteen fetuses were confirmed by pathological anatomy after induced labor. While in February 2011 to December 2012, without using segmental analysis, 4 fetuses with heterotax syndrome were missed diagnosis compared with autopsy, only 1 case was detected by prenatal ultrasound. Conclusion: Segmental analysis for prenatal ultrasound can significantly reduce the misdiagnosis rate of fetal heterotax syndrome, in favor of the prenatal counseling and clinical treatment of decision-making in the perinatal period.

Value of the angle between the first and fifth metatarsal bones on plantar plane in#br# prenatal diagnosis of fetal congenital clubfoot

Objective: Discuss the value of the angle between the first and fifth metatarsal bones on plantar plane in diagnosis of fetal congenital clubfoot. Methods: There were 186 feet of 117 fetuses with gestational age from 19 to 37 weeks in control group. There were 37 feet from 24 fetuses which were suspected with congenital clubfoot in case group. Use TUI mode to measure the angle between the first and fifth metatarsal bones on plantar plane, and analyze the correlation between the angle and the gestational age. Compare the results with the case group and use ROC curve to judge its diagnostic value. The fetuses in case group were followed to obtain the pregnancy outcomes and confirm whether the deformity existed. DR examination was used to measure the angle of bilateral feet of the fetuses with congenital clubfoot on postero-anterior view. Compare the results with ultrasonic measurement and calculate Kappa value. Results: The angle between the first and fifth metatarsal bones on plantar plane in control group was (22.83±2.07)°. It had no obvious correlation with gestational age(r=0.104, P=0.158>0.05). The angle of case group was (32.91±3.30)°, and compared with control group, the difference is statistically significant(P=0.000<0.05). The area under ROC curve was 0.978, the cut-off was 27.47° with sensitivity of 94.6% and specificity of 97.8%. The angle of normal feet and clubfoot under DR check were (23.28±1.16)°, (33.59±3.02)°, the differences had no statistically significant between ultrasonic measurements and DR check(P=0.471>0.05, P=0.281>0.05). The Kappa value of measure of agreement were 0.811 and 0.894(P=0.000<0.05). Conclusions: The angle between the first and fifth metatarsal bones on plantar plane has no obvious correlation with gestational age. Compare to the normal fetuses, the angle of fetal congenital clubfoot between the first and fifth metatarsal bones increases obviously.

Classification diagnosis of lumbosacral plexus injuries with 3T MRI

Objective: To investigate the lumbosacral plexus anatomical structure and lumbosacral plexus injuries classification at MRI. Materials and Methods: Twenty-one patients with lumbosacral plexus injuries underwent normal and 3D-STIR MR scan. All of the images were analyzed. Results: Twenty-one patients with lumbosacral plexus injuries underwent 3D-STIR MR scan. Not only the formation, direction, continuity, shape, signal of lumbosacral plexus but also the sign of injuries could be showed clearly, including 4 cases of nerve root rupture, 11 cases of nerve plexus rupture and 6 cases of nerve trunk rupture. Conclusion: The anatomical structure of lumbosacral plexus can be showed clearly by 3D-STIR. It is also an important method for diagnosis of lumbosacral plexus injuries.

Measurement of foramen intervertebrale by spiral CT volume rendering

Objective: To measure foramen intervertebrale of adults’ lumbar segments by spiral CT volume rendering and to analyze the differences and changing tendency between lumbar foramen intervertebrale. Method: The lumbar foramen intervertebrale of 120 subjects without lumbar disease were analysed using spiral CT by volume rendering(VR). The superoinferior diameter and anteroposterior diameter of foramen intervertebrale were measured and compared between different sex and age groups. Result: For superoinferior diameter of foramen intervertebrale, there were no significant differences between male and female (P>0.05) but significant differences between different lumbar segments or different age groups(P<0.05). For anteroposterior diameter of foramen intervertebrale, there were no significant differences between male and female or different age groups(P>0.05). But there were significant differences between different lumbar segments(P<0.05). Conlusion: The superoinferior diameter and anteroposterior diameter of foramen intervertebrale vary from different lumbar segments, which provides reference to bone tunnels before transforaminal endoscopy operation or interventional treatment.
