609 Comprehensive analysis of the high density of middle cerebral artery sign in hyperacute cerebral infarction

Objective: To explore objective criteria which are suitable for clinical application by comprehensive analysis of unilateral hyperdense middle cerebral artery sign(HMCAS) on non-enhanced computed tomography(NECT). Methods: One hundred sixteen patients with hyperacute cerebral infarction were retrospectively evaluated. The presence of HMCAS was evaluated between inter- and intra-observers. K value was calculated. The cut off value of MCA attenuation and MCA ratio(the affected side/the healthy side) in the HMCAS(+) and HMCAS(-) was measured through the ROC curve in all patients with hyperacute cerebral infarction. The sensitivity and specificity was analyzed. Results: The interobserver and intraobserver agreement were substantial(K value of 0.82 and 0.86, respectively) and increased to almost perfect(K value of 0.95) when the reader was provided with clinical information. MCA attenuation≥40 HU and MCA ratio≥1.2 was the best threshold for HMCAS recognition. When both MCA attenuation≥40 HU and MCA ratio≥1.2 were satisfied, sensitivity(91.23%) and specificity(100%) were the highest relative to other standards. Conclusion: It is effective and feasible to evaluate the unilateral HMCAS on NECT images by combining with clinical history. MCA attenuation≥40 HU and MCA ratio≥1.2 was the best threshold for HMCAS recognition, which is suitable for application in daily clinical practice.

612 A study of SWE for the diagnosis of infants congenital muscular torticollis and its application in assessing the no-surgical treatment effect

Objective: The diagnostic value of real-time shear wave elastography(SWE) to infants congenital muscular torticollis(CMT) and the clinical application of SWE in assessing the no-surgical treatment effect of children’s CMT were discussed. Methods: One hundred and two cases of suspected children with CMT(outpatient in our hospital from Jan. 2015 to June. 2017) were collected. All of them were examined by ultrasound and SWE was used to collect the elastic modulus data of sternocleidomastoid muscle on the affected side and the healthy side. After that, the children patients’ therapeutic process was followed up. The no-surgical treatment mainly includes massage and ultrasonic short wave physiotherapy. When a follow-up to three months was fulfilled, ultrasound reexamination was performed and the elastic modulus data of the children patients’ sternocleidomastoid muscle was recollected. Results: In the 102 cases of children patients, 76 cases were diagnosed with CMT. The area under ROC curve of diagnosing CMT by SWE was 0.844, and the diagnostic threshold was 182 kPa, and the sensitivity was 84.2%, and the specificity was 73.1%, and the positive predictive value was 90.1%, and the negative predictive value was 61.2%, and the Youden index was 0.573. To the children patients with CMT, a comparison of the elastic modulus data between sternocleidomastoid muscle on the affected side and the healthy side was performed, and the difference is statistically significant(P<0.05). Through the reexamination after no-surgical conservative treatment, the average elastic modulus value of sternocleidomastoid muscle on the affected side was significantly reduced, and the difference is statistically significant(P<0.05). Conclusion: Real-time SWE yields theoretical foundations to the diagnosis and the treatment effect evaluation of infants CMT. It embraces a certain value of popularization and application.

616 The differential expression of magnetic resonance imaging between male breast cancer and inflammatory granuloma

Objective: To investigate the differential diagnosis of MR imaging findings of male breast cancer and inflammatory granuloma. Methods: Confirmed by operation and pathology, 4 cases of male breast cancer and 6 cases of granulomatous mastitis were underwent dynamic contrast-enhanced MR imaging and DWI. Results: The location of the lesions in the two groups were all behind the nipple, and all showed mass lesion. 2 cases of breast cancer were morphologically oval, irregular in 2 cases; clear edge in 2 cases and irregular shape in 2 cases. Internal inhomogeneous enhancement in 2 cases and edge enhancement in 4 cases. 2 cases of skin were involved. The TIC curve showed 3 cases of type Ⅲ, 1 cases of type Ⅱ. The average ADC value was of 0.897×10-3 mm2/s. Inflammatory granuloma were morphologically oval in 2 cases and irregular in 4 cases. There was irregular edge and spiculated in 2 cases. Internal inhomogeneous enhancement in 2 cases and edge enhancement in 4 cases. The TIC curve were all of type Ⅱ. The average ADC value of 0.901×10-3 mm2/s. The edge of the mass were enhanced highly, edge enhancement is higher than that of the center(3 cases) or uneven enhancement(1 cases). There was no enhancement zone (4 cases) in the inflammatory granuloma group. There was no obvious enhancement in the area where high intensity on T2WI. Centrifugal enhanced and the edge of the lesion gradually blurred in the dynamic images(2 cases). Conclusion: For the diagnosis of male breast cancer and inflammatory granuloma lesions, internal enhancement and enhancement curves play an important role in the differential diagnosis of disease in two groups. Breast cancer lesions were of inhomogeneous enhancement or obvious enhancement. Edge enhancement is higher than that of center curve and with TIC curve type Ⅲ. No enhancement area could be observed in granuloma group lesions. Centrifugal enhancement and the edge of lesions gradually blurred could be observed on dynamic image. Enhancement curve were type Ⅱ.

620 Pulmonary mucosa-associated lymphoid tissue lymphoma: CT and PET/CT imaging findings and follow-up

Objective: To investigate and improve the CT and PET/CT findings of pulmonary mucosa-associated lymphoid tissue(MALT) lymphoma. Methods: The findings and pathologic data of 27 patients with pathologically confirmed pulmonary MALT lymphoma were retrospectively analyzed on the CT and PET/CT image. Of all cases, 3 cases were with silicosis, Sjogren’s syndrome and tuberculosis, and 4 cases invaded the lung. Twenty four cases had chest CT non enhanced and enhanced scan, 2 cases were of CT scan, 10 cases were underwent PET/CT and CT examination, 1 case was only with PET/CT scan. Results: Twenty four cases with multiple lesions and three cases with solitary lesion were founded in all pulmonary MALT lymphoma cases(twenty seven cases). Twenty seven cases of CT were manifested as pulmonary consolidation, along the subpleural or bronchial vascular bundle and segmental distribution, which crossed the lobes in 16 cases, not in 11 cases, of which 24 cases(24/27, 89%) had expansion of inflamed bronchus, the edge of the lesion with the frosted glass or nodules; while the rest of the lungs associated with multiple ground glass or nodules along bronchial vascular bundles in 18 cases. Lesion density was uniformity, and 4 cases had a little calcification. Mild degree enhancement were in 7 cases, and moderate degree enhancement was in 17 cases, 18 cases were with angiographic signs. Hilar and mediastinal lymphadenopathy was in 4 cases. Pleural effusion was in 7 cases, pleural thickening was in 5 cases. The SUV of 11 cases were 2.5±14.4, with an average of 5.6±2.7. Mediastinal or hilar lymphadenectasis were concentrated was in 7 cases. The SUV ranged from 1.5 to 6.6, among which 3 cases were treated with lacrimal gland, liver or femur radioactivity, SUV ranged from 3.4 to 6.8. Twenty one cases were followed up, stable or complete remission was in 18 cases, progress was in 3 cases. CD20 and CD79a were all positive, CyclinD1 was all negative, Ki-67 positive expression was more than 20%. Conclusion: Pulmonary MALT lymphoma usually appears as multiple nodules and consolidations with air bronchogram and uniform density on CT, and heterogeneous high FDG uptake on PET/CT, and the final diagnosis depends on pathology.

624 CT findings of intrapulmonary lymph nodes and its differential diagnosis with inferior pleural pulmonary nodules

Objective: To explore the CT findings of intrapulmonary lymph nodes(IPLNs) and compared with the benign and malignant pulmonary nodules inferior pleural, so as to improve the diagnosis and identification of IPLNs. Methods: A total of 38 patients with 49 IPLNs were enrolled in the study. 75 patients with 80 malignant pulmonary nodules were confirmed by operation and pathology. Twenty-eight patients with 33 benign pulmonary nodules were examined by operation or clinically. All cases were scanned by MSCT and 1 mm thin slice post-processing images were analyzed. Compared the CT findings among three groups of pulmonary nodules, and using SPSS 17.0 statistical software to analysis. P<0.05 for the difference was statistically significant(P<0.0167 for the difference was statistically significant between two groups). Results: There was significant difference in gender among the three groups(P<0.05), the incidence was significantly higher in the malignant group(66.7%); IPLNs were located below the tracheal cartilage level, 71.4% were located the lower lobe(P<0.05). The average diameter of IPLNs was (0.66±0.23)cm lower than that of benign and malignant nodules group(P<0.001), and the CT value was (32.9±66.1)HU higher than malignant nodules group ((-275.0±255.9)HU) and benign nodules group((-47.6±177.0)HU), the difference was statistically significant(P<0.001). The round shape of IPLNs were more common than benign and malignant nodules group(P<0.05), and the 98% of IPLNs were clear and smooth higher than benign and malignant nodules group(P<0.05); In IPLNs, lobulated sign, spicule sign, pleural depression syndrome, and vascular bundle sign were less than malignant nodules group(P<0.001) but the sign of narrow line(44.9%) was significantly higher than that of benign and malignant nodules group(P<0.001).

629 Experimental study of Ultrafast pulse wave velocity technology in quantitative evaluation of#br# abdominal aortic elasticity in atherosclerotic rabbit models

Objective: The purpose of this study was to explore the value of Ultrafast pulse wave velocity(UFPWV) technology in quantitative evaluation of arterial elasticity. Methods: Forty-five male New Zealand white rabbits were divided into two groups, a group of 35 rabbits fed up with HFD to establish animal models of atherosclerosis and another group of 10 rabbits fed normal diet as the control group. UFPWV was used at the beginning and 24th week. Rabbits were sacrificed for pathological examination at 24th week. Rabbits with plaques were divided into plaque group, and the ones without were divided into non-plaque group. Parameters of the HFD group and the normal control group were compared, and self contrast was made. Meanwhile the parameters in the non-plaque group, the plaque group and the normal control group were compared at 24th week. The ROC curve was plotted to evaluate the efficacy of UFPWV technology. Result: Both PWVBs and PWVEs of the HFD group were significantly higher than the control group(P<0.05), and both the parameters of the plaque group were higher than the normal control group and the non-plaque group(P<0.05). There was also a increase in the non-plaque group compared with the control group(P<0.05). The area under the ROC curve of the parameters in diagnosing atherosclerosis were 0.873 and 0.892, respectively for PWVBs and PWVEs. Conclusion: Arterial elasticity might decrease paralleled with the increase of arterial stiffness, and there’s a decrease of arterial elasticity in the early stage of atherosclerosis before the morphological changes which could be successfully detected by the effective technology called UFPWV.

633 Evaluation of remodeling of femoral artery and its correlation with neutrophil to lymphocyte ratio in#br# patients of type 2 diabetes mellitus by high-frequency ultrasonography

Objective: To evaluate remodeling of the femoral artery and its correlation with neutrophil to lymphocyte ratio(NLR) in patients of type 2 diabetes mellitus(T2DM) by high-frequency ultrasound. Methods: Eighty-eight patients with T2DM were divided into group B(48 cases, the course≤10 years) and group C(40 cases, the course>10 years). 36 healthy volunteers were recruited as the control group(group A). The intimal medial thickness(IMT), total vessel area(TVA), lumen area(LA) were measured and vascular remodeling types were determined. NLR was calculated and obtained from the peripheral blood routine examination. Results: IMT and NLR of the femoral in group B and C were higher, has significant difference. And IMT and NLR of the femoral in group C were higher than that in group B(all P<0.05). RI in group B was higher than that of group A (P<0.05). RI in group C was lower than that of group B(P<0.05). The vascular remodeling rates in group B and C were higher than that in group A(all P<0.05), and in group C was higher than that in group B(P<0.05). The positive remodeling rate of group B was higher than that of other two groups, and the difference were significant. The rate of negative remodeling in group C was higher than that of group A and B(all P<0.05). Vessels without remodeling was predominant in group A. The rate of negative remodeling was higher than the rate of positive remodeling in group C. The rate of positive remodeling was higher than the rate of negative remodeling in group B. IMT was positively correlative with NLR(r=0.56, P<0.05). Conclusion: The atherosclerosis, vascular remodeling of the femoral artery and the relationship with NLR in the patients of T2DM can be evaluated conveniently and accurately by high-frequency ultrasonography with an important clinical value.

636 Evaluation of liver steatosis and fibrosis in patients of type 2 diabetes mellitus and correlation between#br# liver steatosis and fibrosis by FibroTouch technique

Objective: To evaluate the extent of liver steatosis and fibrosis in patients of type 2 diabetes mellitus(T2DM) and correlation between liver steatosis and fibrosis by FibroTouch technique. Methods: Ninety-two cases of T2DM patients were enrolled and divided into group A(46 cases, diabetic duration>10 years) and group B(46 cases, diabetic duration≤10 years). Forty-six healthy subjects were enrolled as controls(group C). FibroTouch technique was used to measure the liver fat attenuation parameter(FAP) and liver stiffness measurement(LSM). Results: The FAP and LSM of group A and group B were significantly larger than those of group C(all of P<0.05); the FAP of group A was smaller than that of group B and the LSM of group A was larger than that of group B(all P<0.05). The degree of liver steatosis increased in group A and group B compared with that of group C(all of P<0.05), and the degree of liver steatosis decreased in group A compared with that of group B(P<0.05); The degree of liver stiffness increased in group A and group B compared with that of group C (all of P<0.05), and the degree of liver stiffness increased in group A compared with that of group B(P<0.05). There was a negative correlation between liver fibrosis and steatosis(r=-0.81, P<0.05). Conclusion: The fibrosis and steatosis of the liver in T2DM patients can be evaluated non-invasively, conveniently and objectively by FibroTouch technique, which has an important clinical value.

640 Feasibility study of dual-source CT dual-energy quantitative parameters evaluation of colon Crohn's disease activity

Objective: To investigate the feasibility of dual-source dual-energy CT quantitative evaluation of Crohn’s disease activity. Methods: Thirty-eight patients with Crohn’s disease(CD) diagnosed by clinical manifestations, endoscopy, radiology and histopathology underwent CT enterography dual-energy double-phase contrast-enhanced radiography. According to the colonoscopy localization of Montreal, the bowel activity of the lesion was divided into three groups according to the Simple Endoscopic Score for Crohn’s Disease(SES-CD): mild activity, moderate activity and severe activity period. The differences of CT value, iodine value, normalized CT value and normalized iodine concentration(NIC) in intestine between different phases of arterial phase and portal venous phase were compared. According to the receiver operating characteristic(ROC) curve Diagnostic efficiency. Results: There were 53 lesions in 38 cases, including 7 segments of mild, 26 of moderate and 20 of severe. There were significant differences among the four parameters in the stages of intestine arterial phase and portal venous phase in different severity of disease and normal group(P<0.05), and increased with the increase of the severity. Except for the arterial phase CT value, the double-phase diagnosis of severe CD parameters are better performance, and portal phase were better than the arterial phase, portal phase NIC diagnostic efficacy of the highest threshold 0.59 mg/mL, sensitivity, specificity respectively, 80%, 84.8%, AUC=0.858. Conclusion: Quantitative parameters of dual-energy CT have great clinical value in assessing the severity of CD wall in CD patients, which can provide a basis for clinical diagnosis of CD activity.

645 Imaging diagnosis research of aggressive angiomyxoma

Objective: To study imaging characteristics of aggressive angiomyxoma(AAM). Methods: Ten patients(F∶M=7∶3) with preoperative imaging examinations confirmed as AAM by surgery and pathology were collected from 2012 to 2016 in our hospital and their clinical and imaging data were retrospectively analyzed. Results: In 10 cases of patients, 6 lesions occurred in the perineum and pelvis, three of which grew across the diaphragm and other incidence sites were vulva, right thigh, retroperitoneal and ileocecal junction. The maximum diameter of 9 cases of lesions was vertical diameter. In 7 cases of patients with enhanced CT examination, 3 cases showed mild enhancement and 4 cases showed significant enhancement. In 3 cases, laminated enhancement was showed. Calcification was seen in 1 case. In 6 cases of patients with MRI examination(5 cases with enhancement), 5 lesions showed high signal on T2WI. 4 cases showed significant enhancement, and laminated enhancement was seen in 3 cases. Conclusion: AAM had some clinical and imaging characteristics. For female patients of reproductive age, tumors located in perineum or crossing the pelvic diaphragm, with high signal intensity on T2WI and laminated appearance highly suggest the diagnosis of AAM.

649 The diagnostic value of prenatal ultrasound for fetal brain malformations during 11~14 weeks of gestation

Objective: To investigate the prenatal ultrasound of the diagnosis of value of the clinical application of fetal brain malformations at 11~14 weeks of gestation. Methods: Seventy-one cases of brain malformation fetus as the research object from January 2012 to May 2017 in Southwest Hospital, and ultrasonographic characteristics, clinical data and follow-up results of fetuses with brain malformations were retrospectively analyzed. Results: Among 63 cases of brain malformations diagnosed by ultrasound, 17 cases were anencephaly, 21 cases were encephalomegaly, 8 cases were meningocele and 17 cases were whole forebrain. Eight cases of brain malformations were not detected by ultrasonography during 11~14 weeks of pregnancy, including 6 cases of hydrocephalus, 1 cases of complete forebrain deformities, and 1 cases of meningoele. Among 63 cases of 11~14 gestational weeks, 25 cases(39.7%) were complicated with other system or organ abnormalities, of which 17 cases were complicated with multiple malformations. Conclusion: Prenatal ultrasound has a high diagnostic value for fetal brain abnormalities at 11~14 gestational weeks, by providing important evidence for early clinical intervention.

652 99mTc-MDP SPECT/CT fusion in diagnosis of painful accessory navicular bone

Objective: To investigate the imaging features and diagnostic value of 99mTc-MDP SPECT/CT fusion imaging in painful accessory navicular bone. Methods: 99mTc-SPECT/CT imaging data and clinical data of 32 patients(11 males, 21 females) with confirmed painful accessory navicular bone were retrospectively analyzed. Diagnostic sensitivity, specificity and accuracy of the two different investigations(CT and SPECT/CT imaging) were calculated. Different methods of diagnosis were compared with ?字2 test. Results: Thirty-two patients with 64 feet have 45 case accessory navicular bone, and have the 35 case painful accessory navicular bone. For diagnosis of painful accessory navicular bone, CT has a sensitivity of 40%(14/35), specificity of 81.25%(13/16), positive predictive value of 82.35%(14/17), and negative predictive value of 38.24%(13/34). SPECT/CT has a sensitivity of 100%(35/35), specificity of 81.25%(13/16), positive predictive value of 92.10%(35/38), negative predictive value of 100%(13/13). 99mTc-MDP SPECT/CT has more accurate than CT imaging(P<0.05). Conclusion: 99mTc-MDP SPECT/CT fusion imaging can be used to diagnose painful accessory navicular bone, and has clinical application value.

655 Comparison of whole-body PET/CT and PET/MR in clinical diagnosis of malignant tumors

Objective: The aim of this study was to demonstrate stability and accuracy in tumor semiquantitative assessment of the integrated PET/MR by analyzing difference and correlation between PET/CT SUVmax and PET/MR SUVmax.  Methods: Fifty-seven patients(22 males, 35 females, average age (54±12) years), who underwent same day whole-body PET/CT and PET/MR in Shengjing Hospital Affiliated to China Medical University, the time interval between the two scans was (78±35) minutes. Patients underwent the whole-body PET/CT and PET/MR from the base of skull to upper thighs, TOF-PET technique of GE company was used to reconstruct images and SUVmax was obtained by fusion images to delineate ROI of lesions and some normal tissues. The difference analysis of SUVmax were performed with Paired t test and the correlation was analyzed with Spearman correlation analysis. Results: 97 hypermetabolism lesions were detected by PET/CT, 100 hypermetabolism lesions were detected by PET/MR, a total of 96 hypermetabolism lesions were detected by two modalities. The PET/MR SUVmax of lesions was significantly higher than that of PET/CT SUVmax(P<0.0001) in the case of PET/CT and following PET/MR. The PET/MR SUVmax of most normal tissues significantly lower than that of PET/CT(P<0.0001), only the PET/MR SUVmax of psoas was slightly higher than that of PET/CT, but there was no statistical difference(P=0.056). The ratios of PET/MR SUVmax of the lesion and the normal tissues were significantly higher than PET/CT(P<0.0001). There were significant correlations of SUVmax between PET/MR and PET/CT for lesions(r=0.92, P<0.0001) and for normal tissues(lung r=0.64;liver r=0.87; spleen r=0.73; psoas r=0.57, P<0.0001). Conclusions: The SUVmax of PET/MR in tumor was significantly higher than that of PET/CT, which may be due to PET/MR technical advantages and delayed glucose uptake by malignant lesions. PET/MR and PET/CT SUVmax highly correlated, suggesting that integrated PET/MR on semiquantitative analysis of malignant tumor lesions has a very good stability and accuracy.

660 Application of lower extremity CT angiography with low tube voltage and iterative model reconstruction

Objective: To investigate the application value of low tube voltage in combination with iterative model reconstruction(IMR) in lower extremity CT angiography(CTA). Methods: Forty-four patients were randomly divided into experimental group(n=22) and control group(n=22). All the patients underwent CTA with automatic tube current modulation. The patients in experimental group(group A) were scanned with 80 kVp tube voltage, 75 mL of contrast agent, flow rate at 3 mL/s and IMR. The patients in control group(group B) were scanned with 120 kVp, 100 mL of contrast agent, flow rate at 4 mL/s, and filtered back projection(FBP). CT values of vessel were measured on bilateral lower extremity arteries of the axial images, including aortic bifurcation, common femoral artery, middle superficial femoral,proximal popliteal artery and tibiofibular trunk artery. The standard deviation(SD) of CT value of vessel lumen was defined as image noise. The SNR and CNR were calculated. The subjective image quality was scored. The results of CT volume dose index(CTDIvol) and dose length product(DLP) were recorded. Results: The CT values、SNR and CNR were higher in group A than group B(P<0.01). There was no difference in subjective image quality between the two groups(P>0.05). The CTDIvol and DLP of group A were lower than that of group B(P<0.01). Compared with group B, the iodine load decreased by 25%. Conclusions: Lower extremity CTA with low tube voltage and IMR can provide good image quality for clinical diagnosis with a significant reduction in radiation dose and the iodine load.

As a noninvasive imaging technique, coronary CT angiography(CCTA) plays an important increasingly role in the follow-up of coronary stenting. Although CT imaging of coronary stent is still limited by beam hardening artifact, partial volume effect etc, CCTA of coronary stenting has been significantly improved with the continuous updating of CT devices and the gradual improvement of reconstruction algorithms. At the same time, CCTA combined with functional techniques provides different functional indicators, adding the functional level based on the anatomical level of assessment, so as to provides comprehensive informationa of morphology and metabolism for clinical. In this paper, imaging research progress of coronary artery stent evaluationin CCTA were reviewed.
