405 A clinical study on carotid elasticity in diabetic retinopathy patients by E-Tracking technique

Objective: To detect carotid elasticity in patients with diabetic retinopathy by E-Tracking technique. Methods: Ninety patients of type 2 diabetes and 30 healthy volunteers were recruited in the study. The systolic diameter of the carotid artery(Ds), thickness of intima media(IMT), peak systolic velocity(PSV), resistance index(RI), pressure strain elastic modulus(Ep), stiffness parameter(β), arterial compliance(AC), pulse wave conducting velocity(PWVβ), augmentation index(AI) were evaluated. Results: ①The Ds, PSV, RI of group A, B, C were not obviously different from those of the control group. The IMT of group B and C was statistically different from that of the control group(P<0.05 and P<0.01). ②Ep, β, PWVβ and AI of group A were higher than those of the control group(P<0.01 for Ep and β; P<0.05 for PWVβ and AI); AC of group A was lower than that of the control group(P<0.05). Statistic difference of the Ep, β, PWVβ, AC existed among different diabetic groups(P<0.05). Conclusion: In patients with type 2 diabetes, damage of endothelium function already exists before the morphology changes of sclerosis observed in carotid artery. The elasticity of carotid artery decreases more obviously in patients with diabetic retinopathy.

408 Association between Tei index and left ventricular filling models as well as prognosis in patients with acute myocardial infarction

Objective: To investigate the correlation of Tei index with left ventricular filling models and prognosis in patients with acute myocardial infarction(AMI). Methods: One hundred and sixty-five patients with AMI were studied with echocardiography. Tei index was defined as the sum of isovolumetric contraction time(ICT) and isovolumetric relaxation time(IRT) divided by ejection time(ET), which were measured from mitral flow and left ventricular outflow Doppler velocity profiles. According to mitral E-wave deceleration time(EDT) and early diastolic flow propagation velocity(Vp), a total of 165 patients with AMI were divided into normal filling, impaired relaxation, pseudo-normal and restrictive filling group. Multivariate Cox proportional hazards analysis was performed to identify independent predictors of cardiac death. Results: Left ventricular filling was normal in 43 cases; impaired relaxation in 55 cases; pseudo-normal in 35 cases; and restrictive in 32 cases. Compared with normal filling group, Tei index was higher in impaired relaxation, pseudo-normal and restrictive group(P<0.05~0.01); left ventricular end-diastolic volume index(EDVI) and end-systolic volume index(ESVI) were significantly increased in restrictive group (P<0.05~0.01). During follow-up of 12(range 0~17) months, 22 patients(13.3%) died. Compared with surviving group, Tei index was higher in cardiac death group(P<0.01). Cox analysis illustrated that Tei index was a powerful predictor of cardiac death(relative hazard coefficient is 3.997, P=0.028). Conclusions: Tei index reflects severity of left ventricular dysfunction and have independent prognostic value in patients with AMI.

412 Liver changes demonstrated by B-mode ultrasound in patients with long term treatment of antipsychotics

Objective: To study the liver changes in schizophrenic patients treated with long term antipsychotics by B-mode ultrasound. Methods: B-mode ultrasound examination was given to 1255 schizophrenic patients who had used antipsychotics more than 2 years. In the 1255 patients, the first group contained 472 patients who had the treatment for more than 5 years, and the second group contained 783 patients who had the treatment less than 5 years, this group served as the control group. Related items were compared between the two groups. Results: In the first group, intake antipsychotics for more than 5 years, the liver abnormality detected by B-mode ultrasound was 28.05%, among which fatty liver is the most common abnormality, it accounts for 88.09% of all the abnormalities in this group. The change of liver is related to age, course of the disease and the duration of administration of the antipsychotics. The positive rate of liver changes was higher in patients who were more than 50 years old; who had longer course of the disease and who had used medicine 5 years or longer than 5 years, but there were no statistical differences of sex and the type of schizophrenia. Conclusion: B-mode ultrasound can indicate clearly the lesions of liver in patients with long term taken antipsychotics.

415 Integrin receptor MR imaging with targeted superparamagnetic liposome

Objective: The ability of αvβ3 integrin targeted superparamagnetic liposomes to detect tumor angiogenesis was evaluated by in vitro fluorescence microscopy and in vivo MR molecular imaging. Methods: SPIO or calcein loaded liposomes were conjugated with or without Arg-Gly-Asp(RGD) peptides. Human umbilical vein endothelial cells(HUVECs) were incubated with calcein loaded targeted and non-targeted liposome or PBS and analyzed by flow cytometry and confocal laser scanning microscopy. VX2 tumor bearing rabbits were injected I.V with the two liposome preparations. MRI scanning was performed and the contrast to noise ratios(CNR) of the tumor pre- and post-contrast agent injection were compared. Results: Binding and uptake of RGD-liposomes by HUVECs was significantly increased when compared with non-targeted liposomes. After the contrast agent injection, the signal intensity and CNR of tumor drop to the bottom at 15 hour and 2 hour respectively for the targeted group and non-targeted group. Conclusion: The RGD targeted super-paramagnetic liposome we prepared got high relaxivities. It can bind specifically with αvβ3 integrins and be detected by in vivo MR imaging by 1.5T clinical MR machine.

418 Study of the feasibility of a novel, clinical applicable cell labeling method to trace the transplanted cells in vivo

Objective: To find a new labeling agent to trace and visualize the transplanted cells in vivo. Methods: Bone marrow MSCs were incubated with SPIO for 36 hours. The labeling efficiency was tested through Prussian Blue staining, the growth ability was evaluated through MTT, and cells viability was tested through Trypan Blue rejection method, the migratory ability was assessed with Costar Transwell plates, the differentiation of labeled cells to adipogenic and osteogenic cells was induces with induction medium. The labeled or unlabeled cells were transplanted into the myocardium or coronary artery. MRI was carried out immediately and 1~4 weeks respectively. After MRI the heart was excised, the segment in which injections performed were thin cut and stained with hematoxylin-eosin and Prussian Blue staining. Results: There were intracytoplasmic blue particles in nearly every cell in Prussian Blue staining. SPIO had no poison effect on the cell growth and proliferation. The viability of cells was more than 98%. The migratory ability was not affected. The labeled MSCs could still be differentiated from adipogenic and osteogenic cells after being incubated in the inductive medium. The labeled cells could be detected through MRI and confirmed by pathology. After 4 weeks the injected labeled cells could still be detected through MRI. The pathology showed the injected cells could survive in the MI area, and parallel in the same direction. Conclusion: The cells could be efficiently and safely labeled with SPIO and the labeled cells could be reliably detected by MRI in vivo.

423 A comparative study of BOLD-fMRI between acute stage and early chronic stage of cerebral infarct

Objective: To investigate the cortical activation changes after stroke. Materials and Methods: BOLD-fMRI findings in 16 patients with cerebral infarction confirmed by clinical and imaging materials were analyzed. Results: The activation of patients were mainly located in the contralateral SM1 and occasionally scattered in the non-SM1 cortex. There was no statistical significance in activation volumes, activation frequencies of cortex relative to movement by longitudinal comparison between acute stage and early chronic stage. At the same time, the difference of grip strength of the two stages had no statistical significance. For better rehabilitation patients, the grip strength was becoming higher(P<0.05), the activation volume was becoming bigger too(P>0.05). There were three forms of changes in the contralateral SM1: became larger, became smaller and no obvious changes. Conclusions: BOLD-fMRI is a good way for developing the changes of cortical activation. Both in acute and early chronic stage, the motor areas can be activated. The contralateral SM1 is an important motor area. The change forms of SM1 were various.

427 Study of patent foramen ovale and its shunt direction by transthoracic echocardiography in children

Objective: Using transthoracic echocardiography(TTE) to investigate the prevalence, characteristic and shunt direction of patent foramen ovale(PFO). Methods: A total of 426 children were divided into infant group(0~3 year, 243 cases) and child group(4~7 year, 183 cases), then divided into infant associated with congenital heart disease(CHD) group(group A, 93 cases), normal infant group(group B, 150 cases), child with CHD group(group C, 60 cases) and normal child group(group D, 123 cases) according to whether complicating CHD. The characteristic and shunt direction of PFO was observed by TTE. Right-sided contrast echocardiography were performed if necessary. Results: Eighty-six cases were detected to have PFO in all the 426 children. TTE showed echo separating between foramen ovale valve and deuto-atrial septum from 1.5mm to 5.7mm and small oblique shunt at atrium level. Forward direction, low speed and continuous shunt in Doppler frequency was showed in most PFO cases. The size of PFO between infant group and child group without significant difference[(2.9±0.7)mm vs (2.6±1.0)mm, P>0.05]. The left to right shunt in 73 PFO cases, right to left shunt in 7 PFO cases, bidirectional shunt in 4 PFO cases and potential shunt in 2 PFO cases were diagnosed. Patients with right to left shunt or bidirectional shunt were those complicating with certain CHD or pulmonary artery hypertension. The detection rate of PFO in infant group and child group was 26.7% and 11.5% respectively. Simultaneously the detection rate of PFO in group A, group B, group C and group D was 33.3%, 22.7%, 18.3%, 8.1% respectively. Compared the detection rate of PFO in each groups: infant group exceed child group, group A exceed group C, group B exceed group D and group C exceed group D(P<0.01 or P<0.05). Conclusion: The detection rate of PFO in infancy is higher than childhood obviously, some kinds of CHD may delay the closure of PFO. Children with PFO often showed left to right shunt in conditions without certain CHD and pulmonary hypertension. TTE has high value in the diagnosis of PFO in children, contrast echocardiography can help to identify latent shunt of PFO.

Objective: To study the hemodynamic differences between distal abdominal aorta and common iliac artery caused by different degrees of ipsilateral common iliac arterial stenosis in swine. Methods: The lateral wall of common iliac artery was pinched with vascular clamp resulting in 1/4, 1/2, 3/4 stenosis in 6 juvenile domestic piglets. The parameters of PSV, MDV were obtained in all objects and analyzed with SPSS 13.0 statistical package. Results: PSV had significant negative correlation with stenotic degree, and MDV increased negatively with aggravated stenosis. The ipsilateral common iliac artery hemodynamics caused by common iliac artery stenosis changed more significant than the distal abdominal aorta. The backward needle ridge emerged in ipsilateral common iliac artery when stenosis reached a certain degree and increased with aggravated stenosis. Conclusions: The different position upstream hemodynamics caused by stenosis is different. The more striking the hemodynamics changes, the nearer to the position of stenosis. The backward needle ridge illustrates its position adjacent to the stenosis.

435 Analysis of bone metastases in lung cancer patients with 18F-FDG PET/CT

Objective: To investigate the distinction of 18F-FDG uptake between osteolytic and osteogenic bone metastases and the correlation between 18F-FDG uptake of bone metastases and different pathological types of lung cancer. Methods: 18F-FDG PET/CT scans were performed in 60 patients who were histologically proved as lung cancer at initial diagnosis with bone metastases. The SUVs of bone metastases were automatically measured by drawing the regions of interest(ROI). The osteolytic or osteogenic bone metastases were definited according to their densities dispalyed in integrated CT. Results: Three hundred and ninety-two bone metastases were confirmed by clinical follow-up, in which 389 bone metastases had intense 18F-FDG uptake. The other three lesions without intense 18F-FDG uptake had intense bone density change on CT. There were 201 lesions of osteolytic bone metastases, 75 of osteogenic bone metastases and 116 without bone density change. The SUV of them were 3.5±1.3, 2.7±0.6, 3.0±1.2, respectively. The SUVs of osteolytic bone metastases were higher than those of the latter two groups(P<0.01), while there were no statistical difference between the latter two groups(P>0.05). The SUVs of bone metastases of adenocarcinoma, squamous carcinoma, big and small cell carcinoma group were 3.4±1.3, 3.1±1.4, 3.3±1.1, 2.5±0.6, respectively. The SUV of small cell lung cancer group was lower than that of adenocarcinoma and big cell lung cancer groups, but no significant difference with squamous carcinoma. There were no significant differences of 18F-FDG uptake between the bone metastase of adenocarcinoma, squamous carcinoma, big and small cell carcinoma groups(P>0.05). Conclusions: The detection rate of bone metastasis of lung cancer in PET is higher than that in integrated CT. The osteolytic bone metastase has a higher FDG uptake than osteogenic ones. The role of PET and integrated CT can complement and confirm each other in the detection of bone metastastatic lesions of lung cancer.

439 Comparison between color Doppler sonography and DSA in detecting extracranial vertebral artery stenosis and hypoplasia

Objective: To compare the value of color Doppler sonography(CDS) and digital subtraction angiography(DSA) in the diagnosis of extracranial vertebral artery stenosis and hypoplasia. Methods: In the retrospective cohort analysis, 68 patients with extracranial vertebral artery stenosis or hypoplasia underwent CDS and DSA, and in correlation analysis of CDS and DSA, Kappa statistics was used. Results: Correlation of CDS and DSA in detecting extracranial vertebral artery stenosis had excellent consistency, but not in detecting extracranial vertebral artery hypoplasia. Conclusion: Although DSA was the gold standard, CDS detected the extracranial vertebral artery stenosis effectively in our analysis, so CDS should be suggested to be the preliminary screening examination for detecting extracranial vertebral artery stenosis.

442 Assessment of liver fibrosis with perfusion-weighted MR imaging: preliminary study

Objective: To characterize liver perfusion in Wistar rats with experimental liver fibrosis by dynamic contrast-enhanced MRI and to correlate it with the severity of liver fibrosis. To evaluate the diagnostic efficacy of perfusion MR imaging for liver fibrosis. Methods: Mixed solution of CCl4 and olive oil was hypodermically injected to fifty Wistar rats in the experimental group. Ten healthy rats served as control. MR perfusion study with dynamic contrast-enhanced MRI was carried out on the rats under anesthesia. The rats were divided into S0~S4 stages according to the histological results of liver. Time to peak(TP), wash-in rate and wash-out rate was compared between the groups using variance analysis, and the relationship between each perfusion parameters and severity of liver fibrosis were made by correlative analysis. Results: Forty-eight rats underwent MR perfusion study, including ten rats in the normal group. Histological results S0(n=13), S1(n=5), S2(n=9), S3(n=13), S4(n=8). On signal intensity time curve(SI-T) derived from MR perfusion weighted imaging(PWI), TP of portal vein and liver in fibrotic group[mean, (26.29±3.52)s and (41.92±5.76)s] increased progressively compared with control group[mean, (15.03±1.05)s and (25.93±1.70)s]. S2~S4 stage can be distinguished from S0 stage(P<0.05, <0.001, <0.001, respectively). Wash-in rate and wash-out rate of portal vein and liver in fibrotic group[mean, PV(292.61±40.02)1/s and (139.73±13.22)1/s, liver (76.70±9.55)1/s and (97.56±15.73)1/s] decreased progressively compared with control group[mean, PV(420.46±27.26)1/s and (193.50±16.76)1/s, liver(140.10±16.73)1/s and (121.00±15.16)1/s]. S3 and S4 stage can be distinguished from S0~S2 stage(P<0.05). TP value of the SI-T curve was significantly positive correlated to the severity of liver fibrosis(P<0.01). The wash-in rate and wash-out rate value were significantly negative correlated to the severity of liver fibrosis, respectively(P<0.01). Conclusion: MR-PWI has a potential in diagnosing and grading experimental liver fibrosis. PWI is an effective method to detect moderate and severe fibrosis(S2~S4 stage).

446 Correlative study between CT features and expression of MVD and Ki67 in renal chromophobe cell carcinoma

Objective: To study the correlation between CT features and expression of MVD and Ki67 in renal chromophobe cell carcinoma(RCCC). Methods: CT was performed preoperatively in 15 patients with RCCC by pathology. MVD and Ki67 expression were determined by immunohistochemical PV9000 method in all patients. The correlation between expression of determined proteins and CT features in RCCC were studied. Results: The tumor boundary is correlated with increased expression of Ki67, but not correlated with MVD. The positive expression of MVD and Ki67 in RCCC tissue is significantly higher than those of normal kidney tissue. The positive MVD and Ki67 were significantly correlated with the diameter of the tumor, the morphology of tumor, the degree and the inhomogeneity of enhancement. Conclusion: The enhanced CT scan is reliable for RCCC preoperative diagnosis. Part of CT signs is associated with the expression of MVD, Ki67. They can indirectly show the biological behaviour and malignant extent of RCCC.

451 CT and MRI findings of renal lymphoma

Objective: To investigate the CT and MRI findings of renal lymphoma. Methods: Imaging materials of 12 cases with renal lymphoma confirmed pathologically were analyzed retrospectively. Results: There were multiple lesions in 4 cases, solitary lesion in 2 cases, direct extension from retroperitoneal adenopathy in 3 cases, lymphoma of the perinephric space in 1 case, diffuse infiltration of kidney in 1 case and renal sinus involvement in 1 case. The lesions were ill-defined but homogeneous in 11 cases. The attenuation of the lesions were slightly higher than that of normal renal parenchyma at unenhanced CT in 12 cases. The lesions exhibited slightly hypointense on T1WI and hypointense on T2WI in 5 cases with MR imaging. The lesions showed less enhancement than that of normal renal parenchyma in 12 cases after intravenous administration of contrast material. The lesions demonstrated progressive enhancement in 10 cases and obviously enhancement during corticomedullary phase in 2 cases. Conclusion: Renal lymphoma has a wide variety of manifestations. The CT and MRI findings of renal lymphoma show some characteristic signs and play an important role in the diagnosis, staging and follow up of renal lymphoma.

455 Diagnostic value of CT urography in subtypes of renal cell carcinoma

Objective: To investigate the differential diagnostic value of common subtypes of renal cell carcinoma using CT urography(CTU). Methods: The images of CTU including characteristic manifestations of plain scanning and three-dimensional reconstruction in 150 cases of renal cell carcinoma proved by pathology were analyzed retrospectively and the CT value of different renal cell carcinoma in each period were compared. Results: There were 109 cases of clear cell renal cell carcinoma, 21 cases of papillary renal cell carcinoma, 8 cases chromophobe renal cell carcinoma, 9 cases of unclassified renal cell carcinoma and 3 cases of multilocular renal cell carcinoma. The above subtypes of renal cell carcinoma demonstrated characteristic features. Clear cell renal cell carcinoma exhibited inhomogeneous low density and hypervascular. Papillary renal cell carcinoma exhibited inhomogeneous high density and hypovascular. Chromophobe renal cell carcinoma was relatively homogeneous low density and hypovascular. Unclassified renal cell carcinoma is similar to clear cell renal cell carcinoma. Multilocular renal cell carcinoma presented as a multilocular cystic mass lacking an expansile nodule. The CT value of papillary is higher than clear cell renal cell carcinoma, chromophobe renal cell carcinoma and unclassified renal cell carcinoma(P<0.05) on plain scanning. The CT value of the latter three is indistinctive and the CT value of the above four subtypes is higher than multilocular renal cell carcinoma. There is no difference of clear cell renal cell carcinoma and uncalssified renal cell carcionma in CT value in arterial phase and venous phase(P>0.05), but CT value of them is higher than papillary renal cell carcinoma and chromophobe of which are indistinctive(P>0.05). The CT value of multilocular renal cell carcinoma is lower than the others(P<0.05) in arterial phase and venous phase. The CT value of multilocular renal cell carcinoma is lower than the others(P<0.05) of which are indistinctive(P>0.05) in delayed phase. Conclusion: Common subtypes of renal cell carcinoma demonstrated characteristic features in CTU and it is helpful for differentiation.
