MRI findings of ganglioglioma with correlation of clinical and pathological analysis

Objective: To analyze MRI features of gangliogliomas with correlations of clinical and pathological findings. Methods: MR imaging, clinical and pathological features of gangliogliomas in 20 cases were analyzed retrospectively. Results: All cases were solitary. Eighteen lesions were supratentorial, 2 lesions were subtentorial. Amomg them, 8 lesions located in the frontal lobe, 7 lesions in temporal lobe, 1 lesion in the parietal lobe, occipital-parietal lobe, frontal-temperal lobe cerebella and medulla-cervical cord in each. Seventeen lesions were detected preoperatively MRI, 3 recurrence tumors detected postoperatively. The MR imaging appearance of gangliogliomas is variable. Gangliogliomas may be divided into solid tumors and cystic tumors on the basis of their MR characteristics. On enhanced MRI, cystic tumors may be nonenhancing or appear as a cystic lesion with an enhancing mural nodule, the focal or diffuse enhancement can be seen in the solid tumors. No or mild perilesional edema were seen for most tumors. The activation detected by EEG-fMRI was concordant with pathological focus. Conclusion: The MRI of gangliogliomas has some characteristic imaging features. It is helpful for the evaluation of gangliogliomas preoperatively combining with EEG-fMRI.

3.0T 1H-MRS and MRI volumetry analysis in the hippocampus of children with focal temporal lobe epilepsy

Objective: To study the diagnostic value of quantitative proton magnetic resonance spectroscopy(1H-MRS) and volumetric MR(vMR) in the hippocampus of children on focal temporal lobe epilepsy (fTLE), and analyse the relationship between the volume change of the hippocampus and clinical factors. Methods: 3.0T 1H-MRS and vMRI were used to study 30 children(mean age 8.5 years, age 5 to 14 years) with unilateral fTLE, proven by 24 hours active electroencephalogram, and 30 random healthy children(mean age 8.8 years, age 5 to 14 years) served as controls. The spectral resonance lines of N-acetylaspartate (NAA)/creatine(Cr) and volume of the hippocampus were calculated quantitatively on both hippocampal regions of children patients and either side of control group, and then compared among the lesion group, the contralateral group and control group. Explore the relationship between the hippocampus shrink and the seizure frequency etc. Results: The mean concentration of NAA/Cr was significantly decreased in the focus side(1.47±0.24) compared with the nonfocus side(1.79±0.35) and the controls(1.81±0.27), but nonsignificantly between the nonfocus side and controls. Moreover, the mean hippocampal volume of focus side of patients[(2.75±0.35)cm3] was significantly reduced compared with that of nonfocus side[(3.06±0.46)cm3] and the controls[(3.11±0.17)cm3], reference ranges of NAA/Cr and hippocampal volume in control group as standard. Furthermore, seizure frequency has no relation with hippocampus volume shrink in fTLE children. The degree of hippocampus volume reduction was not related with the seizure frequency, but the history of febrile convulsions had significant influence on the hippocampus. Conclusions: 3.0T 1H-MRS and vMRI could be useful for localization in the region of hippocampal lesions, and quantify biochemical metabolic disorders and morphological changes. They could be a complementary modality to provide the reliable information for diagnosis and treatment in fTLE children.

Correlation between cystic necrosis of cranio-hemangiopericytoma and the expression of P73

Objective: To investigate the correlation between cystic necrosis and P73 protein expression in cerebral hemangiopericytoma. Methods: Twenty-nine cases with intracranial hemangiopericytoma were chosen, 25 cases of angiomatous type meningioma were also enrolled as control group. All cystic or necrotic signs on MRI of the two tumors were analyzed, which were scored according to Asari’s method. P73 protein expressions were detected by S-P immunohistochemical method. Results: Twenty-nine cases showed positive expression, and there was significant difference in the expression of P73 between intracranial hemangiopericytoma and angiomatous type meningioma(P<0.01). The positive expression of P73 protein increased with increasing of cystic or necrotic signs on MRI of intracranial hemangiopericytoma(P<0.05). Conclusions: Cystic or necrotic sign on MRI was closely related to the high expression of P73 protein in intracranial hemangiopericytoma, which can been used as reference information evaluating the treatment and prognosis of intracranial hemangiopericytoma.

The diagnostic value of three-dimensional ultrasonography in single thyroid nodule and the analysis of ROC curve

Objective: To evaluate the diagnostic value of three-dimensional ultrasonography in single thyroid nodule. Methods: 128 patients with single thyroid nodule were examined with three-dimensional ultrasonography to assess the characters of gray-scale and blood vessel morphology and distribution of thyroid nodules in three-dimensional ultrasonography. The results were compared with post-operative pathological results. The diagnostic value of three-dimensional ultrasonography was evaluated using receiver operating characteristic(ROC) curve. Results: Three-dimensional ultrasonography showed significant difference in the characters of gray-scale; vascular morphology and distribution of inner and surrounding vessels in malignant thyroid nodules in comparison to benign lessions(P<0.01). Using characteristics of three-dimensional gray-scale ultrasound as criteria of malignant and benign thyroid nodules, the area under ROC curve was 0.9218, diagnostic performances was better. Using surrounding blood vessels of thyroid nodules as criteria, the area under ROC curve was 0.8131, diagnostic performances was moderate. Using the whole character of gray-scale and blood vessel morphology and distribution in three-dimensional ultrasound as criteria, the area under ROC curve was 0.9608, diagnostic performance was the best. Conclusions: Three-dimensional ultrasound imaging can evaluate the characters of gray-scale and blood vessel morphology and distribution of thyroid nodules, and improve the capability in differentiating between malignant and benign thyroid nodules.

Edge based differentiation between the malignant and the benign breast tumor in ultrasound images

Objective: Extracting the edge of breast tumor in ultrasonic images and distinguishing malignant from benign lesions. Method: This paper proposes an improved anisotropic diffusion filtering method which chooses diffusion equation with different diffusion coefficient according to the gradient of the image, and this method can smooth the image as well as finely preserve the boundary. Then presenting a new variational formulation in Level Set method which can avoid the step of re-initialization and make the formation of the initial Level Set function flexible, and applying a semiautomatic segmentation procedure with the rough contour is sketched first. This procedure improves not only the accuracy, but also the efficiency of the segmentation algorithm. Result: Computing the texture features of breast tumor by integrated GLCM and distinguishing the malignant from the benign by fuzzy C-means method, and the accuracy rate is 72.64%. Conclusion: The experiments show that the proposed algorithms could extract the contour of the breast tumor effectively and accurately, providing reliable foundation for differentiation of the malignant and the benign breast tumor.

Clinical value of color Doppler flow imaging and left ventricular angiography in the diagnosis ofVSD with aneurysms before interventional occlusion

Objective: To explore the diagnostic value of color Doppler flow imaging(CDFI) and left ventricular angiography in the diagnosis of ventricular septal defect(VSD) with aneurysms. Methods: Twenty-seven cases of VSD with aneurysms were confirmed by CDFI and left ventricular angiography. Before occlusion the size and the type of membranous aneurysm, distance from VSD to the aortic valve or tricuspid valve were measured. The diameter of occluder waist was measured after occlusion. Results: The detection rate of membranous aneurysm was 100% via left ventricular angiography and 92.6% via CDFI. The detection rate of the types and the size of membranous aneurysm via CDFI and that via left ventricular angiography had significant difference(P<0.05). The two methods in the measurement of the orifice of VSD had no significant difference but in the measurement of the distance from VSD upper margin to the ring of aortic valve had significant difference(P<0.05). The diameter of occluder waist was significantly correlated with the size measured by CDFI or angiography. Conclusion: There was a positive correlation between CDFI and left ventricular angiography in estimating membranous aneurysm before occlusion of VSD. They possess very important value in the transcatheter closure of VSD with membranous aneurysm.

CT and MRI diagnosis of primary vertebral chondrosarcoma

Objective: To analyze the CT and MRI findings of the primary verbebral chondrosarcoma and correlate with pathology in order to improve the imaging diagnosis. Material and Methods: The clinical and imaging materials of 11 cases with primary chondrosarcoma in spine were analyzed retrospectively. Two radiologists evaluated the localization, erosion, tumor-morphology, calcification, enhancement and characteristics of tumor. Results: The location of tumor included cervical spine(n=1), thoracic spine(n=7), sacral vertebra(n=3). Four cases revealed osteolytic destruction and lesions were originated in vertebral body(n=4), in appendix and vertebra(n=5), in costovertebral joints(n=2). All the tumors with scallop-like soft tissue mass. There were 8 cases with mineralization in tumors. Involvement of adjacent vertebral levels was seen in 4 cases. Conclusion: The CT and MRI findings of the primary chondrosarcoma in spine are similar to the chondrosarcoma happened in other position. Calcification is better delineated by CT, which is helpful to determine the nature of the tumor. MRI is better in delineating the soft tissue neoplasm and vertebral canal encroachment, which is helpful to clinical therapy.

The diagnostic value of MRI in atypical elbow injury in children

Objective: To approach the diagnostic value and characteristic appearance of MRI in atypical elbow injury in children. Methods: To analyze MRI appearance of 41 children who were suspected of having atypical elbow injury. All patients performed lateral and postero-antarior position of the elbow X-ray films and 3 cases performed CT scan. Results: In 41 cases, 31(75.6%) were diagnosed as atypical children elbow injury by MR. Among patients who had no fracture on X-ray or CT examination, epiphyseal injury was found in 7 cases, 3 cases had articular cartilage fracture; bone contusion in 18 cases; none had tendon injury. On the X-ray or CT image, 6 cases had a clear diagnosis of fracture but can not show articular surface involvement. MR confirmed the diagnosis of X-ray or CT image, and 3 patients of them had atypical fracture. Three cases had simple fracture without atypical injury, conservative treatment was accepted by gypsum fixation; 5 cases only had simple soft tissue injury or joint effusion, no treatment was needed. Four cases had two fractures at the same time. Two cases had suspicious fracture on X-ray but MR explicitly excluded bone injury and only had soft tissue swelling. Conclusion: MRI is the first choice for examination of children’s elbow atypical injury and can clearly show the location, nature and extent of children’s elbow atypical injury which is very helpful to the diagnosis.

MR imaging of the synovium changes in knee joints with degenerative osteoarthrosis

Objective: To study the MR imaging of the synovium changes in knee joints with degenerative osteoarthrosis. Methods: The MRI findings were retrospectively analyzed in 20 patients with degenerative osteoarthrosis in knee joints confirmed by operation finding and arthroscopy. The synovium changes were mainly studied. Results: Edema of synovium was found in 20 cases, hydrarthrosis in 20 cases, proliferation of synovium in 13 cases, ossification of synovium in 1 case, synovial joint body in 1 case. Conclusion: The synovium changes were important manifestations in the degenerative osteoarthrosis of the knee joints and it could be clearly demonstrated by MR examination and is of important value in management of degenerative osteoarthrosis in knee joints.

Functional anatomy of the anterior cruciate ligament in vivo with MRI

Objective: To research the functional anatomic change of anterior cruciate ligament(ACL) in vivo. Methods: A total 40 knees from 30 healthy subjects, with mean age of 23 years, were studied to measure the length of the anteromedial bundle(AMB) and the posterolateral bundle(PLB) and to observe the change of the tension and the elevation angles and the architecture of AMB and PLB on oblique coronal and sagittal images at different flexion of the knees(0°, 30°, 60°, 90°) with MRI 3-dimensional multi echo data imaging combination(3D-MEDIC) sequence. Results: When the knee is extended, PLB is tight and longer and AMB is moderately lax and shorter. As the knee is flexed to 60°~90°, AMB is to tighten and longer and PLB is loosen up and shorter, meanwhile ACL displayed two types of architecture: parallel type(22 cases) and twist type(18 cases). The elevation angles are 38.5°±4.9° in AMB and 48.3°±6.9° in PLB as the knee is flexed to 0°. Conclusion: It is feasible to make 3D-MEDIC of MRI to study the functional anatomy of ACL in vivo.
