837 Preliminary clinical study in stroke patients with hemispatial neglect

Objective: To explore the occurrence and clinical characters, location in image, and mechanism of hemispatial neglect(HSN), HSN test and SPECT rCBF imaging were performed on patients with stroke. Methods: Thirty-four dextromanual patients who were diagnosed as unilateral stroke clinically were recruited. A HSN test including line bisection test, star cancellation test and drawing test was performed on the subjects. The severity of neglect and rCBF, the range, number of the foci, the flow deficit size and the total numbers of pixels in the foci were measured on HSN test and SPECT rCBF imaging. Results: Thirty patients were diagnosed as HSN by the HSN test battery. Contralateral neglect(CN) and ipsilateral neglect(IN) happened on both the right and left hemispheres. On SPECT imagings, the patients with neglect had decreased rCBF in the frontal cortex mostly, parietal, occipital, temporal cortex secondly, and basal ganglia and thalamus the last. The patients who had two or more regions damaged showed neglect more easily and severe. The most significant region was temporal-parietal-occipital junction. The correlation coefficients between rCBF, the decreased percentage of rCBF and the severity of neglect were -0.175(P>0.05) and 0.266(P>0.05). The correlation coefficients between the range, number of foci, the flow deficit size, the total number of pixels of the foci and the severity of neglect were 0.543(P<0.01), 0.462(P<0.05), 0.429(P<0.05), 0.437(P<0.05), respectively. No difference reached significant between CN and IN on SPECT imaging. Conclusions: The severity of neglect correlates insignificantly with rCBF and the decrease percentage of rCBF, while it correlates positively with the range, number of foci, the flow deficit size and the total numbers of pixels of the foci. And CN and IN showed no difference on the occurrence, severity of neglect and the rCBF, the decreased percentage of rCBF, the range, and number of foci, the flow deficit size and the total numbers of pixels in the foci. We suggest that HSN is the damage of multiple sites, combine damage of which results in more severe neglect.

842 Differential diagnosis between high grade gliomas and solitary metastases with post-contrast multivoxel 1H-MRS

Objective: To discuss post-contrast multivoxel 1H-MRS can be used to differentiate high grade gliomas and solitary metastases on the basis of the differences in metabolite levels in enhancing region and peritumoral region. Materials and Methods: Twenty-seven patients with brain tumor[12 high-grade gliomas(WHO III~IV) and 15 solitary metastases] was performed with conventional MRI and post-contrast multivoxel 1H-MRS. Ten age-matched healthy volunteers underwent post-contrast 1H-MRS for comparison. The Cho/Cr, NAA/Cho, NAA/Cr-n value of enhancing region and peritumoral region were compared respectively between gliomas and metastases. The results were analyzed statistically. Results: All the enhancing regions of the two kinds of tumor were characterized by reduced NAA/Cho, NAA/Cr-n and increased Cho/Cr, which had no significant difference(P>0.05) except NAA/Cho(P<0.05). The Cho/Cr in peritumoral region of the gliomas and metastases was 1.642±0.24 and 0.862±0.41 respectively; the NAA/Cho was 0.462±0.32 and 2.512±1.58 respectively; the NAA/Cr-n was 0.436±0.22 and 0.725±0.27 respectively. All ratios had significant difference(P<0.01). Conclusion: Combining with conventional MRI scan, post-contrast multivoxel 1H-MRS can improve the differential diagnostic ability between high grade gliomas and solitary metastases.

846 Evaluation of early efficacy of tumor radiation therapy using NIRF imaging

Objective: In order to investigate whether near-infrared fluorescence(NIRF) labeled annexin V could also demonstrate apoptotic cells in small animal model. Methods: Cy5.5-annexin V, a novel NIRF contrast agent was synthesized and characterized. Its binding ability to target apoptotic cells was demonstrated in vitro. MCa-29 tumor cells were inoculated in the hind legs of mice to set up the animal models. The mice were divided into radiation and non-radiation groups. Cy5.5-annexin V was injected into mouse intravenously 6 hours after radiation. The imaging was performed 24 hours after injection. Results: The binding affinity, Kd, to phosphatidylserine(PS) was 2.51nM for the Cy5.5-annexin V and 1.54nM for the unmodified annexin V respectively. The results of flow cytometry and confocal fluorescence microscopy showed Cy5.5-annexin V targeted non-viable mouse thymocytes in similar patterns to FITC-annexin V. Fluorescence images of mice with MCa-29 tumor cells demonstrated that the fluorescence intensity in the radiated tumors was higher than that in untreated tumors. Conclusion: Cy5.5-annexin V could be useful in the early evaluation of radiation effect of the tumor.

850 The clinical study of regional T2* value changes in Alzheimer’s disease brain

Objective: To explore T2* values and R2* values in several cerebral regions in patients with Alzheimer’s disease(AD), and to evaluate the correlation between T2* and R2* values in AD patients and clinical mental scale scores. Methods: Twenty-six AD patients and 20 healthy control subjects were performed using a multi-echo fast field echo sequence at a 3.0T MRI scanner. The T2* values were read from region of interest(ROI) drawn at several cerebral regions on the T2* maps. And the R2* values were calculated. The difference of T2* and R2* values in cerebral structures between AD patients and control subjects was compared. And the correlation between T2* and R2* values in AD patients and mini-mental state examination(MMSE) score was tested. Results: Compared with control subjects, the T2* values of the hippocampus, globus pallidus and temporal cortex in patients with AD were lower, the R2* values in these regions in AD patients were significantly increased. The T2* values of the hippocampus, temporal cortex and globus pallidus in patients with AD correlated positively with the MMSE score(r=0.48, 0.56, 0.44 respectively, all P<0.05). The correlation coefficients between the R2* values of these structures in AD patients and MMSE score were negative(r=-0.49, -0.58, -0.43 respectively, all P<0.05). Conclusions: The T2* and R2* values of cerebral regions in AD were significantly abnormal and were correlated with clinical mental scale scores, which suggested the measurements of T2* and R2* might be helpful in the diagnosis and longitudinal monitoring for AD.

855 Correlation study between Cho/Cr value of MRS and Ki-67 labeling index in meningiomas

Objective: To investigate the correlation between Cho/Cr value of 1H magnetic resonance spectroscopy(1H-MRS) and the Ki-67 labeling index in meningiomas. Methods: Thirty-six patients with meningioma underwent conventional MR and 1H-MRS before operation(Grade I 30 cases; Grade II+III 6 cases). 1H-MRS sequences included PRESS 35, PRESS 144. Expression of Ki-67 nuclear antigen were detected by immunohistochemical method of streptavidin-biotin-peroxidase(SP). Results: The mean Cho/Cr ratio in the benign and malignant groups were 3.80±1.08 and 7.22±0.70, respectively(t=7.40, P=0.000). Ki-67 in the benign and malignant groups were 6.43±1.09 and 12.15±4.14, respectively. A significant linear correlation was observed between the Cho/Cr ratio and the expression of Ki-67. The MRS features of meningiomas were as follow: Cho increased markedly, Cr decreased evidently. Conclusion: 1H-MRS is a useful diagnostic method for determining the proliferation or malignant potential of meningiomas according to the Cho/Cr ratio and the latter is correlated to the Ki-67 value.

862 Medical treatment observation of unrupture tubal pregnancy with TVCDS

Objective: To explore the clinical value of the transvaginal color Doppler sonography(TVCDS) in conservative treatment of unruptured tubal pregnancy. Method: Fifty women who suffered from unruptured tubal pregnancy were selected to be treated by MTX(1mg/kg) and examined by TVCDS before and after treatment to monitor the alteration of the mass in fallopian tube. Results: Among 6~12 weeks the mass in fallopian tube reduced in 43 women(86%), tubal abortion in 2 women(4%) and inefficacy in 5 women(10%). Conclusion: TVCDS can help to diagnose tubal pregnancy earlier and monitor the alteration of fallopian tube during medical conservative treatment, and guide the further clinical treatment.

865 A correlative study of MSCT enhancement presentation with histo-differentiation and p53 expression in gastric cancer

Objective: To investigate the relationship of enhanced MSCT signs of gastric carcinoma to its histological differentiation and expression of p53. Materials and Methods: Total 66 patients of gastric carcinoma were routinely studied preoperatively with dual-phase enhanced CT scan. Routine hematoxylin and immunohistochemical staining of the surgical tumor specimens of all patients was visualized for the histological differentiation and the expression of p53 respectively. The correlation between enhanced MSCT signs of gastric carcinoma and its histological differentiation and expression of p53 were analyzed. Results: The correlation was found between enhanced characteristics of gastric carcinoma and its histological differentiation and expression of p53. Homogenous enhancement showed high/moderate differentiation of adenocarcinoma in 73.1%(19/26). Layered or inhomogenous enhancement showed poorly differentiated adenocarcinoma in 75%(30/40), the difference had statistically significance(P=0.000). Dual-phase enhanced rate had no correlation with histological differentiation of gastric carcinoma(P>0.05). The positive expression of p53 in layered or inhomogenous enhanced characteristics of gastric carcinoma was significantly different from that in homogenous enhanced characteristics(P<0.05). The positive expression of p53 was relative to arterial-phased enhanced rate and tumor size and lymph node metastasis of gastric cancer(P<0.05). Conclusion: Tumor different enhancement and dual-phase enhanced rate reflects various histological differentiation and the positive expression of p53 in gastric carcinoma, which suggests that enhanced MSCT signs of gastric carcinoma are closely correlative to its histological differentiation and expression of p53.

Objective: To explore the characteristics of the transplanted kidneys’ morphology, blood flow perfusion when rejection occurred by 2D and 3D ultrasound. The volume and the vascularization index(VI) were analyzed quantitatively. Methods: Thirty transplanted kidneys with rejection and thirty normal transplanted kidneys were examined by 2D and 3D ultrasound. 3D ultrasound imaging was reconstructed off-line. The length, width and height were measured on 2D imaging. The volume and VI were calculated by 3D imaging. Results: The volume of transplanted kidneys in rejection group is significantly different from that of control group(P<0.05). It is more sensitive than the length, the width and the height. There is significant difference of VI between the rejection and the control group(P<0.05). Conclusion: When rejection occurred, the volume of the transplanted kidneys obtained by 3D is more sensitive than the size obtained by 2D. VI can describe the decrease of transplanted kidneys’ blood flow perfusion.

872 99mTc-MIBI scintigraphy and molybdenum mammography in the diagnosis of breast cancer

Objective: To compare the accurate rate of 99mTc-MIBI scintigraphy and molybdenum mammography of the breast and find a more correct method in the diagnosis of breast cancer. Methods: One hundred and thirty-five female patients with breast lesion were enrolled in the study. All patients received molybdenum mammography and 99mTc-MIBI scintigraphy examinations. Compared the result of the two methods with pathologic result after operation. Results: Compared with pathologic results, the sensitivity, specificity and accurate rate of 99mTc-MIBI scintigraphy and molybdenum target mammography are 93.3%, 90.3%, 92.6% and 83.7%, 93.5%, 85.9%. Conclusions: There was no significant difference between 99mTc-MIBI scintigraphy and molybdenum mammography, but combination application of the two methods can enhance the diagnostic accurate rate.

876 Diagnostic value of 3D color Doppler energy ultrasound in bone tumors

Objective: To study the diagnostic value of 3D color Doppler energy(3D-CDE) in differentiating malignant from benign bone tumors. Methods: To compare the results of 3D-CDE with pathological findings in 56 patients with bone tumors. Results: The pathological findings showed that 27 cases were malignant and 29 cases were benign. The 3D-CDE features of benign cases were no blood flow within most of the lesion 82.8%(24/29) while in most malignant cases these were rich blood flow within the lesion 77.8%(21/27), in which irregularly circuitous blood vessels could be seen. Conclusion: 3D-CDE is valuable in differentiating malignant bone tumors from benign ones and estimating the blood flow of tumors.

879 The influence of needle-pleural angle in pneumothorax rate of lung biopsy

Objective: By retrospectively analysis the complications of 828 cases CT-guided transthorax needle biopsies(TNB), to study the potential factors that may influence the incidence rate of pneumothorax, especially needle-pleural angle. Materials and Methods: In 828 patients, 1113 coaxial TNB were performed under CT-guidance. The possible factors of patients’ characteristics, lesions and procedure variables were calculated. The minimal angle between needle and pleural was measured in CT imaging during the procedure. The variables were correlated with incidence of pneumothorax. Results: There were 109(13.16%) cases of pneumothorax in the study, 25(5%) biopsies treated with chest tube placement, and 13 cases with pleural punctural aspiration. Needle-pleural angles(<75°) and dwelling time were associated with higher pneumothorax rate. The size of lesions(<30mm), location(close to the base of lung and the diaphragm), puncture site(in the lateral or oblique chest wall) and the needle path length(>40mm), were associated with higher pneumothorax rates significantly. The pleural contact of lesions, pleural thickening and pleural or thorax invasion of lesions were associated with lower pneumothorax rate. Conclusion: A small puncture needle pleural angle and lung dwelling time may increase the pneumothorax rate significantly.

883 Semiquantitative evaluation of in-stent restenosis in coronary arteries with MDCT

Objective: To evaluate the clinical value of multidetector computed tomographic angiography(MDCTA) in the judgement of in-stent restenosis(ISR) after coronary stenting. Materials and Methods: Thirty-four patients who had been performed coronary stenting with 65 stents were included in the study. All the patients were examined by 64-row MDCTA and invasive coronary angiography for evaluating ISR after coronary stenting. Results: These findings resulted in a sensitivity of 82.4%, specificity of 97.9%, false positive rate of 2.1%, false negative rate of 17.6%, coincidence rate of 93.8%, positive predictive value of 93.3%, and negative predictive value of 94.0%, respectively. Conclusion: MDCTA seems to be an effective imaging modality in identifying ISR and can evaluate ISR semiquantitatively.
