457 MRI features and pathology of intracranial neuronal and mixed neuronal-glial tumor

Objective: To investigate MRI characteristics and pathology of intracranial neuron and mixed neuronal-glial tumor. Methods: Clinical data, histopathology and MRI findings in 26 patients with intracranial neuron and mixed neuronal-glial tumor proved by pathology were analysed retrospectively. Results: Eleven dysembryoplasia neuroepithelial tumor(DNT), mainly seen in the supratentorial brain and beneath cerebral cortex, some were seen as cystic masses, appearing as low signal intensity on T1WI and high signal intensity on T2WI. DNT had a well-demarcated and no or only slight edema, no enhancement or slight inhomogeous enhancement; 11 gangliogliomas mainly seen in the supratentorial brain appearing as low signal intensity on T1WI and high signal intensity on T2WI. Two showed mixed signal intensity, margin of lesion was well-demarcated and no edema or slight edema, no enhancement or slight uneven enhancement, 3 central neurocytomas, which were located in the lateral cerebral ventricle, all showed mixed signal intensity on T1WI and T2WI, with cystic components. Enhancement was inhomogeneous; 1 desmoplastic gangliogliomas seen as cystic masses, located in fronto-temporal-apical lobe, had septation, appearing as low signal intensity on T1WI and high signal intensity on T2WI. Conclusion: Intracranial neuronal and mixed neuronal-glial tumours had no characteristic imaging features. It should be closely referred with clinical manifestations.

461 Ischemic lesions on MRI and histological outcomes after different transient periods of focal ischemia in rabbits

Objective: To investigate the time course of ischemic changes on MRI(including DWI and T2WI) after different brief periods of focal brain ischemia in rabbits and to evaluate the histopathological outcomes in the regions of DWI abnormalities. Methods: Thirty rabbits were subjected to 7 minutes(n=10); 30 minutes(n=10) short time obliteration of middle cerebral arterial occlusion and sham operation(n=10). MRI(including DWI and T2WI) were performed during and before occlusion; 5min after occlusion and at 0.5, 2, 6, 12, 24, 48 and 72 hours of reperfusion. After the last MRI study at 72 hours of reperfusion, the brains were evaluated by histopathology. Results: No MRI abnormalities were observed in the sham-operated rabbits. In the 7-minute and 30-minute groups, the perfusion deficits and DWI hyperintensities that occurred during occlusion disappeared shortly after reperfusion. The DWI and T2WI results remained normal in the 7-minute group, whereas secondary DWI hyperintensity and T2WI abnormalities developed at the 6~12 hour observation point in the 30-minute group. Histological examinations demonstrated neuronal necrosis in the two groups, but the number of necrotic neurons was significantly higher in the 30-minute group than in the 7-minute group(P<0.001). Conclusions: Transient or permanent resolution of initial DWI lesions after reperfusion depends on the duration of ischemia. Transient resolution of DWI lesions was associated with widespread neuronal necrosis; moreover, permanent resolution of DWI lesions did not necessarily indicate complete salvage of brain tissue from ischemic injury. These results may help to explain the neurological deficits in some patients who have normal DWI results after cerebral ischemia and cognitive deficits in some patients who experienced transient ischemic attacks.

465 The imaging characteristics of hereditary spastic paraplegia with thin corpus callosum

Objective: To analyze the imaging characteristics of hereditary spastic paraplegia with thin corpus callosum(HSP-TCC), so as to get a better understanding of the disease. Methods: Three patients from the same family with HSP-TCC were reported with reviewing of the literature. Results: Thin corpus callosum, long T2 signal in the thinning cervical and thoracic spinal cord were seen. Diffused unspecific white matter lesions with mild brain atrophy, were also seen in the three HSP-TCC cases of this group. Conclusion: The central nervous system may be affected extensively by HSP-TCC, characteristic MRI signs including thin corpus callosum, long T2 signal in the white matter around lateral ventricle and in the spinal cord, atrophy and abnormal signal of the spinal cord, as well as subsequent mild or moderate brain atrophy.

469 Interatrial septal aneurysm associated with anomalies

Objective: To explore the characteristics of interatrial septal aneurysm(IASA) complicated with anomalies and its treatment. Methods: Thirty-two inpatients with IASA and complicated with anomalies and 20 outpatients with isolated IASA were identified in our department. In 20 outpatients(4 male, 16 female) with isolated IASA, 8 patients accompanies with patent foramen ovale(PFO), ages ranged from 3.5~54 years old, with an average of 36.38±15.15. In 32 inpatients(8 male, 24 female) with IASA complicated with anomalies, ages ranged from 3 months to 61 years old, with an average of 31.88±18.28. Length of IASA, height of IASA, diameter of right ventricle, and cardiac function tests of the left ventricle were evaluated by electrocardiogram, X-ray, transthoracic echocardiography(TTE), transesophageal echocardiography(TEE). The patients with IASA and complicated with anomalies underwent catheterization and interventional therapy or surgery. Results: In 32 patients with IASA and complicated with anomalies, 16 patients(50%) underwent surgery and 16 had interventional therapy. Multiple atrial septal defects(ASDs) were found, 7 patients underwent surgery and 6 underwent interventional therapy. Conclusion: The commonest anomalies complicated with IASA are ASDs, which are often multiple. TTE and TEE are the key methods to select patients for interventional therapy or surgical treatment.

473 The evaluation of aorta’s compliance with different age by MRI

Objective: To evaluate the aorta’s compliance with MRI. Methods: Sixty-three normal volunteers with different age were measured ascending aorta by phase-contrast cine MR scan. We got the time-area curves, and calculated the scores of compliance by diameter’s methods and area’s methods. Results: With increase of the age, aortic compliance reduced, the scores of compliance increase. The correlation between age and aortic compliance measured by area’s methods was better than by the diameter’s methods(r=-0.6972). MR measurement methods can be highly repetitive(r=0.91). Conclusion: MRI can be used as a better means of observation of aortic compliance.

475 The primary study of the correlation between the MSCT perfusion value ofpancreas and the severe indexes of acute pancreatitis

Objective: To study the correlation of blood perfusion values with CT severe index(CTSI) and clinical severity of acute pancreatitis(AP). Methods: Seventy-nine patients and in which 63 cases had AP and 16 normal cases as control were enrolled in this study and all of them had undergone MSCT perfusion imaging. The mean values of blood flow(BF), blood volume(BV), mean transit time(MTT) and permeability surface(PS) were measured on ADW 4.2 workstation with perfusion 3 software package. Using statistical software was to emphatically analyze the difference between the AP and control group and the correlation of perfusion values with CTSI and APACHEⅡ respectively. Results: There was a significant difference between the AP and control group and a negative correlation of MSCT perfusion values of BF and BV with the CTSI and APACHEⅡ scores(r=-0.865, r=-0.703; r=-0.817, r=-0.703; P<0.01), but MTT and PS values did not have correlation with CTSI and APACHEⅡ scores(r=-0.185, r=0.094; r=-0.192, r=0.141; P>0.05). Conclusion: BF and BV are markedly decreased in AP. BF and BV have a significant correlation with the CTSI and clinical severity of AP and can be the reliable indexes in assessing the severity of AP.

479 The value of CT virtual endoscopy and relative post-processing techniques forintestinal diseases using 64 multi-slice spiral CT

Objective: To study the diagnostic value of CT virtual endoscopy(CTVE) and relative post-processing techniques using 64 multi-slice spiral CT(MSCT) for intestinal diseases. Methods: Seventy-five cases with clinically suspected intestinal diseases had MSCT enhancement scanning with air filling of the bowel lumen. Image post-processing were done at the ADW 4.4 workstation, including CTVE, MPR, VR, VD and MIP images. Comparison between the diagnostic of MSCT and the results of surgery, pathology and clinical follow-up was performed to assess the diagnosis value of 64 MSCT for intestinal diseases. Results: Of the 75 cases examined, 49 cases had organic diseases by combination diagnosis. MSCT identified positive findings in 45 patients. Five cases were missed and 1 was false positive. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy was 89.9%, 96.2%, 97.8%, 83.3% and 92% respectively for MSCT. The subtle lesions of the mucosal surface and morphological changes were well showed by CTVE. Combined with other post-processing methods, intestinal diseases could be diagnosed correctly. Conclusion: CTVE and relative post-processing techniques of 64 MSCT are of significant clinical value in diagnosing intestinal diseases.

484 Correlative analysis of left ventricular functional parameters and transplanted renal hemodynamic parameters

Objective: To investigate the correlative analysis of left ventricular functional parameters and the transplanted renal hemodynamic parameters. Methods: According to Tei index 103 patients with kidney transplantation were divided into group A(0.40<Tei<0.47, 51 cases), group B(0.47≤Tei<0.79, 39 cases), group C(Tei≥0.79, 13 cases). The following indexes of cardiac function(E/A, DT, EF, Tei) were measured by echocardiography. The following hemodynamic parameters of transplanted kidney(PSV, EDV, MN, Q, RI, PI) were measured by abdominal ultrasound examination. Results: There was no statistical significant difference in hemodynamic parameter between group A and D, P>0.05. There was statistical significant difference in hemodynamic parameter between group B and C, P<0.05. EF was positively correlated with PSV, EDV, MN, Q and negatively correlated with RI, PI. DT was negatively correlated with PSV, EDV, MN, Q and positively correlated with RI, PI. E/A had no significant correlation with hemodynamic parameters of transplanted kidney. Tei index was negatively significantly correlated with PSV, EDV, MN, Q and positively significantly correlated with RI, PI. Conclusions: For patients with kidney transplantation, Tei index reflected left ventricle allomeric function was significantly correlated with transplanted renal hemodynamic parameters.

487 Comparative value of transrectal color & power Doppler ultrasonography with microvascular density ofhistopathology by immunochemistry in diagnosing prostate cancer

Objective: To explore the value of transrectal ultrasound(TRUS), color Doppler flow imaging (CDFI) and power Doppler ultrasonography(PDU) in diagnosing prostate cancer, and correlated with the expression of microvascular density(MVD) in histopathology by immunochemistry. Materials and Methods: In 23 cases of prostate cancer and 21 cases of prostate hyperplasia, the images acquired by two-dimensional(2D) TRUS, CDFI and PDU were analyzed retrospectively. The morphology of prostate was analyzed by 2D sonography. The arterial flow signal of prostate was displayed by CDFI, and the resistance index(RI) was recorded. The classification of arterial flow in the prostate was performed by PDU. And then the MVD expression was detected by immunohistochemistry technique(SP method). Results: ①The accurate rate of TRUS sonography in diagnosing prostate cancer and prostate hyperplasia was 82.6%(19/23) and 90.5%(19/21) respectively. The diagnostic accuracy was increased by 2D sonography combined with CDFI and PDU. ②The classification of arterial flow, RI and MVD expression in prostate cancer were higher than those in prostate hyperplasia with significant difference(P<0.001). The classification of arterial flow by PDU was positively interrelation with MVD expression by immunohistochemistry in prostate cancer. Conclusions: The morphology of prostate was well displayed by 2D TRUS combined with CDFI and PDU, also the classification of arterial flow and RI in prostate were detected. Correlated with the MVD expression, the vascular growth in prostate cancer was confirmed in different aspects. It played a key role in evaluating the biology of prostate cancer.

490 Accuracy of 3.0T diffusion-weighted MRI for diagnosing the stage and grade of prostate cancer

Objective: To evaluate the ability of DWI in determining the stage of prostate cancer(PCA), and to measure the correlation between apparent diffusion coefficient(ADC) with histologic grade. Materials and Methods: Forty-six patients underwent DWI with a 3.0T MR scanner using a pelvic array multi-coil. The ADC values of PCA were calculated. Results: The overall accuracy of T stage diagnosis was 80% for T2WI alone, 83% for DWI alone, 89% for T2WI plus DWI. The AZ value was significantly higher(P=0.02, 0.04) for DWI plus T2WI(AZ=0.91) than for T2WI alone(AZ=0.82) and for DWI alone(AZ=0.83). The mean ADC of G1 tumors was 1.31×103mm, 1.14×103mm for G2, 0.7×103mm for G3, The mean ADC of G3 tumors was lower than that of G1 and G2 tumors(P<0.05). Conclusion: Combined T2WI and DWI is better than DWI alone and T2WI alone in the accuracy of T stage diagnosis. The ADC value may predict the histologic grade of PCA.
