837 Clinical application of susceptibility-weighted imaging in monitoring aterial thrombosis in ischemic stroke

Abstract: Objective: To investigate the sensitivity of susceptibility-weighted imaging(SWI) in displaying atery thrombosis in ischemic stroke, and evaluate the role of SWI in the assessment of arterial thrombosis in ischemic stroke. Methods: Thirty-six patients(19 males, 17 females, age 20~83 years, average 65 years) with acute cerebral infarction(within 24 hours after clinical onset) performed and followed up with SWI. The presence of correlated atery thrombosis within the infarction were observed, the correlation between the extent of the correlate aterial thrombosis. NIHSS was analyzed. Results: The correlated aterial thrombosis was revealed by SWI in 27 out of 36 massive cerebral infarctions, the proportion was 75%. Seventeen were signifcantly showed(the length of aterial thrombosis >3cm), 10 were midly showed on the SWI(the length of aterial thrombosis <3cm). There was a significant positive correlation between the severe ischemic stroke aterial thrombosis displayed in the SWI and the NIHSS scores(rs=0.782, P<0.01). Conclusion: SWI was sensitive in detecting the atery thrombosis in infarction, which can be used to predict the evolution of ischemic stroke, which was helpful in guiding the clinical treatment.

840 Experience of 320-slice whole brain dynamic volume CT and perfusion imaging in patients with moyamoya disease

Objective: To evaluate the value of 320-slice whole brain dynamic volume CT angiography and CT perfusion imaging in moyamoya disease. Methods: Eighteen cases with moyamoya disease were examined with 320-slice whole brain dynamic volume CT. Imaging data were generated with volume rendering reconstruction and display in cine mode, and complete CT perfusion imaging were performed simultaneously. Results: We succeeded in obtaining plain images of whole-brain, dynamic CTA and whole-brain perfusion images in 18 patients, all of which showed lesions of stenosis and obstruction in internal carotid artery (ICA), proximum of Willis circle and posterior and middle cerebral arteries; abnormal vessel network in the basal part of brain; and the condition of collateral circulation in the brain. Perfusion weighted imaging revealed mean transit time(MTT) was extended(0.5~13s). Ten cases showed decrease in regional cerebral blood flow(rCBF), the amplitude was 26.0%~54.5%. There were 8 cases of normal and 4 cases decreased in regional cerebral blood volume(rCBV). Conclusion: Whole brain dynamic volume imaging with 320-detector row CT can provide 3D structure of vessels of the whole brain, and serial of the dynamic blood flow and cerebral perfusion in a single scanning, which has the potential diagnostic value in moyamoya disease.

844 MDCT measurement of cervicocerebral arteries of normal adult

Objective: Using multi-detector row CT angiography to measure the value of normal dimension of cervicocerebral arterial trunks and their main branches. Methods: One hundred and twenty adults performing multi-detector row CTA with no cervicocerebral arterial diseases and blood flow abnormalities, were divided into 3 groups according to their age(<40y, 40~60y, >60y), with 40 cases in each group(20 cases for each gender). Images were taken to the workstation and measurements were done on 3D Images. All the results were statistically analyzed. Results: Results from measurements had shown that dimensions of 20 branches(60.6%) had significant difference between two genders(P<0.05). Dimensions of two genders’ ACoA, male’s PCoA, female’s CCA, ECA, BCA and LSCA had significant difference in different age group(P<0.05). Conclusion: Cervicocerebral artery trunks and their branches can be clearly shown by multi-detector row CTA. Measurement of the normal dimension of cervicocerebral artery provides valuable imaging information in the diagnosis of cervicocerebral arterial diseases and diseases that can produce morphologic change of cervicocerebral artery.

849 Relationship between TNF-α, hsCRP and ultrasonic imaging of carotid atherosclerosis

Objective: To study the relationship between serum level of TNF-α, hsCRP and ultrasonic imaging of carotid atherosclerosis. Materials and Methods: The patients with final diagnosis of carotid atherosclerosis as well as healthy individuals were selected and their serum level of TNF-α, hsCRP detected. Statistic methods were used in the interrelationship analysis of the serum level of TNF-α, hsCRP and degree of carotid atherosclerosis. Results: Totally 62 patients with carotid atherosclerosis and 25 healthy individuals were enrolled. The carotid atherosclerosis with ultrasonic image of intimae echo enhancement, rough surface, worse continuity and different shape as well as inequality of size of atherosclerotic plaque were analysed. The serum level of TNF-α in patients with carotid atherosclerosis was (51.4±6.6)μg/L, and was (8.8±1.2)μg/L in healthy individuals, which had significant difference(P<0.01). The serum level of hsCRP in patients with carotid atherosclerosis was (8.8±0.56)mmol/L, and (8.3±0.43)mmol/L in healthy individuals, which had significant difference(P<0.01). It had positive correlation with degree of carotid atherosclerosis(P<0.01). Conclusion: The serum level of TNF-α, hsCRP was significantly higher than those of the healthy individuals, and has positive relation with degree of carotid atherosclerosis.

852 Differentiating segmental autoimmune pancreatitis from pancreatic carcinoma and pancreatic endocrine tumors on the basis of enhancement characteristics at dual-phase CT

Objective: The purposes of this study was to investigate the difference of segmental autoimmune pancreatitis(AIP) from pancreatic carcinoma and pancreatic endocrine tumors(PECT) at dual-phase CT, and to improve the level of diagnosing AIP. Methods: Dual-phase CT scans of 75 patients(9 with segmental autoimmune pancreatitis, 25 with pancreatic carcinoma, 16 with pancreatic endocrine tumors, and 25 with a normal pancreas) were analyzed. Analyze the image findings including the uniformity of density, the shape of lesion, and the contrast density of masses to normal pancreatic parenchyma. The CT attenuation value of the pancreatic parenchyma and pancreatic masses were measured on plain scanning and on arterial- and portal venous phases. Compare the mean CT attenuation value of AIP to pancreatic carcinoma and PECT respectively, all of which were compared with normal pancreas. Results: In the arterial phase, all AIP manifested as hypodense to uninvolved pancreas, and all pancreatic carcinoma manifesed significantly hypodense to normal pancreas, and all PECT manifested hyperdense to normal pancreas; the mean CT attenuation values of AIP, pancreatic carcinoma, PECT and control group were 72.7HU, 49.2HU, 115.6HU and 93.4HU respectively. In the portal venous phase, all AIP manifested slightly hyperdense to uninvolved pancreas, and all pancreatic carcinoma manifesed significantly hypodense to normal pancreas, and all PECT manifested hyperdense to normal pancreas; the mean CT attenuation values of AIP, pancreatic carcinoma, PECT and control group were 86.6HU, 57.6HU, 104.2HU and 83.9HU respectively. Conclusion: At dual-phase CT, the enhancement patterns of AIP was different from that of the pancreatic carcinoma, the pancreatic endocrine tumor and normal pancreas. Dual-phase CT scanning played an important role in differentiating AIP from pancreatic carcinoma and pancreatic endocrine tumors.

858 The research of endothelial function and renal pathology in early-stage chronic kidney disease(CKD) patient

Objective: To study the endothelial dysfunction in early-stage chronic kidney disease(CKD) patients and to explore the relationship between Flow-Mediated Dilation(FMD) and renal pathology. Methods: Sixty subjects were divideded evenly into 3 groups: group A: normal control group; group B: renal patients without renal insufficiency; group C: renal hypertension patients without renal insufficiency. Each group underwent ultrasonographic examination of brachial artery diameter in the basic condition and the reactive hyperaemia. All renal patients had renal biopsy under ultrasound guidance. The degree of renal tissue damage made semiquantitative integral. We correlated endothelial function with age, pressure and renal pathology. Results: The FMD was impaired significantly in group C than that in group A and group B, but there was no difference in group A and group B; The FMD had a high correlation with systolic blood pressure, glomerular and interstitial damage index but no correlation with age and diastolic pressure. Conclusions: The endothelial function is impaired in renal hypertension patients without renal insufficiency; As the endothelial function becoming worse, renal pathological damage increased accordingly; Evaluation of the endothelial function by the high-resolution ultrasonography in early-stage CKD patients have clinical improtance.

860 Carcinoma of renal collecting duct: correlation with characteristics of dynamic enhanced MDCT and pathological basis

Objective: To investigate the relationship between dynamic enhanced MDCT and histopathologic findings of the collecting duct carcinoma(CDC), in order to improve diagnostic accuracy. Methods: Seven cases with pathologically proven CDC of the kidney underwent spiral CT plain scan, post-contrast cortico-medullary phase and nephrographic phase scans before surgery. Results: All of the 7 tumors presented normal renal contour with tumor located in renal medullary. All tumors manifested an infiltrative growth pattern and were ill-defined, and displayed an interlocking arrangement with the surrounding normal renal parenchyma, no capsule was found. The tumor density was correlated with the composition of tumor in pathology. The CT value of the solid component on plain scan images was 30.1~44.7HU(mean=35.5HU), after contrast enhancement, the CT value of tumor on cortico-medullary phase images and nephrographic phase images was 34.2~72.0HU(mean=52.7HU) and 40.0~89.3HU(mean=74.5HU) respectively. Progressive delayed enhancement after contrast enhancement was related to pathologically apparent stromal reaction and fibrosis, while the fibrous stroma often manifested a delayed enhancement. Conclusion: The CT characteristics of CDC were different from the other renal cell carcinoma and the image findings are closely related to pathology.

864 Quantitative estimation of the relationship between foramen ovale-related parameters and the hypoplastic of left heart using fetal echocardiography

Objective: To investigate the clinical significance of foramen ovale-related parameters examined by echocardiography in the prenatal diagnosis of hypoplastic left heart. Methods: One hundred and eighty-eight cases of fetal foramen ovale-related parameters were obtained, to analyze its change with the gestational age, and study the relationship between foramen ovale-related parameters and fetuses with hypoplastic left heart. Results: In the 188 cases, 165 cases were nomal and 23 cases were abmomal associated with hypoplastic left heart(12 cases were hypoplastic left heart syndrome, 8 cases were aortic valve aortic stenosis, 3 cases were coarctation of aorta). Normal fetal foramen ovale valve flap diameter was similar to the aortic diameter, and normal fetuses had a smaller ratio of foramen ovale valve flap diameter/aortic root size than fetuses with hypoplastic left heart(P<0.01), but had a larger ratio of foramen ovale valve flap diameter/atrial septum size(P<0.01). Conclusion: The foramen ovale-related parameters examined by echocardiography has important significance in prenatal diagnosis of hypoplastic left heart.
