The value of nonenhanced CT blend sign in predicting hematoma growth

Objective: To explore the value of nonenhanced CT(NCCT) blend sign in predicting hematoma growth. Methods: Altogether 126 cerebral hemorrhage patients were enrolled in our study. All the patients underwent NCCT scan when they were hospitalized. The follow-up CT scan was preformed within 48 hours after the admission NCCT. It was assessed whether there was blend sign on admission NCCT. The volume of hematoma was compared between two examinations. Results: Blend sign was observed in 40 of 126 patients with intracerebral hemorrhage on admission NCCT scan. Of the 38 patients with hematoma growth, 30 patients had blend sign on admission NCCT scan. The volume of hematoma in the positive blend sign group was greater than that in the negative group. The sensitivity, specificity, positive and negative predictive values of blend sign for predicting hematoma growth were 78.95%, 88.64%, 75.00% and 90.70%. Conclusion: The NCCT blend sign is of clinical value in predicting hematoma enlargement after intracerebral hemorrhage.

Correlation between anterior communicating artery aneurysm formation and anterior cerebral artery bifurcation geometry

Objective: The anterior communicating artery(Acom) aneurysm is considered the most complex in all cerebral aneurysms, and the relationship between the Acom morphology parameters and the Acom aneurysm formation is not clear. In this study, we aimed to determine the relationship of bifurcation angle, anterior cerebral artery(ACA) and Acom diameters associated with Acom aneurysm. Methods: The digital substraction angiography(DSA) of 698 patients was used in this study, including 171 patients with Acom aneurysm and 527 control subjects. The Acom/A2 angle, artery diameter and Acom aneurysm geometric characteristics were examined. Results: With aging, the apex angle of ACA-Acom bifurcation(Acom/A2) angle, was significantly increased(P<0.000 1). Conversely, the Acom/A1 and A1/A2 angles were significant decreased(P=0.000 2 and P<0.000 1, respectively). The Acom/A2 angle was significantly wider in patients with Acom aneurysm than in the controls(P<0.000 1), whereas A1/A2 and Acom/A1 angles in the aneurysm group were significantly smaller than in the control group(P<0.000 1 and P=0.003 7, respectively). Furthermore, a statistical positive correlation between the Acom, A1/A2 segment of ACA diameter and aneurysm geometric size(P<0.000 1), and aneurysm neck was observed(P<0.000 1). Conclusion: Acom aneurysms are associated with the wider Acom/A2 angle junction of the Acom complex, and bifurcation artery diameter shows a significant positive correction with aneurysm size.

The value of T2-MRI whole tumor histogram analysis for differenting astrocytoma from ependymoma in children

Objective: To investigate the value of histogram analysis derived from T2-MRI imaging in differentiating astrocytoma from ependymoma in children. Methods: Retrospective analysis of pathologically confirmed 85 cases of posterior fossa tumors was performed, including 36 cases of astrocytoma and 49 cases of ependymoma. After drawing the region of interest on each slice of T2-MRI maps including tumor and doing the histogram analysis, we can get mean, variance, skewness, kurtosis, the 1st, 10th, 50th, 90th, 99th percentile characteristics. The histogram parameters were analyzed statistically to find out the characteristics of the significant differences between the two groups. Then, the ROC curve was drawn to assess diagnostic efficiency. Result: As for the 9 parameters extracted from histogram, the differences of the variance, skewness, kurtosis and the 50th percentiles between the two groups showed statistical significance(P<0.05). Areas under the ROC curve were 0.668, 0.744, 0.679, and 0.651, respectively. The sensitivity and the specificity of differentiation for variance were 61.1% and 61.2% respectively. For skewness, kurtosis and the 50th percentile, the sensitivity and the specificity were 69.4% and 77.8%, 65.3% and 69.4%, 61.1% and 61.2% respectively. Conclusion: T2-MRI whole tumor histogram analysis can provide more quantitative information characteristics, which provides a new method for the differential diagnosis of children with astrocytoma and ependymoma.

MRI findings on cognitive impairment and alteration of cerebral grey matter density in COPD patients

Objective: To investigate the type of cognitive impairment, the change of grey matter density and the relationship between grey matter density change and cognitive impairment in the stable chronic obstructive pulmonary disease(COPD) patients. Methods: Twenty-five stable COPD patients and 26 healthy controls were enrolled from June 2014 to June 2016. MRI examination and neuropsychologic tests were performed in all the subjects. The voxel-based morphometry(VBM) method was used to compare the grey matter density between the two groups. Results: Compared to healthy controls, the COPD patients had a lower score on the mini-mental state examination(MMSE), montreal cognitive assessment(MoCA), digit span, digit symbol test, trail making-A test, and trail making-B test. The VBM results showed that the grey matter density of the right supramarginal gyrus, inferior parietal lobule and right cerebellum were significantly lowered compared to that of the controls. The grey matter density of the right cerebellum was positively correlated with the score of MoCA and digit span backward test. Conclusion: The cognitive impairment of the stable COPD patients lies mainly on non-amnestic cognitive impairment and presents with the impairment of the executive control function. Many brain areas exhibits lowered grey matter density in stable COPD patients. The reduced grey matter density is associated with the cognitive impairment. MRI examination can provide useful information in diagnosis and evaluation of the cognitive impairment in COPD patients.

The effect of type 2 diabetes mellitus on the gray matter of Alzheimer’s disease: a preliminary study involving DKI

Objective: To investigate the microstructure of cortex and gray nucleus in Alzheimer’s disease(AD) with and without type 2 diabetes mellitus(AD+T2DM, AD-T2DM) by measuring and comparing the diffusion kurtosis imaging(DKI) data. Methods: Thirty-five patients who were diagnosed AD by neurology department were included in the study and were divided into AD+T2DM group(13 cases, 6 males, 7 females, average age (74.46±8.26) years) and AD-T2DM group(22 cases, 9 males, 13 females, average age (70.82±8.72) years). The mental status was assessed by MMSE and MoCA scores by two neurologists. MR protocols included conventional MRI and DKI. The raw DKI data were processed using Functool 2 software in ADW 4.6 workstation. Bilateral MK, Ka, Kr, MD, Da, Dr and FA values in the superior frontal gyrus(SFG) and middle temporal gyrus(MTG), hippocampus(HIP), head of caudate nucleus(HCN), putamen(PUT), globus pallidus(GP) and thalamus(THA) were manually measured twice by two radiologists. Two independent sample t-test was used to compare the diffusion parameters in all regions and clinical indexes between the two groups. Spearman correlation analysis was used to analyze the relations between DKI parameters and MMSE or MoCA score. Result: Compared with those in AD-T2DM group, FA value significantly increased only in MTG(P<0.05), and then MD and Da value also increased in HCN and THA(P<0.05) in AD+T2DM group. Only in THA the MK and Ka value significantly decrease(P<0.05) in AD+T2DM compared to AD-T2DM group. In the correlation analysis, there were negative correlations between the diffusion rate of HIP, FA value of MTG and the clinical MMSE score in AD+T2DM group. However, there were negative correlations between the diffusing rate of HIP, FA value in SFG, MTG, THA and clinical MoCA score(P<0.05). Conclusion: For AD patients, hyperglycemia may aggravate the microstructure damage in gray matter and nucleus, and HCN and THA are probably the parts that most easily affected. DKI parameters also reflect cognitive status to some extent.

Comparison of mono and bi fitting method based IVIM imaging in breast lesions

Objective: To investigate the difference of parameters from mono and bi based intravoxel incoherent motion(IVIM)-deprived algorithm in breast lesions, and the value in differential diagnosis of benign and malignant breast lesions. Methods: IVIM-MR parameters from a total of 58 breast lesions(27 benign and 31 malignant lesions) were taken from 54 female, including D value, D* value and f value generated and measured by two fitting methods. SPSS 22.0 statistical software was used to compare the coefficient of variation of the parameters measured by two fitting methods, and investigated the difference between mono and bi based IVIM fitting methods. The diagnostic performance in differentiating benign and malignant breast lesions was also compared between mono and bi fitting methods. Results: The coefficient of variation of IVIM parameters by mono fitting method was less than bi from breast lesions. There were significant differences for D and D* values between mono and bi in breast lesions, but the f value was not. The values of D and D* were statistically different by mono fitting method in benign and malignant breast lesions, and the D values on bi fitting method was also so. The area under curve for D value measured by mono fitting method was larger than bi in differentiating benign and malignant breast lesions. Conclusion: Compared with the bi fitting method, mono fitting method is more valuable for the differential diagnosis of benign and malignant breast lesions, and is more suitable for IVIM parameter measurement of breast lesions.

The application value of IVIM in the assessment of early reaction after neoadjuvant chemotherapy for breast cancer

Objective: To investigate the monitoring ability of early response to neoadjuvant chemotherapy in breast cancer based on intravoxel incoherent motion(IVIM). Methods: A retrospective analysis was performed on 30 cases of female patients with breast cancer neoadjuvant chemotherapy, who underwent IVIM examination of multiple b-value dual exponential signal attenuation model. Fifteen b-values were used, including 0, 25, 50, 75, 100, 150, 200, 500, 700, 800, 900, 950, 1 000, 1 200, 1 400 s/mm2 respectively. We divided all cases into pre-chemotherapy group and post-chemotherapy group. And pathologic diagnosis as golden standard, divided post-chemotherapy group into valid group and invalid group. Results: Compared with pre-chemotherapy, ADCslow value increased and f value decreased significantly in post-chemotherapy group(P=0.001; P=0.021). After two cycles of chemotherapy ADCslow was higher in the valid group and f value was lower in the invalid group(P=0.000; P=0.000). ADCfast showed no statistical significance(P=0.408). The prediction performance of ADCslow was higher than f value with AUC of 88.9%. The AUC of f value and ADCfast were 74.3% and 64.9% respectively. After comparing the difference before and after chemotherapy, ΔADCslow and Δf value were statistically significant(P<0.05) with AUC of 92.4% and 88.9% respectively. ΔADCfast showed no statistical significance(P=0.211) and the AUC is 70.8%. Conclusion: IVIM has value in evaluating the efficacy of breast cancer chemotherapy. ADCslow and f value have such potential ability while ΔADCslow value most.

The application of cost-sensitive algorithm in the diagnosis of right ventricular failure by echocardiography

Objective: To improve the efficacy of echocardiographic method in evaluating right ventricular function, and to explore the value of cost-sensitive machine learning method model based on ultrasound parameters in the diagnosis of right ventricular failure. Methods: Eighty-two subjects with right heart failure(RHF group) and 106 subjects without right heart failure(non-RHF group) were enrolled in this study. Multi-echocardiographic indices were selected as features, including the right ventricular fractional area change(RVFAC), tricuspid annular plane systolic excursion(TAPSE), tricuspid flow peak velocity at early diastole(E), tricuspid annular peak systolic velocity(Sm) and peak early diastolic velocity(Em), Tei index, isovolumic acceleration(IVA) at tricuspid annular, and the diameter of the left ventricle, right ventricle, right atrium were measured and calculated. Cost-sensitive naive bayes analysis was used to build diagnosis model by using those parameters. The model was tested by 10-fold cross-validation. The diagnostic results of the cost-sensitive model and the classical naive bayes model were compared. Results: Compared with any single parameter, machine learning model can achieve better accuracy in diagnosis of right heart failure(91%). The cost-sensitive naive bayes method achieved a smaller overall cost. Conclusion: The diagnostic model established by machine learning is better than any single ultrasound parameter, which can reduce the diagnostic cost compared with conventional method. This method has potential practical value in screening of right ventricular failure by imaging.

A comparative study of iterative reconstruction technique in CT coronary dual-low scan stent imaging with different convolution kernels

Objective: To evaluate the improvement of Safire iterative reconstruction technique in CT coronary stent imaging with different convolution kernels, and to assess the influence of convolution kernel(I50f) image on coronary stent display. Methods: Forty-two patients with 84 coronary stents were examined by dual-source coronary computed tomography angiography(CCTA). Images were reconstructed at convolution kernel of I26f, I46f and I50f with Safire iterative reconstruction technique. Subjective and objective evaluation of image quality of the three groups were performed and compared by two experienced radiologists independently. Results: The subjective evaluation analysis of the three groups of images showed that images of I50f group was rated the best for clearly showing the details of the stents and its lumen showed the highest score of 4.07±0.32, and the difference in the quality of the three groups of images was statistically significant(P=0.000). With the increase of the convolution kernel value, the noise of the image increased, and statistical significance could be found among the three groups(P=0.000). The objective evaluation showed that the noise of in-stent lumen among the three groups was not significantly different(P>0.05). Conclusion: Safire technology can effectively reduce noise and improve image quality, meanwhile its combination with the convolution kernel of I50f is the best evaluation for stent display in this study, which can provide better image quality for clinics.

Significance of applying wave intensity technology to study Renying and Cunkou pulse manifestation

Objective: To analyze the feasibility of applying wave intensity(WI) technology in the study of Renying and Cunkou pulse manifestation through applying WI technology to detect bilateral radial arteries and common carotid arteries. Methods: Forty cases of healthy people were enrolled. Prosound α7 color Doppler ultrasonography of ALOCK company was used to detect the WI technology parameters of the bilateral radial arteries and bilateral common carotid arteries. DDMX-100 pulse condition apparatus was applied to detect Cunkou pulse manifestation parameters. The correlation of the parameters was analyzed. The WI parameters of bilateral radial arteries and bilateral common carotid arteries were compared and analyzed. Results: There were good correlations between the WI technology parameters of bilateral radial arteries as well as common carotid arteries and Cunkou pulse manifestation parameters. The difference between the WI technology parameters of the common carotid artery and those of the radial artery was statistically significant. Conclusion: This study demonstrated that the good correlation exists between the WI technical parameters of radial artery, common carotid artery and the pulse instrument parameters of radial artery. Therefore, WI technology can be applied to the study of Renying and Cunkou pulse condition.

Using the CT screen drawing line to plan and guide the percutaneous liver tumor

Objective: To explore the clinical value of using the CT screen drawing line method to guide the microwave ablation of percutaneous liver tumor. Methods: Thirty-nine cases of liver cancer patients(54 lesions, the average maximum diameter (2.6±1.3) cm) were enrolled. CT guided percutaneous microwave ablation was performed by CT screen drawing line plan. Real-time CT check after ablation was performed to observe and check whether the actual needle was corresponding to the plans. The success of screen wire guided puncture was evaluated. The complication and the recent therapy effect were analyzed. Results: The microwave ablation was successfully completed on the 54 lesions. Scattered small bubbles were seen in all lesions, in the 38 lesions with no iodine oil deposition, carbonized ablation railway lines were shown, which were consistent with plans. The completely melt rate was 94.4%(51/54), and partial residual rate was 5.6%(3/54). No severe tissue damage was found in surrounding important organs. Conclusion: It is worth promoting the use of screen drawing line to guide ablation needle puncture, which can ensure the complete ablation and ablation safety.

Quantitative research of DKI in the differential diagnosis of prostate cancer and #br# its correlation with pathological grades

Objective: To investigate the value of the related parameters of diffusion kurtosis imaging(DKI) in differential diagnosis between prostate cancer(PCa) and benign prostatic hyperplasia(BPH), and its relationship with pathological grades. Methods: A total of 57 cases of prostate diseases proved by surgery or biopsy, including 25 PCa and 32 BPH, were collected in the retrospective study. All patients underwent routine prostate MRI and DKI scan. Mean kurtosis (MK), mean diffusivity(MD) and fractional anisotropy(FA) values were obtained. The receiver operating characteristic curve(ROC) was used to evaluate the diagnosis efficiency of these parameters on PCa and BPH. The correlation between DKI parameters and the clinical pathological grade of PCa according to the American Joint Committee on Cancer(AJCC, 8th edition) was analyzed. P<0.05 was considered statistically significant. Results: The difference of MK and MD value between BPH and PCa were statistically significant(P=0.000). The ROC curve showed that the MK value has the greatest diagnostic efficacy(AUC=0.99). There were significant differences in MK and FA value among different clinical groups and Gleason scores(P<0.05). There was no significant difference in MD value among different Gleason scores(P>0.05). There was a significant positive correlation between the MK value of the DKI model and the AJCC clinical classification and Gleason score(r values were 0.860 and 0.805, respectively) and FA values were moderately negatively correlated(r values were -0.463 and -0.465, respectively). Conclusion: Quantitative analysis of DKI model is helpful for the differential diagnosis of BPH and PCa. The MK value is highly sensitive and specific in diagnosis, which can help predic tthe clinical pathological grade of PCa.

The value of R2* in quantitative evaluation of the biological behavior of endometrial carcinoma: a preliminary study

Objective: To explore the biological behavior of endometrial carcinoma(EC) by analysing the correlation between the R2* value of enhanced T2 star-weighted angiography(ESWAN) sequence and the pathological type, depth of myometrium infiltration, vascular invasion, invasion and metastasis of EC. Methods: We retrospectively analyzed the imaging data of 122 cases who were diagnosed as EC by postoperative pathology and underwent 1.5T MRI examination(including ESWAN sequence). The R2* values of EC lesions were measured. All patients were divided into four pairing groups: the high-risk pathological type group(type Ⅰ grade 3 and type Ⅱ, group A1) and low-risk pathological type group(type Ⅰ grade 1 and grade 2, group B1), deep myometrial invasion group(group A2) and non deep myometrial invasion group(group B2), vascular invasion group(group A3) and non vascular invasion group(group B3), invasion and metastasis group(stage Ⅱ, Ⅲ and Ⅳ, group A4) and non invasion and metastasis group(stage Ⅰ, group B4). The R2* values of the paired groups were compared by independent sample t test. The diagnostic efficiency of R2* in the diagnosis of EC high-risk pathological type, deep myometrial invasion, vascular invasion, invasion and metastasis was evaluated by receiver operator characteristic(ROC) curve, and the corresponding thresholds were found. Results: There were 40 cases in group A1, 82 cases in group B1, 44 cases in group A2, 78 cases in group B2, 25 cases in group A3, 97 cases in group B3, 27 cases in group A4 and 95 cases in group B4. The R2* values of the four pairing groups were (17.60±4.02) vs. (12.81±2.66) Hz, (17.57±3.96) vs. (12.58±2.40) Hz, (18.62±4.68) vs. (13.29±2.75) Hz, (18.33±4.22) vs. (13.26±2.94) Hz, respectively, and the differences had statistically significant(t=6.840, 7.613, 5.456, 5.858, P<0.001). The AUC of R2* values for EC high-risk pathological type, deep myometrial invasion, vascular invasion, invasion and metastasis were 0.856, 0.876, 0.834 and 0.855, respectively. The thresholds for predicting high-risk pathological type, deep myometrial invasion, vascular invasion, invasion and metastasis of EC were R2* value≥15.24 Hz, R2* value≥14.76 Hz, R2* value≥14.67 Hz, R2* value≥15.25 Hz, respectively. The sensitivity and specificity were 77.50% and 84.00%, 82.90% and 68.00%, 79.50% and 85.20%, 79.50% and 76.80%, respectively. Conclusion: R2* value can be used as a quantitative index for evaluating the biological behavior of EC in non-enhancement MR.

The value of assessment of median nerve mobility by ultrasonic dynamic imaging in #br# the diagnosis of carpal tunnel syndrome

Objective: To analyze the severity of carpal tunnel syndrome(CTS) by evaluating the transverse displacement of the median nerve during the active flexion and extension of the fingers through the real-time ultrasound images, and to further explore the relationship between median nerve activity and the severity of nerve conduction. Methods: The vertical distance between the median nerve and the ulnar artery was measured in 80 CTS patients(41 mild and 39 severe) and 103 healthy volunteers at the extension and flexion positions of the fingers respectively using Philips HD15 ultrasonic diagnosis apparatus, and to calculate the difference between the distances of them(ie, transverse displacement of the median nerve). Results: The transverse displacement of the median nerve in each group of CTS was smaller than that in the control group(P<0.01). The transverse displacement of the median nerve was positively correlated with the distal motor latency and negatively correlated with the sensory conduction velocity(P<0.01). Conclusion: Ultrasound can dynamically show the activity of the median nerve during finger movement by measuring the transverse displacement of the median nerve during the active flexion and extension of the finger. The changes in transverse displacement(ie, transverse slide) of the median nerve in the carpal tunnel were related to the presence and severity of CTS. The transverse sliding patterns of the median nerve during finger movements were demonstrated to be useful for quantitatively evaluating median nerve dysfunction in CTS patients.

The application value of MSCT in classifying distal radius fracture in elderly patients

Objective: To investigate the application value of MSCT in the classification of distal radius fracture in elder patients after comparison with X-ray. Methods: Ninety-six elderly patients with distal radius fractures were retrospectively enrolled with 34 males and 62 females, aged from 60 to 86 years((67.4±7.6) years in average). All cases performed X-ray and CT examination before any therapy. Detection results of different types of fractures were compared between the X-ray and MSCT scans and its image reconstruction, referring to the classification of AO/ASIF(Arbeitsgemeinschaft für Osteosynthesefragen/Association for the Study of Internal Fixation). Results: Among the 96 cases, 88 cases(91.67%) were diagnosed as fractures by X-ray and 8 cases were misdiagnosed. Patients with type A, B and C fractures were 45 cases(46.88%), 9 cases(9.38%) and 34 cases(35.42%) respectively. Ninety-six cases with fractures were diagnosed by MSCT and patients with type A, B and C fractures were 11 cases(11.45%), 10 cases(10.42%) and 75 cases(78.13%) respectively. There were statistical difference between the two technologies in evaluating the classification of distal radius fracture(χ2=35.02, P<0.01), with less consistency(Kappa=0.242, P<0.01). Conclusion: MSCT is more detailed and accurate for differential diagnosis and classification of elderly distal radius fractures, especially for complex intra-articular fracture, compared with conventional X-ray.

Pregnancy rate and affecting factors of infertile women due to tubal obstruction treating with fallopian tube recanalization

Objective: To study the pregnancy rate and affecting factors of infertile women due to tubal obstruction treating with fallopian tube recanalization(FTR). Methods: Totally 68 qualified infertile women were included. After collecting their clinical information, we evaluated the pregnancy rate and analyzed the affecting factors with chi-square test, Fisher’s exact test and Logistics regression analysis. Results: The pregnancy rate was 60.29%, and the age, the type of infertility, pelvic operation history, the duration of infertility, unilateral or bilateral tubal obstruction, the shape of fallopian tube after dilation as well as the contrast agent diffusion in pelvic cavity affected the pregnancy rate insignificantly(P>0.05). Nevertheless, among women aged <35 years at the time of FTR, single variable analysis implied that the type of infertility was associated with successful pregnancy(P<0.05); In addition, the type of infertility and the even or uneven diffusion of contrast agent in pelvic cavity were the two independent risk factors of successful pregnancy according to Logistics regression analysis(b=2.410 and 1.876 respectively, P<0.05). However, none of the studied variables was significantly associated with pregnancy among women aged ≥35 years(P>0.05). Conclusion: FTR is an effective treatment with high pregnancy rate for infertile women suffering from fallopian tubal obstruction. Women aged <35 years were recommended with spontaneous pregnancy without technical assistance after FTR, and the primary infertility and the uneven-diffusion of contrast agent in pelvic cavity are the two independent risk factors. The pregnancy rate among women aged ≥35 years is so low that assistant reproductive technology is recommended after FTR.
