733 The preliminary study of the relationship between epileptic focus identified bymagnetic source imaging and hypertensive signal on fluid attenuated inversion recovery

Objective: To evaluate the relationship between epileptic focus identified by magnetic source imaging(MSI) and hypertensive signal on fluid attenuated inversion recovery(FLAIR), so as to provide the basis for epilepsy surgery. Methods: MRI and MSI examination were performed in 20 patients with epilepsy and encephalomalacia. Compared the location of the epileptic focus determined by MSI with the location of the hypertensive signal on FLAIR. All 20 patients were operated on. Results: The location of the epileptic focus identified by MSI was all consistent with the location of the hypertensive signal on FLAIR in 12 cases; the epileptic focus identified by MSI was partially consistent with the hypertensive signal on FLAIR in 8 cases, 5 of the 8 patient’s extent of the epileptic focus defined by MSI were smaller than that of the hypertensive signal on FLAIR, the other three were larger than that of the hypertensive signal on FLAIR. Seizure free in 19 cases according to Engel classification of epilepsy. Conclusions: There are obvious correlation between the epileptic focus identified by MSI and the location of the hypertensive signal on FLAIR. The results of MSI might have many values to make the operative plan.

736 The imaging features of synovial chondromatosis and correlation with pathology

Objective: To explore imaging features of synovial chondromatosis correlated with pathology, and evaluate the value of different imaging examinations. Methods: The imaging data of 18 cases of pathologically confirmed synovial chondromatosis, who underwent the preoperative plain film, CT and MR plain-scan and enhanced-scan were analysed retrospectively. Amonge these cases, 11 males, 7 females, 30~78 years old, average age was 41.5 years old. One case was elbow joint, one case was shoulder joint, five cases were hip joint, eight cases were knee joint, two cases were ankle joint, one case was temporomandibular joint. Results: On X-ray, typical feature of synovial chondromatosis was free bodies in the joint space and soft tissue, which showed the “concentric lamina” sign, particularly in the cases of Milgram Ⅱ and Ⅲ stage. CT images showed much more free bodies and thickened hyperdense synovium. MR images clearly showed thickened synovium, free bodies, joint effusion and joint degenerative lesions, free bodies displayed two kinds of appearance, cartilaginous body and bony body with ring enhancement after using contrast material. For the Milgram Ⅰ stage cases, CT and MR showed the thickening synovium without positive features on plain film. CT and MR were superior to X-ray in showing the number and position of free body and synovial lesions especially on showing the cartilaginous free bodies. Conclusion: Synovial chondromatosis have characteristic radiological features with different imaging examinations, particularly CT and MR images provide much more information about free body and synovial lesions, are superior in the diagnosis of Milgram Ⅰ stage cases.

740 The value of MRI in the diagnosis of desmoid tumor

Objective: To summarize MRI manifestation of desmoid tumor and to investigate the fundamental histo-pathology. MRI finding of desmoid tumor including location, shape, margin, signal intensity on T1WI and T2WI, homogeneity of signal, and enhancement were analyzed retrospectively in this study. Methods: Eight cases of desmoid tumor proved by surgery were enrolled. MRI manifestations were comparatively studied with histopathologic findings. T1 and T2 sequence were used in non-contrast scan, while STIR sequence were performed in 4 cases and enhanced scan in 7 cases. Results: ①Six cases were located in haunch, 1 case was in right chest wall and 1 case in the soft tissue around left femoral bone; ②Desmoid tumor’s size was various, the shape of 7 cases was irregular, 1 case was elliptical; ③The margin in six cases was indistinct with soft tissue around the tumor. The margin in 1 case was well defined; ④Compare to the signal intensity of muscle, 6 cases were isointense and 2 cases were slightly hypointese on T1WI. On T2WI 4 cases were hyperintense and 4 cases were slightly hyperintense but all of their signal intensity was lower than that of fat and the interior was inhomogeneous. Four cases which used STIR appeared hyperintense. Two cases were homogeneously enhanced and 5 cases were inhomogeneously enhanced. Conclusion: There are certain regularities of desmoid tumor on MRI, and these are helpful in determining the nature of the tumor.

743 Radiological analysis of pulmonary sarcomatoid carcinoma correlated with clinical and pathological findings

Objective: To investigate the value of radiography in the diagnosis of pulmonary sarcomatoid carcinoma(PSC) correlated with the clinical and pathological findings. Methods: Eight patients with PSC proven by surgery and pathology, underwent radiographic examination(X-ray and CT). The images were reviewed retrospectively and correlated with pathological manifestation. Results: In 8 cases, 6 cases had single mass(4 in right lung, 2 in left lung), and 2 had more than one mass. Seven patients underwent CT examination. The pre-contrast CT films of 5 cases exhibited large globular masses, and 3 showed irregular node. After administration of contrast medium, most of them had a heterogeneous enhancement. The CT films also showed other features: cavity in 1(with fungus), spiculation sign in 1, enlargement of lymph node of the hilum and mediastinum in 3, pleural fluid in 2, pleural nodes in 3, pulmonary atelectasis in 1, obstructive pneumonia in 2, metastasis out of the lung in 2, metastasis of the lung in 2 and rib destruction in 1. Microscopically, 1 was spindle cell carcinoma, other 7 cases were composed of spindle cell and epithelial cell. Immunohistochemically, CK was positive(++~+++) in 7 cases, and Vimentin(++~+++) in 6 cases, CEA(+++) in 1 case. Conclusion: Although PSC has no specific clinical features, it had both lung cancer and sarcomatoid features in most of the radiological signs. Finally, it is dependent on pathologic examination to establish the diagnosis.

746 An experimental study of 18F-FDG PET/CT multi-time point imaging for differentiating tumor from inflammation

Objective: To investigate SUVmax and Glut1 for the differentiation of tumor from inflammation using 18F-FDG multi-time point imaging obtained by PET/CT scanner in animal models. Materials and Methods: Tumor group rats were subcutanceously injected wk256 into right oxter, inflammation group rats were intramuscularly injected turpentine into left hind leg. Each animal model was scanned at 20, 40, 60, 80, 100, 120min after FDG injection, SUVmax was measured. HE and immunohistochemistry were analysed for lesions. Results: FDG accumulation of the two groups increased with time, they had a significant difference at 100min(t=2.59, P<0.05); with the early image acquired at 40min, calculated IR40(SUVmax at every time point divided by the SUVmax at 40min), they had a significant difference from 80min(t=3.45, P<0.01). The Glut1 was significantly higher in the tumor(12.67±3.25) than inflammation(7.18±1.52)(P<0.05); The SUVmax of 80min and RI in tumors had positive correlation with Glut1(r1=0.69, P<0.05; r2=0.86, P<0.001). The SUVmax of 80min in inflammation had positive correlation with Glut1(r=0.61, P<0.05), RI had no correlation with Glut1(r=0.57, P>0.05). Conclusion: The higher Glut1 expression level in the tumor may partially explain the higher FDG accumulation in the tumor than inflammation. FDG of two groups had positive correlation with Glut1, RI was better than SUVmax for differentiating lesions. The proper delayed imaging could shorten the overall testing time rather than single-time-point imaging.

750 Application of MR perfusion weighted imaging in the diagnosis of uterine cervix cancer

Objective: To explore the characteristics of time-signal intensity curve(TIC) and the differences in perfusion parameter of uterine cervix cancer and normal uterine cervix. Methods: MRI routine scanning and perfusion imaging were performed in 43 patients with uterine cervix cancer and 23 patients without uterine cervix cancer. Data of MRI perfusion imaging in ROI were derivatized into perfusion parameter by Functool software package, and drew into revised TIC by Excel 2003, the differences of TIC and perfusion parameter in both groups were analysed. Results: In 43 patients with uterine cervix cancer and 23 patients without uterine cervix cancer, the shape of TIC and revised TIC of each group were similar, they showed an obvious steep up slope, a long plateau after peak value. Compared to normal uterine cervix group, the upslope of revised TIC of uterine cervix cancer groups were steeper; among MRI perfusion parameters, the median of the PH value had no difference between the normal cervix group and uterine cervix cancer group(P=0.0687), PEI and MSI in obvious enhancement areas of uterine cervix cancer group were higher than those of normal cervix group(P=0.0010, 0.0013, respectively), but TTP were shorter(P=0.0250). Conclusion: TIC can directly describe the difference in blood perfusion between uterine cervix cancer and normal uterine cervix overall, and perfusion parameters can accurately reflect the hemodynamics characteristics of uterine cervix cancer.

755 Clinical implications of transient left ventricular ischemic dilation of coronary artery disease in men

Objective: To study the clinical significance of left ventricular transient ischemic dilation(TID) with extensive myocardial ischemic coronary artery disease in men. Methods: Fifty male patients with extensive myocardial ischemic coronary artery disease from 272 outpatients and inpatients, and 33 without myocardial ischemic coronary artery disease were enrolled into this study with the latter as the control group. SPECT scintigraphy imaging of stress/rest myocardial perfusion were completed after iv. injection of 740MBq 99Tcm-MIBI. The degree of coronary artery disease and ischemia were graded with SSS and SDS. TID was calculated with software in-synchronism. Results: TID in male patients with extensive myocardial ischemia and those without ischemia was 1.13±0.13 and 1.01±0.10 respectively(t=4.214, P<0.01). There was a significant difference. Area under the ROC curve was 0.759, the best criterion was 1.02, sensitivity was 78%, specificity was 63.6%. TID>1.18 had maximum LR+(P<0.01, Area=0.5). Conclusion: The results suggest that TID assessed using the stress/rest sestamibi SPECT is a useful index for separating extensive myocardial ischemia from those without ischemia in men.

758 An fMRI study for different cognition components of working memory in schizophrenia patients

Objective: To investigate the difference of neural mechanism for different cognition components with encoding, maintenance and retrieval process of working memory(WM) between schizophrenia patients and normal subjects using event-related fMRI(ER-fMRI). Methods: Sixteen normal subjects and twenty schizophrenia subjects completed a Sternberg item recognition task using fMRI. Brain activation regions for encoding, maintenance and retrieval process were obtained. Result: Compared to normal subjects, some functional regions associated with encoding, maintenance and retrieval process showed increased activation in schizophrenia patients, i.e. right precuneus for encoding process, left PMA, left DLPFC, right precuneus and left VLPFC for maintaince process, left PMA for retrieval process. In addition, subcortical structures, primary motor cortex and some verbal regions in left temporal lobe also showed more activation. Conclusion: Some functional regions associated with encoding, maintenance and retrieval process showed relatively inefficiency in schizophrenia patients.
